1.Two Different Penicillin Preparations in the Treatment of Gonorrhhea.

Penicillin is still the first choice of drug in the treatment of gonorrhea. The ever lowering sensitivity of Neisseria gonorrhoeae to penicillin and the emergence of PPNG (Penicillinase Producing Neisseria Gonorrhoeae) in the Far East and other parts of the world neceessitate reapprisal of treatrnent schedules with penicillin for gonococcal infections. In Korea, no authentical study in this regard has been done recently. The presence of intracellular gram r.egative diplococci in urethral smear as weIl as the presence of oxidase positive typical, round, convex, smooth, glistening, tratmslucent, greyish colonies of 0.5 to 2mm in diameter a 24 hours composed of grarn negative diplococci are used for presump'ive evidence of gonococccal infection 224 male patients with acnte uncomplicated gonococal infections were treated one of following regimens. Regimen A: 4.8mega unit combicillin (3.6mega unit procaine penicillin+l.2 mega unit sodium penicillin) i.m.. preceded by 1gm probenecid p.o., Regirnen B: 4.8 mega unit procaine penicillin i.m. preceded by 1gm probenecid P.O. Regimen A showed 14 failures(11.3% arnong 124. Regimen B showed 19 failures (19%) among 100 It is suggested that in Korea sesnsitivity to penicillin of Yeisseria gonorrhoeae is quite low.

2.A Case of Early Congenital Syphilis.

We report one case of 1 month old male with early congenital syphilis Who represented the characteristic clinical features and laboratory findings including darkfield examindtion and quantitative VDRL test. The histopathologic findings of the patients skin biopsy specimen showed extravasation of erythrocytes and perivascular infiltration of plasma cells were found. in addition to the previous described findings by Fraser. He was managed by daily injection of 200,000 units of aqueous procaine penicillin for 8 days and then three injections of 150,000 units of PA.M at two days intervals with excellent effect

3.The effects of UVA and UVB on proliferation and melanization in SK 23 melanoma cell lines.

BACKGROUND: The visible cutaneous pigmentary response to ultraviolet A(UVA) is immediate, whereas ultraviolet B(UVB)-induced pigmentation appears after a delay of several days. However, some investigators reported that UVA also can induce delayed taniing. OBJECTIVE: In order to determine whether the pigmentation induceil by UVA irradiation is accompanied by melanocytes responses which are the same or different from those following a UVB-indueed tan, we irraiated malignant melanoma cells to UVA or UVB. METHOD: On the 7th day after irradiation of UVA or UVB, we exa nined the cell counts and the melanin content of control and experimental groups. RESULTS: Although a dose of 10 J/cm decreased the number of manignant melanoma cells, there was no significant difference between the control and UVA-exposure griiups. But there was a significant decrease after UVB-exposun. UVB-exposure groups showed a significant increase of melanin cortent. A dose of 10 J/cm of UVA also induced a significant increase of melanin content. CONCLUSIONS: The data suggest that UVA at a clinically relevant dose has a significant effect on human melanocytes. So, UVA very likely plays a role in the increased, melanization associated with delayed tanning.

4.Cutaneous Metastasis of Pancreatic Carcinoma by Percutaneous Fine Needle Aspiration Biopsy.

The cutaneous metastasis of pancreatic carcinoma by this technique is rarely reported in the literature. A 67-year-old woman was evaluated for left-sided abdominal pain and a palpable abdominal mass. Abdominal CT confirmed the presence of a solid mass involving the pancreas. Ultrasonically guided percutaneous aspiration biopsy was performed. About six month later, the patient was readmitted with a tender tumor mass at the site of the previous aspiration biopsy. Physical examination disclosed a 4 × 4 cm sized firm, tender tumor mass on the abdomen. Histopathological findings of the tumor mass are consistent with metasta tic lesions from a pancreatic carcinoma. We report a case that suggests cutaneous metastsis of pancreatic carcinoma by fine needle aspiration biopsy.

5.A Case of Hypohidrotic Ectodermal dysplasia.

Hypohidrotic ectodermali dysplasia is a rare and heterogenous genodermatosis that is characterzed by hypohidrosis, hyptrichosis, dental hypoplasia, and a characteristic facies. A 16-month-old male was evaluated for anodontia and anhidrosis since birth. There was no family history of this disease. His face was characterized by a broad nose and sparse eyebrows and periorbital wrirkling. The skin was dry, and scalp hairs were thin and sparse. Skull X-ray finding revealed noth ing remarkable except de;ayed teeth eruption. His developmental status for which we used DDS'I' was slightly reduced on inental development and showed normal on physical development. A skin biopsy was done on skin from the left palm, which showed a few immature eccrine glands in the dermis. We report a typical form of hypohidrotic ectodermal dysplasia with a review of the literature.

6.Excision of a Cirsoid Aneurysm of the Scalp after Proximal Ligation of the Main Feeder: Case Report.

Cirsoid aneurysm of the scalp is difficult to manage because of complicated vascular anatomy and massive intraoperative hemorrhage. The authors report a patient in whom a large cirsoid aneurysm of the scalp was eliminated by excision after proximal ligation of the main feeder. Where possible, total excision followed by proximal ligation of feeders may be a effective ablative technique to manage cirsoid aneurysm of the scalp.

7.A Case fo Pigmented Lichen Sclerosus et Atrophicus.

Lichen sclerosus et atrophicus (LSA) is an uncommon condition of the skin with small white macular or atrophic plaques which predominantly affects the genital regions of middle aged and elderly women. We present an atypical case of LSA on a 58 year old woman. The patient had a brownish-black colored patch of 2.5 * 7.0 cm in size on the right clavicular area of one year's duration. The histopatholgy of the lesion disclosed classical findings of LSA. A marked incontinence of pigment is noted in the upper dermis which accounts for the clinical picture of the lesion. She was treated with topical hydrocortisone cream resulting in a marked diminution of pigmentation.

8.A Case of Acrodermatitis Enteropathica-like Syndrome.

We report a case of acrodermatitis enteropathica-like syndrome after long-term parenteral nutrition in a 19-year-old female with a choriocarcinoma metastasized to the lung. Skin lesions consisted of vesicopustular periorificial eruptions, psoriasiform plaques on the knees and elbows, eczamatoid patches on the acral regions, paronychia, alopecia, and of poor wound healing, Severe diarrhea was accompanied. Zinc sulphate was administered 200mg three times daily for one week with improvement of the skin eruptions and diarrhea. The patient, however, died on the 7th day of zinc therapy with the poor general condition.

9.A case of erythema elevatum diutinum.

Erythema elevatum diutinurum is a relatively rare disease of unknciwr etiology. It is charaterized by persistent erythematous plaque and nodule that are usually affected symmetrically over extensor surface of extremities. Our patient, a 16-year-old female was referred for evaluation of asy nptomatic papules and nodules on extensor aspects of the hands, legs and buttocks. She complained of arthralgia on the knees. The laboratory values were all within normal limits with the exception of slight elevation of white blood cell counts and erythrocyte sedimenation rate. A skin biopsy specimen we obtained from the nodular lesion of the leg that showed preclominantly perivascular neutrophilic influration with occasional lymphocytes and unclear dusts. She was treated with 75mg/day of dapsone. Shin lesions were much improved after ten days. Eight weeks later, the lesion had almost resolved. After the discontinuation of dapsone, new lesions developed on the legs. Retreatment with dapsone resulted it a dramatic response of her lesions after three weeks.
