4.Reconstructive Surgery for Old Ligament Injuries of the Knee

Reconstructive surgeries for old ligament injuries of the knee vary according to the site and nature of injury as well as the age, health, physical demand and cartilage condition of the patients. No single best procedure is present for any old ligament injuries, but the most adaptable technique should be applied according to accurate diagnosis and experiences of surgeon. We reviewed our experiences of several reconstructive surgeries for the 38 cases with chronic ligamentous instabilities and summarized as following. 1) The most prevalent age of the patients was the third decades and males were 5.3 times more common than females. 2) Traffic accidents was the most common cause. 3) Tear of anterior cruciate ligament and medial collateral ligament appeared to be the most common injury. 4) Results at final follow up was not seemed to be affected by severity or location of ligament injury. 5) Intraarticular reconstruction without combined extraarticular reconstruction was not considered to be effective enough for anterior cruciate ligament deficient knee. 6) Although the number of cases was not large enough, we reviewed various reconstructive surgical techniques for comparison and evaluated the effects.

5.Bone Graft or Cement Fill for Tibial Defects in Total Knee Arthroplasty

The purpose of this study is to evaluate the results of bone graft or cement fill for tibial bone defect in total knee arthroplasty. 51 knee in 45 patients which had bone graft or cement fill for tibial bone defect of more than 1cm3 in total knee arthroplasty, between March, 1990 and July, 1994 at the department of Orthopedic Surgery in Hanyang University Hospital were followed for average 30 months. Bone grafts were performed in 33 knees and cement fills in 18 knees. The size of tibial defect was measured after removal of sclerotic and dense bone in the tibial plateau. The average size of tibial defect treated by bone graft was 4.9 cm3 in volume, 11 mm in height and the average size of tibial defects treated by cement fills was 1.5cm3 in volume, 6.8 mm in height. The results were as follows ; 1. In true A-P and lateral x-ray checked by fluoroscopic guide, trabecular union was seen in all 33 knees of bone grafts without collapse, loosening and bone resorption. In one of 18 knees of cement fill, radiolucent line was detected 2 mm in width between cement mantle and the adjacent tibial bone. In other 17 knees, radiolucent was seen 1 mm or less in width. 2. According to the system of Knee Society Score, average knee rating score was improved from 23.2 points preoperatively to 90 points postoperatively in bone graft cases, from 23.4 points preoperatively to 93.2 points postoperatively in cement fill cases. Average functional score was improved from 14.8 points preoperatively to 81.5 points postoperatively in bone graft cases, from 13.3 points preoperatively to 81.4 points postoperatively in cement fill cases. 3. Finally, we obtained the good results by cement fill during cement fixation of tibial component or bone graft without cement fixation of tibial component for small tibial bone defect and by bone graft for large tibial bone defect.

Demyelinating syndrome can rarely occur in Sjogren's syndrome or rheumatoid arthritis. We describe a patient of Sjogren's syndrome with multiple sclerosis-like features whose rheumatoid arthritis has been managed for 3 years. The patient presented paraparesis and urinary retention, and improved with high-dose corticosteroid therapy.

7.Results of Total Knee Arthroplasty with NexGen(R) system (5-year follow up).

PURPOSE: To evaluate the mid-term (over 5 years) clinical and radiologic results of NexGen(R) total knee replacement arthroplasty (TKRA)retrospectively. MATERIALS AND METHODS: Between Nov. 1996 and Jun. 1997, 57 knees in 43 patients who had been followed up for 5 years after TKRA with NexGen(R) were evaluated retrospectively for clinical and radiologic results. RESULTS: The average range of motion increased from 116degree preoperatively to 125 degrees at the last follow-up. In patients with osteoarthritis, the average preoperative knee score (59.9) and functional score (40.3) improved to 94.7 and 83.7, respectively. In rheumatoid arthritis patients, the mean knee score and functional score also improved from 55.6 and 35.9 to 88.9 and 79.9. Roentgenographic evaluation revealed a radiolucency rate of 60%. Complications were periprosthetic fractures in 2 cases and aseptic loosening in 1 case. The latter had received a reimplantation. CONCLUSION: The mid-term results of NexGen. TKRA were reliable and satisfactory in terms of improvements of; range of motion, restoration of function, and rare complications. However, long-term follow-up evaluation is necessary to monitor many cases of radiolucent line (60%) in the femoral component.

8.A Case of Fetal Cardiac Tumor Diagnosed by Ultrasonography.

Primary tumors of the heart are uncommon in fetus and neonate, and most tumors are histologically benign. They remain clinically unimportant and regress with age, but they may cause mechanical obstruction and induce even life-threatening symptoms. Hemodynamic disturbance are correlated with the size and location of the tumors. Murmurs, arrhythmia, cyanosis, respiratory distress, and heart failure are main presenting signs of cardiac tumors in perinatal periods. The most common histological form of cardiac tumors in perinatal periods is rhabdomyoma. Rhabdomyomas are associated with tuberous sclerosis in 50% of cases. The widespread use of echocardiography and other new imaging techniques has contributed significantly to eariler diagnosis, treatment, and thus improving survival. We have experienced a case of fetal cardiac tumor which were detected by prenatal ultrasonographic evaluation in 21 weeks and 4 days of gestation. After termination, autopsy was performed and rhabdomyoma was diagnosed, but not accompanied tuberous sclerosis.

9.A Case Report of Giant Pigmented Nevus Associated with Porencephaly: A Variant of Neurocutaneous Melanosis.

Giant pigmented nevus is a variety of congenital melanocytic nevi, and may be associated with various abnormalities, such as skeletal malformation, lipoma, vascular nevi, melanosis in the meninges, meningocele, and intracranial arteriovenous malformation. We report a case with congenital giant pigmented nevus and porencephaly. This association should be considered as the spectrum of neurocutaneous syndromes related to congenital giant pigmented nevus.

10.Cortical Laminar Necrosis associated with Osmotic Demyelination Syndrome.

Cortical laminar necrosis has been rarely observed in osmotic demyelination syndrome. We report a 32-year-old female patient who became comatose after the rapid correction of hyponatremia. There were high signal intensities in the pons and bilateral deep gray nuclei on T2-weighted MRI images, and linear hyperintensities along the cerebral cortices on T1-weighted images with a diffuse gyriform enhancement. MR spectroscopic findings showed a decrease of the N-acetyl aspartate peak and an increase in those of the lipid and lactate complex. The case demonstrates that a severe form of osmotic demyelination syndrome accompanying cortical laminar necrosis can result from the rapid correction of hyponatremia.
