1.A Study on Identified Red Cell Irregular Antibodies in Korean.

34,073 prospective transfusion candidates sera, among 127,087 hospitalized patients in Hanyang University Medical Center between Jan. 1989 and Dec. 1993, were analyzed for detection of red cell irregular antibodies screening and identification. This study revealed investigation of the distribution and the characteristics of irregular antibodies. Tube method was performed including saline room temperature, saline 37 and antiglobuline phase. The following results were obtained. 1. Of 127,087 inpatients, prospective blood transfusion candidates were 34,078, 26.8%. 2. 151 cases out of 34,078 sera was positive irregular antibody, 0.443 %. 3. Frequency of irregular antibodies were 15 cases(0.044 %) of anti-Lea, 14 cases(0.041%) of anti-Leb, 14 cases(0.041%) of anti-P1, 8 cases(0.0234%) of anti-IH and 8 cases(0.0234 %) of anti-E in order. 4. Undetectable cold antibody was 39 cases(0.1145%), warm antibody, 27 cases(0.0792 %). 5. It is required that various testing method for the safty transfusion.

4.Clinical Experience of Automated Percutaneous Lumbar Discectomy

One hundred and twenty patients with symptomatic lumbar disc herniation were treated with automated percutaneous lumbar discectomy(APLD) from June 1988 to October 1989 in Seoul National University Hospital. Among them, 95 patients were followed up for more than 6 months. Number of male patients was 58 and female 37. Age ranged from 15 to 57 years with average 28 years. L4-L5 was the most commomly involved level comprising 80%, and 2 level involvement was 9%. Average duration of procedure was 38 minutes, and average amount of tissue aspirated was 5.2 grams and average hospital stay was 2.4 days. The average follw up was 10.8 months. Eighty seven percent of patients were satisfactory and 13% unsatisfactory and there was no major complication. There was no significant disc space narrowing in all the cases and the size of herniation revealed no significant change in all the patients, even the patient had a satisfactory result. APLD was compared with chemonucleolysis performed during 1988 in terms of clinical results, disc space narrowing on plain films and herniation size change on follw up CT. Chemonucleolysis also had satisfactory result in 89% with reduction of herniation size in 41% but induced significant disc space narrowing in 84%. Authors experienced some special cases, which are generally considered poor indication for percutaneous discectomy. Those were degenerative, calcified and possibly ruptured discs. Satisfactory results were obtained in 10 of 14 these patients.

9.Adding Posterior Lumbar Interbody Fusion to Pedicle Screw Fixatin and Posterolateral Fusion after Decompression in Spondylolytic Spondylolisthesis

STUDY DESIGN: This is a retrospective study analyzing 76 patients treated by decompression, pedicle screw instrumentatin and fusion for spondylolytic spondylolisthesis with symptomatic spinal stenosis. OBJECTIVES: This is to verify the advantages of adding posterior lumbar interbody fusion (PLIF) to the usual posterolateral fusion (PLF) with pedicle screw instrumentation. SUMMARY OF BACKGROUND DATA: Stabilization after decompression of spondylolytic spondy- lolisthsis is difficult due to insufficiency of fusion base, gap between the bases and incompetent anterior disc support. PLIF offers anterior support, reduction and a broad fusion base. METHODS: Forty patients were treated with PLF and 36 were treated with additional PLIF. They were compared for union, reductin of the deformity and clinical results. RESULTS: The patients were followed up for more than 2 years (mean: 4.6 years). Nonunion was observed in 3 PLF cases (7.5%) and none in PLIF. Reduction of slippage was 28.3% in PLF and 41.6% in PLIF (p < 0.05). In PLF group, 8 patients (20%) had recurrence of deformity with loss of reduction more than 50%. Hardware failures occurred in 2 with PLF. There was no major neurologic complications in both groups. Both groups had a few difference in the satisfactory results, but some difference in the excellent result by Kirkaldy-Willis criteria. Excellent result was 45% in PLF and 75% in PLIF. CONCLUSIONS: Addition of PLIF to PLF following a complete decompression and pedicle screw flxation is a recommendable procedure for the treatment of spondylolytic spondylolisthesis with spinal stenosis.
