1.Predictive Value of Urinary Cytology in the Recurrence and the Progression of Superficial Bladder Cancer.

A retrospective analysis was done on 68 patients with primary superficial bladder cancer between September 1989 and December 1995. We evaluated the predictive value of urine cytology in the recurrence and the progression of superficial bladder cancer. Positive cytology was shown in 52.9% (36/68) and was significantly associated with tumor grade (p=0.001). The recurrence rate in patients with negative cytology was 28.1% (9/32) compared to 77.8% (28/36) in those with positive cytology (p=0;001). No patients of negative cytology had tumor progression while 4 out of 36 (11.1%) patients of positive cytology had progression to invasive or metastatic disease. In conclusion, urinary cytology appears to be a significant prognostic factor in superficial bladder cancer.

2.A Cases of Familial Spastic Paraplegia (FSP).

We present a family of FSP with review of literatures. Seeligmuller (1876) and Strumpell (1880) were the first to describe familial case of spastic paraplegia characterized by progressive weakness and spasticity of the lower limbs with little or no involvement of the upper extremities. This syndrome is heterogenous in inheritance, age of onset, severity and associated signs. A pure type without complications has been named "Strumpell's FSP". We have recently experienced a case of 17 years old male who had bilateral weakness and stiffness of lower extremities, and gait disturbance with family history. Differential diagnosis, particularly with familial amyotrophic lateral sclerosis, is discussed.

3.The Effect of Gonadotropin Releasing Hormone Analogue and Growth Hormone on Final Height in Children with Advanced Puberty.

PURPOSE: Gonadotropin releasing hormone analogue (GnRHa) or growth hormone (GH) improve final height in girls with central precocious puberty. We studied the effect of these agents on adult height in children with advanced puberty. METHODS: We analysed height, bone age, growth velocity, predicted adult height (PAH), and final adult height (FAH) in 61 girls and 19 boys with advanced puberty, who were treated with GnRHa combined GH or GH. RESULTS: In Girls 1) FAH (SDS) of combination group (GnRHa+GH, n=7) was similar to their pretreatment PAH (SDS) [153.9+/-6.0 cm (-1.3+/-1.2) vs 152.8+/-4.7 cm (-1.5+/-0.9)]. In GH group (n=18), FAH was significantly increased [155.7+/-4.9 cm (-0.9+/-1.0) vs 149.9+/-4.6 cm (-2.1+/-0.9)] (P<0.001). 2) PAH (SDS) of combination group increased from 151.5+/-5.9 cm (-1.8+/-1.2) to 157.8+/-7.1 cm (-0.5+/-1.4) and that of GH group increased from 149.5+/-5.9 cm (-2.2+/-1.2) to 155.8+/-5.8 cm (-0.9+/-1.2) (P<0.001). During first year of treatment, growth velocity of GH group was significantly higher than that of combination group (6.6+/-2.1 cm/year vs 9.4+/-2.5 cm/year, P=0.001) In boys 1) In both group (7 boys of combination group and 8 boys of GH group), FAH was similar to their pretreatment PAH and their growth velocity during first year of treatment had no significant difference (7.6+/-2.3 cm/year vs 9.2+/-2.9 cm/year). CONCLUSION: In girls with advanced puberty, GnRHa delayed bone maturation but had no significant effect on FAH. In contrast, GH increased FAH through increment of growth velociy. In boys with advanced puberty, no significant effect of GnRHa or GH.

4.A Case of Primary Malignant Leptomeningeal Melanomatosis.

Primary malignant melaomatosis of leptomeninges is uncommon, not reported in the Korea to this time. Primary melanomatosis of leptomeninges are not associated with any specific clinical or radiological symptoms. The diagnosis was made by ultrastructural demonstration of malignant melanoma cells from CSF. The patient is 23 years male, who has had meningeal irritating symptoms such as intermittent headache. Nausea, vomiting and disturbed consciousness. Brain CT and MRI showed no mass lesion but cytology of cerebrospinal fluid revealed malignant melanoma cells. There was no evidence of extracerebral melanoma.

5.The associations of health behaviors and working hours with high-sensitivity C-reactive protein levels in Korean wage workers: a cross-sectional study

Objectives:
We investigated differences in high-sensitivity C-reactive protein (hs-CRP) levels by age group according to working hours, socioeconomic level, health behavior and status, and occupational class, and aimed to identify factors affecting hs-CRP levels in various age groups using data from the Korean National Health and Nutrition Examination from 2016 to 2018.
Methods:
The study included a total of 4,786 male wage workers across the nation, aged between 19 and 65. Data from 4,674 workers were analyzed using multiple logistic regression analysis.
Results:
Obesity, metabolic syndrome, and weekly working hours were associated with hs-CRP, a biomarker of inflammation. Participants with a body mass index (BMI) ≥25.0 kg/m2 showed significantly higher hs-CRP levels than those with a BMI 23.0 to 25.0 kg/m2 . Workers with highrisk drinking and metabolic syndrome showed significantly higher hs-CRP levels in the 50 to 65 years group. Obesity, walking 0 to 149 min/wk, and working ≥61 hours a week were associated with significantly higher hs-CRP levels in the 35 to 49 years group. The factors that significantly affected hs-CRP levels were different among age groups.
Conclusion
Plans to adjust working hours should be considered health behaviors, such as drinking and physical activity, and health conditions, such as metabolic syndrome and obesity, according to workers’ age.
6.The value of CDC42 effector protein 2 as a novel prognostic biomarker in liver hepatocellular carcinoma: a comprehensive data analysis

Objectives:
The prognostic significance of CDC42 effector protein 2 (CDC42EP2) and its association with tumor-infiltrating immune cells (TIICs) have not been explored in liver hepatocellular carcinoma (LIHC). This study aims to assess the potential prognostic value of CDC42EP2 by conducting a comprehensive analysis of online databases pertaining to LIHC.
Methods:
We evaluated the potential of CDC42EP2 as a prognostic biomarker by utilizing online databases such as TIMER, GEPIA2, KM, OSlihc, HPA, and LinkedOmics.
Results:
In LIHC, we observed that the mRNA and protein expression of CDC42EP2 were upregulated compared to normal tissues. Upregulated CDC42EP2 expression was associated with a worse prognosis based on the clinicopathological characteristics of patients with LIHC. Furthermore, CDC42EP2 was positively associated with TIICs. In the co-expression and functional enrichment analyses of CDC42EP2, 11,416 genes showed positive associations with CDC42EP2 while 8,008 genes showed negative associations. CDC42EP2-related co-expression genes were involved in protein localization to the endoplasmic reticulum, translational initiation, and RNA catabolic processes in gene set enrichment analysis-Gene Ontology (GSEA-GO), and regulated the ribosome, spliceosome, and primary immune deficiency in the GSEA-Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway. In a survival map, 23 and 17 genes that exhibited positive associations with CDC42EP2 showed a significant hazard ratio (HR) for overall survival and disease-free survival, respectively.
Conclusion
Our findings demonstrated that CDC42EP2 is a novel prognostic biomarker and a potential tumor immune therapeutic target in patients with LIHC.
7.Morphological Difference of Polyethylene Section in Knee Arthroplasty.

The purpose of this study is to see the value of transsectional morphology of polyethylene for evaluation of wear and characteristics of ultra-high molecular weight polyethylene. Retrieved nineteen polyeth- ylene tibial components were examined for this study. Ten components were the directly molded 1900 Himont polyethylene, and 9 components were the machined GUR 415 polyethylene. Transsection of polyethylene(thickness: 60 - 150 micron) was obtained by microtome and was examined microscopical- ly for the fusion defects, subsurface white bands, and delamination as a mode of wear. Significant differ- ences between two groups of polyethylene were found. Fusion defects greater than 100 micron were observed in 5 among 9 GUR 415 polyethylene. Subsurface white band were observed in 7 among 9 GUR 415 polyethylene. Delamination was found in 6 arnong 9 GUR 415 polyethylene. Neither fusion defects greater than 100 micron, nor delamination were observed in 1900 Himont polyethylene. Morphological examination of polyethylene section is an efficient way to differentiate the polyethylene and to evaluate the wear behavior of retrieved polyethylene. This study suggests that delamination of polyethylene tibial components is influenced by resin type and / or manufacturing method.

8.Multifocal Acute Osteomyelitis in a Mole Adolescent: A Case Report.

A 13-year-old boy developed fever and right leg pain, and multiple acupunctures were done on legs, then visited our ER for high fever, chill and painful swellings in both legs. Bone scan showed multiple hot spots on both distal femur, both proximal tibia, and left distal tibia. MRI revealed subperiosteal abscess in right proximal and left distal tibia and signal changes without abscess in other hot spot sites on bone scan. Surgical drainage of subperiosteal abscess and curettage were performed in the right proximal and the left distal tibia. Percutaneous drillings were done to other hot spot sites. All the lesions were cured after combined antibiotic chemotherapy. Multifocal osteomyelitis in a rapidly growing adolescent has been reported not rarely. It seemed that more attentions should be paid to the possibility of multifocal acute osteomyelitis in an adolescent

9.Cranioplasty Using Pericranial flap and Split-Rib Graft: Case Report.

PURPOSE: The goals of cranioplasty are to protect the brain from trauma and restore normal cranial contour with as few complication as possible. In patient who suffered from cranial defect, the most satisfactory form of cranioplasty may be archived with autogenous bone grafts. We report on the treatment of large frontal bone defect using pericranial flap and split-rib graft. METHODS: A 29-year-old male was referred to our department. He was involved in an automobile accident resulting in large frontal bone and sinus defect and skull basal defect. The reconstruction was undertaken using galeal frontalis myofascial flap and split-rib cranioplasty using bicoronal incision. RESULTS: The postoperative course was successful. For 9 months follow up period, there was no complication about hematoma, infection, CSF leakage and washboard deformity. He was pleased with the results even when absolute symmetry was not achieved. CONCLUSION: We experienced large frontal bone and sinus defect and skull basal defect patient and successfully treated with split-rib graft and galeal frontalis myofascial flap.

10.Hemoptysis: Comparison of High-resolution CT with Fiberoptic Bronchoscopy.

PURPOSE: To compare the precise roles of high-resolution computed tomography (HRCT) and fiberoptic bronchoscopy (FOB) in the evaluation of patients presenting with hemoptysis and to determine the optimal timing for HRCT. MATERIALS AND METHODS: The results of HRCT and FOB were compared in 23 patients (15 men, 8 women) presenting with hemoptysis. Etiologies included bronchietasis (n=4), parenchymal pulmonary tuberculosis (n=4), lung cancer (n=4), endobronchial tuberculosis (n=2), and broncholithiasis (n=2). Hemoptysis was proved to be due to miscellaneous causes in an additional three cases and to be cryptogenic in four. The diagnostic results of FOB performed before and after HRCT were compared as were those of HRCT performed within and after the first 48 hours of active bleeding. RESULTS: FOB and HRCT offered a correct diagnosis in 39% and 65% of cases, respectively (p=0.005). HRCT demonstrated three cases of bronchiectasis and three of parenchymal pulmonary tuberculosis which were beyond the range of a bronchoscope. In two of five cases in which HRCT findings were nonspecific, chondromatous hamartoma and lung cancer were confirmed by FOB. In cases where HRCT was performed prior to FOB, the latter demonstrated the location and diagnosis in 82% and 47% of cases, respectively (p=0.303) ; when HRCT was performed after FOB, HRCT was correct in 67% and 17% of cases, respectively (p=0.178). In none of three cases (0%) in which HRCT was performed during the first 48 hours of active bleeding did the procedure allow a specific diagnosis. In 15 of 20 (75%) cases in which HRCT was performed after the first 48 hours, however, the diagnosis provided by CT was correct. CONCLUSION: The results of this study suggest that in patients presenting with hemoptysis, both HRCT and FOB should be used for evaluation, since they are diagnostically complementary. FOB is more useful for the diagnosis of endobronchial lesion, and HRCT for bronchiectasis and parenchymal pulmonary tuberculosis. If, in cases of hemoptysis, initial diagnosis is attempted within the first 48 hours of active bleeding, FOB should be the initial step, and HRCT images should not be obtained until active bleeding has been shown on plain chest radiograph to have abated. If this initial approach takes place after the first 48 hours of active bleeding, FOB and HRCT are equally suitable.
