2.Healing of the Bony Gap with Periosteal Osteogenesis After Tibial Diaphysectomy in Adult: Report of A Case

The diaphysectomy of a large tubular bone is occasionally done as a method of treating chronic osteomyelitis. Obliteration of the bony gap with stable complete reunion by periosteal new bone formation is rare in adult and is usually not expected to be complete. Therefore, the bony defect generally is replaced or refilled to bridge the gap by massive bone graft after complete subsidence of the inflammatory signs, The authors report a case of chronic osteomyelitis of tibial shaft which was treated by diaphysectomy and healed with complete and stable periosteal new bone formation in the gap of diaphysectomized tibia without graft or replacement in a healthy young adult.

3.A Case of Congenital Metatarsus Varus Feduced by Surgical Method

A case of fixed matatarsus Varus developed in right foot in 10 years old girl is reported, which was reduced surgically. Roentgenologically the navicular bone was showed to be displaced laterally from the head of talus and all metatarsals were angulated medially. Though most deformies should be corrected by the use of corrective cast, this deformity was treated surgically without initial conservative method. After surgery and cast immobilization the foot was corrected and function is good.

4.Two Different Penicillin Preparations in the Treatment of Gonorrhhea.

Penicillin is still the first choice of drug in the treatment of gonorrhea. The ever lowering sensitivity of Neisseria gonorrhoeae to penicillin and the emergence of PPNG (Penicillinase Producing Neisseria Gonorrhoeae) in the Far East and other parts of the world neceessitate reapprisal of treatrnent schedules with penicillin for gonococcal infections. In Korea, no authentical study in this regard has been done recently. The presence of intracellular gram r.egative diplococci in urethral smear as weIl as the presence of oxidase positive typical, round, convex, smooth, glistening, tratmslucent, greyish colonies of 0.5 to 2mm in diameter a 24 hours composed of grarn negative diplococci are used for presump'ive evidence of gonococccal infection 224 male patients with acnte uncomplicated gonococal infections were treated one of following regimens. Regimen A: 4.8mega unit combicillin (3.6mega unit procaine penicillin+l.2 mega unit sodium penicillin) i.m.. preceded by 1gm probenecid p.o., Regirnen B: 4.8 mega unit procaine penicillin i.m. preceded by 1gm probenecid P.O. Regimen A showed 14 failures(11.3% arnong 124. Regimen B showed 19 failures (19%) among 100 It is suggested that in Korea sesnsitivity to penicillin of Yeisseria gonorrhoeae is quite low.

5.A Case of Early Congenital Syphilis.

We report one case of 1 month old male with early congenital syphilis Who represented the characteristic clinical features and laboratory findings including darkfield examindtion and quantitative VDRL test. The histopathologic findings of the patients skin biopsy specimen showed extravasation of erythrocytes and perivascular infiltration of plasma cells were found. in addition to the previous described findings by Fraser. He was managed by daily injection of 200,000 units of aqueous procaine penicillin for 8 days and then three injections of 150,000 units of PA.M at two days intervals with excellent effect

6.A Case of Generalized Pustular Psoriasis in Infancy.

A case of familiar benign chronic pemphigus is reported. A 41 years old housewife has recurrent history of developing grouped vesicular eruptions with erythematous erosion on the intertrigenous area since 29 years of her age. The lesions were used to aggrevated in summer and improved in autumn. Familiar history revealed same disease of her grand mother, father, 44 year old elder sister and 39 year old younger sister. All of whom had similar clinical signs and symptoms. Routine physical and Iaboratoryfindings were within normal limits. The histopathological findings, from vesicular eruption of the lateral aspect of neck, showed multilocular suprabasal clefts with acantholsis, villi formation and hyperkeratosis. The lesions were improved hy the local application of corticostcroidoid ointrnent only.

8.A Case of Cutaneous Horn Arising from Actinic Keratosis.

Cutaneous horn is the clinical term for a circumscribed, conical, markedly hyperkeratotic lesion. We report a case of cutaneous horn occuring on the face in a 81 year old women. An about 1.2 X1.3 cm sized dark brownish conical horn was developed on her face 1 year before. Histopathologic findings showed marked hyperkeratosis, parakeratosis, and papillornatosis of the epidermis, and some cells of straturn mslpighii showed pleornorphism and nuclesr atypis. This case was considered to be cutaneous horn arising from actinic keratosis, and we performed minimal local excision.

9.One Case of Erythromelalgia.

Erythromelalgia is a rare disease characterized by burning pain of the extremities associated with erythema and increased temperature of the skin. A case of erythromelalgia was reported with review of literatures. It seemed that this is the first reported case in Korean literatures. patient was 12 years old male having erythematous swelling associated with burning pain on his feet and lower legs for five years. Immersion of his feet in warm water quickly, increased the pain but cold water gave prompt relief that he soaked his feet for many hours during the day and night. No specific abnormalities were noticed in Iaboratory studies, Treatment with various topical, systemic medication revealed of unfavorable result.

10.A Case fo Pigmented Lichen Sclerosus et Atrophicus.

Lichen sclerosus et atrophicus (LSA) is an uncommon condition of the skin with small white macular or atrophic plaques which predominantly affects the genital regions of middle aged and elderly women. We present an atypical case of LSA on a 58 year old woman. The patient had a brownish-black colored patch of 2.5 * 7.0 cm in size on the right clavicular area of one year's duration. The histopatholgy of the lesion disclosed classical findings of LSA. A marked incontinence of pigment is noted in the upper dermis which accounts for the clinical picture of the lesion. She was treated with topical hydrocortisone cream resulting in a marked diminution of pigmentation.
