1.Therapeutic Effects of Ruby Laser Photocoagulation on Retinal Diseases.

I. Central Serous Chorioretinopathies: One hundred and nine patients with central serous retinopathy were an a lysed after treatment with AO Laser Photocoagulator. The patients in this study were mainly male (87%) between the ages of 30 and 40 years (81.5%). Involvement was exclusively unilateral (98.1 %), and fluorescein fundus angiography revealed leaking points in almost all cases (98.1%). Majority of patients (67.4%) recovered normal vision with one or two sittings of photocoagulation, each sitting composed of 6 to 8 energy pulses producing grade I lesion. During the course of this study normal vision is gained in 80.2% of patients. With the development of fluorescein fundus angiography the lesion of central serous retinopathy were found to be choroidal abnormalities associated serous leakage across Bruch's membrane. Burn is made by the light energy delivered to pigment epithlium. The burn results, through normal healing processes, in a scar, which seals off the communication across Bruch's membrane. This phenomenon constructs the basis of treatment of central serous retinopathy with photocoagulation. II. Diabetic Retinopathies: Diabetic retinopathy is rapidly becoming the prominent cause of blindness in the world. The striking change of retinopathy is in the venous side of retinal capillaries and their altered function leads to exudation, hemorrhage, formation of new vessels, and fibrosis over the retinal plane or into vitreous, resuJting ultimately into blindness. Of the 34 patients treated with AO Ruby Photocoagulator at Han Yang Uni. Hos., the response of the patients in pre-proliferative stage was excellent, all of them recovering vision above 0.8. The patients in proliferative stage, however, responded poorly to photocoagulation. It is learned in this study that the optimal time of photocoagulation is in the pre-proliferative stage. The photocoagulation of retinitis proliferans has some effect only in arresting or delaying the progress of retinal changes. III. Focal Choroiditis: Nonsuppurative intracular inflammation is caused by various etiology. Scrutinization such as serologic and skin test or x-ray finding is important for the diagnosis of uveitis but not so significant as the definitive clinical characteristics of specific forms of ocular inflammation. Focal choroiditis, sized less than 1/4 disk diameter, was coagulated with ruby laser. Four out of 6 patients recovered good vision, the lesion being replaced by a scar. The remaining two patients did not visit the hospital after one session of photocoagulation. It is emphasized by the author that the focal choroiditis is excellent indication to the laser photocoagulation. IV. Eales' Disease: Eales' disease is considered to be due to hypersensitivity to mostly tuberculo-antigen, clinically characterized by retinal perivenous exudate, neovascularization and vitreous hemorrhage. A boy has recovered his normal vision after 18 sessions of ruby laser photocoagulation. There have been no benificent effects where photocoagulation was discontinued after two or three sittings.

2.Chromic Cyclitis, Pars Planitis.

This disease entity, first described under the desgnation of cyclitis by Ernst Fuchs, has received renewed attention in the recenet years, having been described as peripheral uveitis, pars planitis, and cyclitis with peripheral chorioretinitis. Apparently new ophthalmoscopic findings have caused some obserbers to believe they are dealing with a new entity, because indirect ophthalmoscopy with scleral depression shows exudation in the pars plana region in severe or advanced cases. In this entity, The following characteristic findings are noted, occurring bilaterally, inflammatory cells in anterior vitreous, slight flare and cells in the anterior chamber in come instances, dilated veins, and snowball deposition of inflammatory cells over the pare plana and ora serrata, seen with scleral depression and indirect ophthalmoscope extending over the lower 180 degrees. Cystoid macular degeneration are demonstrated with fluorescein angiography in over half the caces; papilledema may occur in late cases; and peripheral retinal perivascular sheathing is noted in long standing cases.

3.A Case of Orbital Liposarcoma.

Liposarcoma is extremely rare malignant tumor which develops primary in the orbit. Here, we report a case of primary orbital liposarcoma in 17 year old female patient. She visited our hospital due to decreased visual acuity and protrusion of Rt. eyeball. Malignant tumor was suspected by many examination. So exenteration was performed and tumor mass was confirmed as liposarcoma by pathologic examination. She was received cobalt 60 radiation therapy, but recurrent tumor mass appeared at the orbital apex 1 year after operation.

6.A Case of von Hippel-Lindau Disease.

Recently, we have experienced a case of von Hippel-Lindau's disease. The patient was a 16 years old Korean girl who visited to our hospital because of visual impairment in both eyes since childhood, mental retardation, dysarthria and gait disturbance. On the eye examination, visual acuity of both eyes were 0.04, incorrectable, funduscopy revealed optic atrophy and typical angiomatosis retinae in the right eye and optic atrophy associated with retinal detachment in the left eye. The cerebral angiography showed a space occupying mass in the left cerebellum. Thus this case was presented as a complete form of von Hippel-Lindau's disease, and a brief review of literatures was also discussed.

7.A Case of Congenital Optic Pit.

Optic pits are congenital craterlike holes or indentation in the surface of the optic disc. They are oval in shape and located in the inferior temporal quadrant of the optic disc. The authors experienced a case of congenital optic pit of the left eye of 18 year old male-patient. Fluorescein angiography showed hypofluorescence of the pit in early arteriovenous phase, becomming hyperfluorescence in the late phase. Ophthalmoscopic view of the diseased eye showed no abnormality of macular area. There was no visual field defect except the enlarged physiologic blind spot. Here we briefly report a case of congenital optic pit. referring the literature relating to the optic pit.

8.A Case of Malignant Melanoma in the Conjunctiva.

Authors repGrted one case of malignant melanoma which is located in the conjunctiva of the left eye This male patient, aged 45, has been a history of progressive enlargement of this melanoma since over ten years. At the time of first visit to the hospital, this melanoma revealed dark brownish strawbery-like pedunclated mass in his medial bulbar conjunctiva, and extended to fornix, palpebral conjunctiva, and lidmargin. Subtotal exenteration, BCG therapy, and X-ray radiation was performed successfully and was conformed histopathologically. In addition to some clinical observation, a brief review of literature has been described.

9.A Case of Wilson's Disease.

The authors have recntly experienced a case of Wilson's disease which was developed Kayser-Fleischer ring. The 13 years old male patient has dysarthria, slurring speech, choreiform movement and gait disturbance about for 2 years. The treatment was performed by B.A.L. and penicillamine and the result has been excellent up to date, 31/2 months after medical treatment. Physical examination on discharge were normal mentality, absence of tremors and pale Kayser Fleischer ring.

10.Comparison of Clinical Outcomes after Total Knee Arthroplasty with and without Patellar Resurfacing in the Same Patients

Background:
Patellar resurfacing is performed in total knee arthroplasty (TKA) to improve pain in the anterior compartment of the knee joint and to restore proper coordination and movement of the patellofemoral joint. The purpose of this study was to analyze differences in clinical outcomes according to patellar resurfacing in patients who underwent bilateral TKA.
Methods:
Forty-three patients who underwent bilateral primary TKA with patellar resurfacing on one side only were included. Patellar resurfacing was performed selectively according to the status of the patella cartilage surface. Knee Society score (knee and function), Feller score, Kujala score, and Samsung Medical Center (SMC) score (pain and function) were evaluated.
Results:
There were no significant differences in Knee Society pain and function scores, Feller score, Kujala score, and SMC pain and function scores according to patellar resurfacing. On the comparison of SMC scores, there was no difference except for two questions.
Conclusions
It is advisable not to perform resurfacing on normal patellae. However, in order to apply this result to damaged patellae, comparative studies between resurfaced patellae and damaged patellae are needed.
