The purpose of this study was to assess how job stiess affects the nursing performance of emergerncy room nurses. Data were collected from 210 emergency room nurses at 20 general hospitals, with more than 200 beds, in Pusan, Taegu and Kungbuk province, between October 5 and October 15, 1998. The instruments used for this study were the job stress scale which was developed by Mae Ja, Kim and Mi Ok, Gu(1984), and nursing performance of nurses scale developed by Kwi Ae, Chung(1989). Data Analysis was done using t-test, ANOVA, Tukey test, and Pearson Correlation. The results were as follows : 1. The highest rank of job stress items, emergency room nursing were as follows with lessening order when physicians delegate their clinical responsibilities to nurses(M=3.12+/-0.69). The most stressed factor among the nursing works was reported as night duty(M =2.99+/-0.87). 2. The mean score of nursing performance items, to prevent patient from falling or other injury(M = 3.69+/-0.50). 3. Our study reveals that there was statistically significant negative correlation relationship between stress level and emergency room nursing performance(r= -0.14, p=0.0454), in other words, the more stress the nurses experienced, the weaker their nursing performance became. Two factors, heavy work load(r=0.30, p=0.0001), and poor compensation(r =0.15, p =0.031). were negativly related to nursing performance. 4. The study revealed that the group with monthly income experienced less stress(F=2.71, p= 0.0461). Also, the degree of stress was significantly lower in the group with work experience (F=3.01, p=0.0313). Stress varied according to position superintendent nurse showed a lower level of stress than lay nurses and change nurses (F=2.93, p=0.0349). 5. There was significant difference in the degree of nursing perfnrm~nre according to religion (F= 3.97, p=0.0088). In conclusion, there was statistically significant negative correlations between job stress and nursing performance, so there is need for special consideration in nursing administration in terms of reducing the job stress in emergency nursing. Suggestions based on this study as follows. 1. Further study is necessary to examine variables that influence job stress and nursing performance. 2. The expem~ntaI study is needed for an effectiveness of systerrntic and in-lividual effort in order to reduce or manage nurses'stress in the emergency room.

This study was conducted to assess the knowledge and educational need of pregnant women relating to antepartum, delivery, puerperium period and to identify the variables which influenced their knowledge and educational need. The study subjects were 77 antepartum women who had antenatal educations and 106 antepartum women who did not have antenatal education. Number of total antepartum women were 183. Data were collected from women who visited for antenatal care at general hospitals and public health centers in Pusan and Kyungnam area using qestionnaires from Feb. 1. 1998 to Apr. 20. 1998. SAS/PC program was used for data analysis. ; Chi-square test, t-test, Pearson's Correlation Coefficient, ANOVA(Duncan procedure) and MANOVA. The results of this study were as follows: 1. Relating to the antepartum period between two groups, the knowledge about the number of antenatal care was low, relating to labor the knowledge about the time fix the fetal position and relating to postpartum, the knowledge about the change of blood pressure and body temperature was low. 2. The comparision means of knowledge were adjusted by social demographic characteristics related to knowledge. There were statistically significant differences in total mean score of knowledge about pregnancy, delivery and postpartum between two groups. In full marks 19, the mean scores of the educational group and non-educational group were 13.08 and 11.74 respectively (P=.000). In resultly, there are statistically significant differences between two groups. The effectiveness of antenatal education group was very high and showed its importance. 3. The comparision means of educational need were adjusted by social demographic characteristics related to educational need. The mean scores of educational need were statistically significantly different in pregnancy, delivery and puerperium period. 4. There were positive correlations between the knowledge and educational level, duration of marriage and total numbers of pregnancy. There were negative correlation between the educational need and age, educational level, duration of marriage and total number of pregnancy. The above results showed the effects of antenatal education.

The purpose of this study was to identify the need for sex education in middle school students. Research subjects were 194 students who attended at 1 seminary in B city. The period of data collection was from July to August, 2001. Research instrument was composed of 61 structured items : 17 items related to demographics and sex, 44 items related to the need of sex education. Data were collected through the self-report method by the researcher and one trained assistant. The instrument developed by the researchers was employed to measure the need of sex education and it's Cronbach's alpha = 0.9349 in this research. Data were analyzed by frequency, percentage factor analysis and t-test using SPSS WIN Program. The findings of this study are summarized as follows: 1. The highest score among characteristics of sex was 'less interest about sex(42.6%)', 39.0% of subjects got information about sex from friends or elders. 52.3% of subjects consulted friends or elders about sex problems. The number of sex education lessons was 1-2 times in school during last semester(61.0%). Education by the school nurse was 35.9% to students. 43.1% of subjects announced moderate satisfaction on sex education. 2. The mean score of the need for sex education was 2.88+/-0.47(lowest value 1, highest 4). The most wanted sex education factor was 'chastity and sexual responsibility(3.33+/-0.67)' and followed by 'family and marriage(3.26+/-0.62)' and 'considerations of sex(3.07+/-0.69)'. Meanwhile, the lowest wanted sex education factor is 'physiological characteristics of male (2.47+/-0. 72)', followed by 'sexual behavior (2.49+/-0.75)'. The most wanted sex education item was 'the meaning of family(3.54+/-0.75)' and followed by 'the cautions to prevention from sexual abuse (3.49+/-0.78)' and sexual activity and responsibility(3.43+/-0.77)'. Meanwhile, the lowest wanted sex education item is 'masturbation(2.16+/-0.97)', followed by 'circumcision(2.32+/-0.97)'. 3. There is no significant difference between boys and girls in mean score about the need for sex education(t=- .715, p= .476). Sex education factor that girl-students wanted was 'physiological caracteristics of female'. Sex education factor that boy-students wanted was 'family and marriage'. Sex education items that girl-students wanted were 'cautions during menstruation', 'the relation of ovulation and pregnancy', 'caution to prevention from sexual abuse' and 'sexual behavior and responsibility'. Sex education items that boy-students wanted were 'the meaning of family', 'the importance of marriage', 'male's sex organs', 'a wet dream', 'the method to resolve sexual curiosity', 'sexual intercourse' and 'the connection with the other sex'. In conclusion, the mean score of the need for sex education is medium. The route to attain knowledge about sex and sexual problem is through consulting friends and elders. Therefore, it is necessary to give sex education that is suitable to the subject's needs and methods. Also, it is necessary to give differently strong point about sex education between female and male students.

Maternal and fetal effects of anesthesis for 781 cases ceasarean sections, performed during the past 10 years (1965~1974) in Hanil Hospital have been evaluated clinically. It is emphasized that the selection of anesthetic agents and methods, should depend upon the physical status of the patients and the ability of both obstetricians and anesthesiologists. Optimal results require effective teamwork amongst obstetricians, anesthesiologists and nursing personnel. The anesthesiologist should be fully infermed of obstetric aspects. A variety of anesthetic techniques can be used for elective and emergency Caesarean sections. Each technique has its own advantages, disadvantages and limitations which must be thoroughly understood for proper application. The choice of anesthesia should be determined primarily by the experience of the operator, the degree of ugency, condition of the monther and the fetus, the physiopathology, the skill of the obstetrician and the desires and the emotional state of the monther.

OBJECTIVES: To determine changes in the incidence and pattern of eclampsia in Il Sin Christian Hospital over a 46-year period. METHODS: Information was collected from medical records of the 1910 eclamptic patients among 233,613 deliveries in Il Sin Christian Hospital from Jan. 1 1953 to Dec. 31 1998. Incidence, presentation, and management of eclampsia were reviewed retrospectively, and maternal mortality rate and perinatal mortality rate were calculated. Statistical analysis was done by Chi-squared and Fisher's exact test through two by two tables looking at relative changes between each study period. RESULTS: The overall incidence of eclampsia was 81.8 per 10,000 deliveries. The incidence of eclampsia had increased from 137.3/10,000 in 1953-1962 to 278.4/10,000 in 1963-1972, but the rate had reduced to 6.5/10,000 in 1993-1998. There was a statistically significant fall in the rate of eclampsia every decade between 1973 and 1992, but there has been steady decrease in the last study period. Convulsion occurred antepartum in 54% of patients, intrapartum in 29% and postpartum in 17%. With the reduction in the proportion of antepartum eclampsia, there has been a relative increase in that of intrapartum and postpartum eclampsia. Maternal death occurred in 59 cases among eclampsia, and maternal mortality rate was 3.1%. Maternal mortality rate had significantly decresed from 11.1% in 1953-1962 to 3.8% in 1963-1972, and there has been no maternal death from eclampsia since 1986. Postpartum eclampsia had increased death risk compared with antepartum or intrapartum eclampsia. There were 280 cases of perinatal death and overall perinatal mortality rate was 144.1 per 1000 deliveries. There was a significant decrease in the rate from 243.2/1000 in 1953-1962 to 141.5/1000 in 1963-1972, but the rate has risen steadily since 1983. CONCLUSIONS: With the improvement in antenatal care and management of eclampsia, the incidence of eclampsia and its associated maternal mortality has decreased over the last 46 years. But eclampsia still remains a significant complication of pregnancy with high maternal and perinatal mortality.

OBJECTIVE: This study was performed to evaluate the effect of cervical cerclage and the clinical characteristics in incompetent internal os of the cervix (IIOC). METHODS: A study was conducted on 170 patients with IIOC, 199 cases of IIOC were admitted and treated with McDonald operation or modified Shirodkar operation at department of Obstetrics and Gynecology Il Sin Christian Hospital from January 1. 1994 to December 31. 1998, of this 14 cases were follow up lost, so 185 cases were analyzed. RESULTS: 1) The incidence of IIOC was 0.43%, 1 in 233 deliveries. 2) The mean age of IIOC patients was 30.54yrs old and the most frequent age group was in 30-34yrs old group (43.78%). 3) The average number of gravida and parity before operation were 4.71 and 1.62. & the success rate of operation was low at high gravida & parity. 4) The predisposing factor was previous history of dilation & curettage (43.75%), Cx. laceration after delivery (6.49%), midtrimester termination (4.32%) etc. 5) The operation methods were McDonald operation (91.35%) and modified Shirodkar operation (8.65%) & the success rate of McDonald operation and modified Shirodkar operation were 85.80% and 87.50%. 6) The successful fetal salvage rate was 85.95% and the highest success rate was 87.50% in 14-16 weeks of gestation group. 7) The more cervix dilate, the more failure occurred. 8) The causes of operation failure were premature rupture of membrane (50.00%), preterm labor (34.62%), FDIU, anomaly, APH etc. 9) The delivery methods after operation were vaginal delivery (65.54%), cesarean delivery (34.46%). CONCLUSION: The 14-16th weeks of gestation group & no cervical dilatation have higher success rate indicating that early diagnosis and appropriate timing of operation is associated with a greater operation success rate.

We report a 14-months-old infant who had obstructive jaundice caused by idiopathic fibrosing pancreatitis. Ultrasonography and abdominal computed tomography showed dilatation of the intrahepatic ducts, common bile duct, and the pancreatic duct. Diffuse swelling of the pancreas was also noted on CT. At laparotomy, the head portion of the pancreas revealed a stony hard consistency, and proliferation of fibrotic tissue was confirmed pathologically. Idiopathic fibrosing pancreatitis is a very rare disease entity in childhood, but should be considered in the differential diagnosis of obstructive jaundice in children who demonstrate bile duct and pancreatic duct dilatation and/or diffuse pancreas swelling.

For the past two decades, a huge number of genetic studies have been conducted to identify the genetic variants responsible for asthma risk. Several types of genetic and genomic approaches, including linkage analysis, candidate gene single nucleotide polymorphism studies, and whole genome-wide association studies have been applied. In this review article, the results of these approaches are summarized, and their limitations are discussed. Additionally, perspectives for applying upcoming new epigenetic or genomic technologies, such as copy number variation, are introduced to increase our understanding of new omic approaches to asthma genetics.

BACKGROUND: TNF-alpha is related to the generation of lung fibrosis in patients with UIP. The precise mechanism leading to lung fibrosis by TNF-alpha is unknown. However, the activation of a transcription factor like AP-1(down stream of c-jun N-terminal kinase, JNK) by TNF-alpha may be related to the induction of fibrogenic cytokines like PDGF or IGF-I. Furthermore, JNK was reported to be activated in the radiation-in-duced lung fibrosis model. This study examined JNK activity in patients with UIP. METHODS: The expression of phosphorous JNK(p-JNK), macrophage/moncoyte specific markers, CD68, and cytokeratin was evaluated by immunohistochemical (IHC) staining of lung tissues from patients with UIP and lung cancer. An in vitro kinase assay was performed with alveolar macrophages obtained by a bronchollung cancer. An in vitro kinase assay was performed with alvolar macrophages obrtained by a bronchol avleolar lavage from patients with UIP and healthy persons as the control. RESULTS: The IHC stain showed that p-JNK is expressed in the almost all of the alveolar macrophages and smooth muscle cells in patients with UIP. In case of the normal areas of the lung from patients with lung cancer, the alveolar macrophages showed little p-JNK expression. Interestingly, increased JNK activity was not found in the in vitro kinase assay of the alveolar macrophages obtained from both patients with UIP and healthy persons as the control. Furthermore, 10 ng/ml of TNF-alpha failed to increase the JNK activity of the alveolar macrophages in both patients with UIP and healthy people. CONCLUSION: The JNK was activated constitutionally in patients with UIP. However, the role of JNK in the pathogenesis of lung fibrosis needs to be clarified.
