1.A Case of Cutaneous Focal Mucinosis.

We report a case of cutaneous focal mucinosis in a 60 year-old male, who had asymptomatic localized small flesh colored papule and nodule on the left side of the neck. The histopathologic finding revealed fibroblastic proliferatior in the dermis and an increased prodact on of hyaluronic acid by these fibroblasts. The lesions were treated by surgical excision.

2.Clinical Observation on the Cryptorchidism.

A clinical observation was made on 39 patients of cryptorchism who had been admitted to the Department of Urology, Seoul Red Cross Hospital during the 7 yeas period from January 1973 to December 1979. 1) During the period, 827 patients hospitalized, there were 39 cases of cryptorchism, giving a rate of 4.6%. 2) The age group, most frequently seen, was between 6 to 10 years. 3) The bilateral cryptorchisms were observed in 10 cases (26%). 4) The position of cryptorchism is most popular in the inguinal type 37 testes (75%) and intraabdominal type is 8 testes (16%).

3.A Clinical Study of Traumatic Urethral Injury and Stricture.

A clinical survey was made on 62 cases of traumatic urethral injury and stricture in the Department of Urology, Seoul Red Cross Hospital, during the period from January 1974 to December 1980. The following results were obtained: 1. The total number of in-patients during the period was estimated to 1120 and of which traumatic urethral injury and stricture were 62 cases of the 144 genitourinary injuries. 2. The most common causes of the urethral injury was traffic accident (45%) and next falling accident (40%), kicking in decreasing order. 3. Age distribution of urethral injury was the highest in the age 30 to 39 (45%) and next age 20 to 29 (39%). 4. Results from the present study show that the primary realignment procedure is more likely to complicate urethral stricture regardless the site of injury, compared with the perineal end-to-end anastomosis.

4.Infantile hemangioendothelioma of liver.

Radiologic findings of hemangioendothelioma of the liver were retrospectively analyzed in twelve infants. The radiological examinations included were sonography in 12 patients, computed tomograpy (CT) in six, magnetic resonance (MR) imaging in five, and angiography in two. Four patients were diagnosed pathologically, two by angiography, five by follow-up sonography, and one by clinical presentation with sonography. The common radiologic findings of the hemangioendothelioma were well circumscribed heterogeneous echogenic mass (75%) on sonography, peripheral massive enhancement (67%) on CT, bright high signal intensity on T2-weighted MR image (100%), and homogenous or peripheral enhancement (75%) on Gd-DTPA enhanced T1-weighted MR image. Dilated proximal aorta and enlarged draining hepatic veins on angiography an d other studies were also important findgdings. The follow-up sonography demonstrated the involution of lesions with some calcification in four patients and complete resolution in one. The authors believe that these findings in an under the age of 6 months strongly suggest the diagnosis of hemangioendothelioma of the liver, and follow-up sonography should be done.

5.Thyroglossal Duct Cyst: Sonographic Findings Revisited.

PURPOSE: The purpose of this study was to review and reevaluate the well-known ultrasonographic characteristics of cysts of the thyroglossal duct. MATERIALS AND METHODS: We retrospectively reviewed the ultrasonographic findings in 31 patients with pathologically proven cysts of the thyroglossal duct. Assessment involved the following variables: relationship to the hyoid bone, location, shape, size, margin, internal echogenicity, the presence of septa, solid component, and thickness of the cystic wall. The echogenicity of cystic contents was evaluated in 23 cysts for which surgical or aspiration biopsy reports were available. RESULTS: Cysts were closely attached to the hyoid bone in 30 cases (97%). Their location was infrahyoid in 14 cases, hyoid in 11, and suprahyoid in six; and midline in 18, both midline and off-midline in nine, and off-midline in four. A triangular shape was seen in 15 cases, a round or oval shape in 12, and a lobulated shape in three. Their diameter varied from 1 to 3.8 (mean, 2) cm, while internal echogenicity was hypoechoic in 12 cases, anechoic in ten, and isoechoic in nine. Septations were noted in six cases, and a solid component in two. The cystic wall was less than 1mm thick in 17 cases, even and greater than 1mm in eight, and uneven and greater than 1mm in six. Among 23 cases, the cystic contents were mucous in 18, serous in three, and purulent in two. The echogenicity of cysts with mucous content varied, while serous or purolent cysts were isoechoic. CONCLUSION: The characteristic sonographic finding of cyst of the thyroglossal duct is a centrally located triangular neck cyst, closely related to the hyoid bone.

7.Torsion of Spermatic Cord.

Authors experienced a case of torsion of spermatic cord recently and reviewed the literatures. The patient was a 33 year-old male with chief complaints of sudden onset on left scrotal swelling and testicular pain. Physical examination was not significant except positive Prehn`s sign on the involving side of testicle. Under the diagnosis of torsion of spermatic cord, he was performed of left orchiectomy and the opposite side orchiopexy.

8.A Case of Ureteral Polyp.

9.A survey on the education, medical practice, research, and fringe benefits of Korean medical school faculty.

In general, medical school faculty have to perform clinical practice in addition to their educational and research activities, unlike the professors of other departments, while simultaneously playing an important role within the medical profession. However, some organizational or environmental factors decrease the job satisfaction of medical professors. This study aimed to determine the current status of medical schools professors' job activities, satisfaction level, factors related to job satisfaction, and so on. A structured questionnaire was used in the survey and 936 valid responses (response rate, 79.1%) were analyzed using SAS version 9.1. Items included in the questionnaire were work tasks, satisfaction with work and environment, fringe benefits, and future plans. Our study found that the satisfaction of respondents with research activities was not high, and they had negative perceptions of their work environment. Also, it was found that job satisfaction was most affected by work environment. In the section on fringe benefits, a variety of fringe benefits were provided to respondents but their actual satisfaction was not high. To enhance the overall job satisfaction of medical school faculty, all the matters related to their work tasks and environmental factors have to be considered in the aspect of their own role in medical school. The limitations of this study were a low response rate to the early online survey and a long duration of the survey period. However, these limitations were resolved by an additional mail survey modality and statistical techniques. It is meaningful that this study was an extensive survey aimed at medical school faculty and dealt with a comprehensive range of issues.

10.Topical Photodynamic Therapy for Treatment of Actinic Keratosis Using Light-Emitting Diode (LED) Device.

BACKGROUND: Photodynamic therapy is a treatment modality which involves the sequential administration of a photosensitizer and light. Topical photodynamic therapy is being increasingly used for superficial non-melanoma skin cancers and their precursors. OBJECTIVE: The purpose of this study was to evaluate the effectiveness and side effects of treating actinic keratosis with photodynamic therapy using a light-emitting diode device and 20% 5-aminolevulinic acid. METHODS: Photodynamic therapy, with a 630+/-50nm light-emitting diode device and 5-aminolevulinic acid, was used to treat 12 lesions of actinic keratosis on 8 patients. The light intensity used was 50mW/cm2 , and the light dose was 100-120J/cm2 . Four weeks after 1 or 2 treatment sessions, the outcome was evaluated by histopathological findings. RESULTS: Complete clearance was achieved in 8 of 12 lesions after a single treatment, and in 3 of 12 lesions after 2 treatment sessions. Only 1 case showed partial response after 2 treatment sessions. Only 2 patients suffered from a stinging sensation when exposed to the light therapy. CONCLUSION: Topical photodynamic therapy using a light-emitting diode is safe and effective in the treatment of actinic keratosis.
