1.Ultrasonography of Herniated Lumbar Disc: Pre- & Postoperative Evaluation.

Sonographic evaluation of the 23 patients with herniated lumbar disc was carried out via anterior approach. The most important sonographic finding of the herniated disc was posterior displacement of the ventral echogenic line. Twenty patients(87%) were coincided in the result of the preoperative sonography and myelography. Of them, 19 patients were well correlated with surgical findings. A 19 of the 20 patients with satisfactory outcome found changes on the postoperative sonogram. In conclusion, disc sonography may be the modality evaluating patients with low back pain and radiculopathy. And also, sonography will become the supplementary study of the patient who has the persistent radiculopathy following discectomy.

We report a case of 21-year-old patient who has had dome-shaped nodules 1 cm in diameter on the right shoulder for one year. Histopathologic findings showed central dilated cystic hair follicles and many abortive hair follicles branching from the follicles and multiple immature follicles. Skin lesions were surgically excised.

3.The Result of Sugioka's Transtrochanteric Rotational Osteotomy

Avascular necrosis of the femoral head is generally localized in the anterosuperior aspect of the head. The posterior part of the head often retains a normal, smooth contour containing healthy cartilage even in much advanced cases. Because of this anatomical localization of the lesion, Y. Sugioka has devised a transtrochanteric rotational osteotomy in young and middle-aged person, which reduces the weight-bearing force on the necrotic area, and transfers shearing forces to the healthy posterior cartilage of the femoral head to prevent progressive collapse of the articular surface and improve alignment when articular collapse followed femoral head subluxiation. The authors reviewed 13 cases of Sugioka operation, performed at the Department of Orthopedic Surgery, Seoul National University Hospital during the period from December 1983 to Feburary 1986 and the following results were obtained. 1. There were 11 males and 1 female. The average age of the 13 cases was 33.3 years. 2. In all the cases of Grade I and Grade II hips, there was not progressiveness of necroticarea of femur heads. 3. By U.C.L.A. hip rating system, preoperative mean scores of pain, walking ability, function and activity were 2.5, 5.7, 4.8 and 4.3, respectively, while postoperative scores of these were increased to 7.7, 9.5, 8.4 and 7.8, respectively. 4. Average ranges of motion of hip joint were increased more postoperatively than preoperatively.

4.Pulmonary hypertension in systemic lupus erythematosus.

BACKGROUND: Pulmonary arterial hypertension (PAH) in systemic lupus erythematosus (SLE) has very poor prognosis and the prevalence of the disease is also not well delineated. This study was designed to assess the prevalence of PAH in asymptomatic SLE patients and to analyze the potential association between the presence of PAH and SLE including disease activity, organ involvement, and serology. METHODS: Asymptomatic 50 SLE patients and 50 healthy controls of a similar age and sex were assessed by an echocardiographic and clinical study. The diagnosis of PAH was made when the right ventricular systolic pressure (RVSP), measured by 2-dimensional echocardiography, was > or =40 mmHg. RESULTS: Compared to control subjects, lupus patients had significantly increased RVSP (mean+/-SD, 34.9+/-8.9 vs 31.2+/-3.6 mmHg, p=0.008). Patients with SLE were divided into three groups: RVSP> or =40 mmHg, RVSP=30~39 mmHg, RVSP<30 mmHg. Seven patients (14%) had RVSP> or =40 mmHg, 36 patients (72%) had RVSP=30~39 mmHg and 7 patients (14%) had RVSP<30 mmHg. In the healthy controls, none of them had RVSP> or =40 mm Hg. Of the controls, 43 (86%) had RVSP=30~39 mmHg and 7 (14%) had RVSP<30 mmHg. There was no statistical difference in organ involvement, Raynaud's phenomenon, vasculitis, anti-RNP and anti-cardiolipin Ab among the three SLE groups. However, lupus disease activity (checked by SLEDAI-2K) was associated with PAH (p=0.014). CONCLUSIONS: The prevalence of PAH as seen on echocardiograms in SLE patients was 14%. Organ involvement, Raynaud's phenomenon, vasculitis, antiphopholipid antibodies and anti-RNP except SLEDAI-2K were not associated with pulmonary arterial pressure.

7.Inflammatory Pseudotumor of the Liver.

Inflammatory pseudotumor of the liver is a rare benign tumor with microscopic finding of fibrous stroma and chronic inflammatory cell infiltration. Predominance of plasma cells, histiocytes, lymphocytes, some eosinophils and whorled pattern of fibrosis is often characteristic. Due to similar radiologic appearance of hepatocellular carcinoma, inflammatory pseudotumor of the liver was often misdiagnosed and hepatic resection is often performed before preoperative tissue diagnosis. We report a case of inflammatory pseudotumor, which was a 47-year-old woman with fever, upper aMominal pain and weight loss, diagnosed by sonoguided liver biopsy and treated with antibiotic therapy only without surgery. After two weeks of treatment, the radiologic image of the mass was almost disappeared with improvement of previous symptoms. This opportunity and the possible effectiveness of antibiotic treatment should keep in mind in the differential diagnosis of the hepatic mass with similar radiologic appearance of hepatocellular carcinoma.

8.A Case of Idipathic Calcinosis of the Scrotum.

The idiopathic calcinosis of the scrotum (Shapiro, 1970) is usually multiple, asymtomatic nodules of the scrotal skin which begins m childhood or early adult life, increase in the size and numbers, and sometimes breakdown to discharge chalky contents. Clinically they are most often misdiagnosed as cysts. The forty eight years old male patient visited our hospital on Oct. 14, 1980, because of multiple asymtomatic scrotal masses which are pin head to walnut sized. There is no family history of calcification, scrotal or otherwise. Histology shows naked amorphous masses in the dermis without any evidence of residual cysts, and is acicular clefts in the masses. Most of the tumors were surgically removed in 5 sessions.

9.A Case of Acrodermatitis Enteropathica-like Syndrome.

We report a case of acrodermatitis enteropathica-like syndrome after long-term parenteral nutrition in a 19-year-old female with a choriocarcinoma metastasized to the lung. Skin lesions consisted of vesicopustular periorificial eruptions, psoriasiform plaques on the knees and elbows, eczamatoid patches on the acral regions, paronychia, alopecia, and of poor wound healing, Severe diarrhea was accompanied. Zinc sulphate was administered 200mg three times daily for one week with improvement of the skin eruptions and diarrhea. The patient, however, died on the 7th day of zinc therapy with the poor general condition.

10.Serum IgE Level in Patients of Atopic Dermatitis and Atopic Dermatitis with Molluscum Contagiosum.

The increase of serum IgE in atopic dermatitis is well known. And atopic patients are frequently complicated by viral infection, such as molluscum contagiosum. So, we studied the serum IgE level in patients of atopic dermatitis and atopic dermatitis with molluscum eontagiosum to evaluate the relationship between molluscum contagiosum and IgE level in patients with atopic dermatitis. The results were summarized as follows : 1. Serum IgE level in patients of atopic dermatitis was 168.8+117.0 IU/ml, which was higher than that of control group (87.6+/-40.0 IU/ml) (p<0.05). 2. Serum IgE level in patients of atopic dermatitis with molluscum contagiosum was 137.8+/-91.9 IU/ml, which was lower than that of atopic dermatitis without molluscum contagiosum (178.3+/-123.7 IU/ml) (p>0.05).
