Testicular biopsy is an important tool in the diagnosis and management of male infertility. Other types of analysis such as plasma hormonal assay, chromosomal studies and biochemical determination of ejaculate components have also a role in characterizing male infertility. There were 53 infertile males who received testicular biopsy in our department during the period from January, 1972 to June, 1982. A clinical investigation was undertaken in aspect of testicular size, past history, testicular biopsy findings and plasma hormonal level. The following results were obtained: 1. On semen analysis, oligospermia was found in 5 cases, azoospermia in 48 cases and the most common age group was 4th decade. 2. The biopsy findings of 41 cases with normal size testis were normal in 20 cases. hypospermatogenesis in 8 cases, peritubular fibrosis with reduced spermatogenesis in 4 cases. maturation arrest in 4 cases germinal aplasia in 4 cases, hyalinized seminiferous tubule with Leydig cell hyperplasia in 1 case. 3. The biopsy findings of 12 cases with small size testis were hypospermatogenesis in 3 cases. hyalinized seminiferous tubule with Leydig cell hyperplasia in 6 cases, peritubular fibrosis with reduced spermatogenesis in 1 case, maturation arrest in 1 case, germinal aplasia in 1 case. 4. There were 20 cases which had induration on epididymis or vas deferens. On their past history, gonorrheal urethritis in 7 cases, nonspecific epididymitis in 4 cases, tuberculous epididymitis in 3 cases, undetermined cause in 6 cases were observed. 5. The biopsy findings of 20 cases with indurated epididymis or vas deferens were normal in 13 cases, peritubularfibrosis with reduced spermatogenesis in 4 cases, hypospermatogenesis in 3 cases. 6. The plasma hormonal assay (FSH, LH, Testosterone) was performed in 8 cases.

A clinical review of 31 cases of multiple myeloma which were diagnosed by criteria of the SWOG between May 1983 and February 1990 at Yeungnam University Hospital was done. The results were as followings: 1. The peak incidence was in 7th decade and male to female ratio was 1.8:1. 2. The most common presenting symptom at first diagnosis was bone pain (58%), but fever, dyspnea, dizziness and palpable mass were also noted. 3. The distribution of laboratory findings as following diagnostic criteria of Southwest oncology group (SWOG): Plasmacytoma on tissue biopsy was noted 6 cases, bone marrow plasmacytosis with more than 10% plasma cells was 22 cases, monoclonal globulin spike on serum electrophoresis was 24 cases, lytic bone lesions was observed 22 cases. 4. Initial clinical stages were classified as 2 cases in stage I, 3 cases in stage II, 26 cases in stage III (84%). 5. Immunoelectrophoresis revealed the distribution of IgG 64%, light chain 22%, IgA 10%, Kappa to Lambda ratio of 1.1:1. 6. Hematologic & biochemical findings revealed anemia with <8.5% of hemoglobin in 42%, hypercalcemia with <10.6 mg% of serum calcium in 22%, azotemia >2.0 mg% of serum creatinine in 19%. 7. The multiple punched out lesion of bone x-ray examination were noticed skull (65%), rib (42%), L-spine (35%), pelvis (23%), T-spine (19%). The initial skeletal roentgenographic findings showed osteoporosis, osteolytic lesion and fracture in 55%, only osteolytic lesion in 23%, only osteoporosis in 10%. 8. Complications of multiple myeloma, such as 10 cases of renal impairment, 8 cases of infection, 16 cases of compression fracture of spine were observed.

Neisseria meningitidis is an important cause of sporadic meningitis and fatal sepsis. Skin manifestations like urticaria, maculopapular eruption, and petechial rash occur in the majority of patients with meningococcal infections. However, meningococcal cellulitis is extremely rare and has not been reported in patients with systemic lupus erythematosus (SLE). SLE patients who have low complement level are more susceptible to meningococcal infections than healthy population with normal complement level. We report a case of cellulitis associated with meningococcal infection, which manifested as knee arthritis, skin erythema, and low grade fever mimiking SLE flare in a 54 year-old female patient who had long standing SLE and hypocomplementemia.
