1.Early T-cell precursor acute lymphoblastic leukemia: report of 1 case and review of literature

Objective:To improve the understanding of the diagnosis and treatment of early T-cell precursor acute lymphoblastic leukemia (ETP-ALL).Methods:The clinical data of a patient with ETP-ALL who was misdiagnosed as peripheral T-cell lymphoma-not otherwise specified (PTCL-NOS) admitted to the Second Hospital of Lanzhou University in October 2020 were retrospectively analyzed, and the relevant literature was reviewed.Results:The patient who presented "inguinal lymphadenopathy" as the first symptom underwent lymph node biopsy and pathological examination at local hospital, and he was diagnosed as PTCL-NOS according to the consultation of another 2 hospitals. After 2 courses of chemotherapy (CHOPE regimen, GLD regimen, unknown specific medication and dosage), the therapeutic efficacy was poor. For further diagnosis and treatment, this patient came to Lanzhou University Second Hospital. Flow cytometry found blast cells in the bone marrow, and then other related examinations were completed, he was finally diagnosed as ETP-ALL. The chemotherapy regimens of Hyper-CVAD and EA were alternatively used, progressive disease (PD) occurred after 3 courses of treatment, and chidamide was added in the 4th and 5th courses of treatment, the disease still progressed, and the patient died after follow-up. The disease course of the patient was about 12 months.Conclusions:ETP-ALL has unique immunophenotypic characteristics. ETP-ALL patients have a low remission rate after conventional induction therapy, high recurrence rate and poor prognosis. Currently, there is no effective standard treatment regimen, and allogeneic hematopoietic stem cell transplantation or timely addition of new drugs may improve the prognosis.
2.Analysis of gastrointestinal symptoms in 80 patients with coronavirus disease 2019

Objective:To investigate the clinical characteristics of gastrointestinal symptoms in patients with coronavirus disease 2019 (COVID-19) during the whole disease process, and provide reference for etiological diagnosis and treatment.Methods:The clinical data of patients with COVID-19 admitted in the Infectious Diseases Branch of the First Affiliated Hospital of University of Science and Technology of China from January 22nd, 2020 to March 8th, 2020 were analyzed retrospectively. According to whether there were gastrointestinal symptoms (poor appetite, nausea/vomiting and diarrhea), all patients were divided into gastrointestinal symptom group and asymptomatic group. The characteristics of gastrointestinal symptoms, such as poor appetite, nausea, vomiting and diarrhea were counted and analyzed, and the correlation between gastrointestinal symptoms and gender, age, basic diseases, disease severity, laboratory examination and drug treatment were analyzed.Results:A total of 80 COVID-19 patients were involved, 43 cases (53.8%) presented with poor appetite, 17 cases (21.3%) had nausea and vomiting, and 33 cases (41.3%) had diarrhea. Among them, 5 cases, 1 case and 4 cases respectively preformed poor appetite, nausea/vomiting and diarrhea before admission, while the others experienced gastrointestinal symptoms within 48 hours after admission. Duration of poor appetite, nausea/vomiting and diarrhea (days) of all patients were 5.3±2.1, 2.2±1.0 and 1.4±0.9, respectively. The patients with poor appetite were older than those without symptoms (years old: 48.2±17.6 vs. 39.3±15.1), albumin (Alb) level and the lymphocytes ratio were lower than those in asymptomatic group [Alb (g/L): 39.8 (35.7, 45.1) vs. 46.1 (42.6, 49.4), lymphocytes ratio: 0.19 (0.09, 0.28) vs. 0.28 (0.17, 0.35)], while the neutrophil ratio, the levels of C-reactive protein (CRP), D-dimer, and lactate dehydrogenase (LDH) were higher than those in asymptomatic group [the neutrophil ratio: 0.74 (0.61, 0.85) vs. 0.64 (0.52, 0.76), CRP (mg/L): 21.4 (3.9, 52.9) vs. 5.6 (2.4, 14.0), D-dimer (mg/L): 0.2 (0.2, 0.5) vs. 0.2 (0.1, 0.3), LDH (μmol·s -1·L -1): 4.49 (3.59, 5.19) vs. 3.12 (2.77, 4.90)]; at the same time, more traditional Chinese medicine was used in the patients with gastrointestinal symptoms [65.1% (28/43) vs. 40.5% (15/37), all P < 0.05]. In addition, 14 cases of 18 patients with cardiovascular diseases presented with poor appetite, 7 patients had nausea and vomiting symptoms. All of the 3 patients with chronic kidney disease presented with poor appetite, nausea and vomiting, and 2 of them had diarrhea. Conclusions:The gastrointestinal symptoms in patients with COVID-19 are common. Whether it is caused by the virus or related drugs, diet and mental conditions, clinicians should analyze the causes of these symptoms timely, and then provide a better treatment for patients with COVID-19.
3.Effects of Fluid Shear Stress on Gene Expression of Piezo1 in the Cells During Osteogenic Differentiation

Objective To investigate the gene expression of Piezo1 in four types of bone cells at different stages of osteogenic differentiation under fluid shear stress (FSS). Methods The mouse-derived mesenchymal stem cells (MSC), osteoblast-like cells MC3T3-E1, post-osteoblasts MLO-A5 and osteocytes MLO-Y4 were exposed to FSS at different magnitude (0.1, 1.1 Pa) with a custom-made cone-plate flow chamber for 0.5, 1, 3, 6, 12 h, respectively. The expression of Piezo1 mRNA was assessed by quantitative real-time polymerase chain reaction. Results Both Piezo1 and Piezo2 were expressed in four types of bone cells. The expression of Piezo1 was significantly up-regulated in all cells under FSS stimulation, and the expression level under 1.1 Pa FSS was significantly higher than that under 0.1 Pa FSS. In addition, the expression of Piezo1 in MSC, MC3T3-E1 and MLO-A5 cells increased to the highest level at 1 h under FSS stimulation. The expression of Piezo1 in MC3T3-E1 cells was much higher than that in the other three types of cells. Conclusions The expression of Piezo1 was related to the process of osteogenic differentiation, FSS level and loading time, and this research finding is of great significance to reveal the mechanism of mechanotransduction in bone tissues and to establish clinical treatment for bone diseases.
4.Immuno-effect of plasmacytoid dendritic cells on bacteria infection induced spontaneous remission of leukemia.

OBJECTIVETo explore the immuno-effect of plasmacytoid dendritic cells (pDC) on bacteria infection induced spontaneous remission (SR) of leukemia.
METHODSBoth pDC and myeloid dendritic cells (mDC) were isolated and purified from leukemic patient with SR and healthy donor by combination of immunomagnetic beads and flow cytometry. pDC were cultured in RPMI1640 medium and stimulated with different bacteria. The T cells proliferation was detected by MTT, and cytokine production by ELISA kits.
RESULTSThe human bacterial pathogen Staphylococcus aureus and Pseudomonas aeruginosa stimulation for 48 h resulted in the maturation of pDC with production of high quantity of IFN-α at (15.34 ± 2.91) ng/ml and (10.38 ± 1.41) ng/ml, respectively, comparing with that of negative group at (1.36 ± 0.13) ng/ml (P<0.01). Activated pDC could promote the differentiation of naive CD4⁺ T cells to Th1 cells with secretion of IFN-γ at (2.16 ± 0.37) ng/ml and (2.73 ± 1.11) ng/ml, respectively, comparing with that of positive control at (2.55 ± 0.23) ng/ml (P > 0.05). Activated pDC showed higher T cell stimulatory capacities [proliferation index (PI) was 4.36 and 4.05, respectively] than that of non-activated pDC (PI was 1.23 and 0.13, respectively) (P < 0.01).
CONCLUSIONStaphylococcus aureus and Pseudomonas aeruginosa activated pDC may play a key role in SR of leukemia following severe infections.

5.Substitutes for Endangered Animal Medicinal Materials: A Review

Endangered animal medicine is an important part of traditional Chinese medicine, which is distinctive in the treatment of diseases. At present, the rare and endangered medicinal materials such as tiger bone, rhinoceros horn, pangolin, antelope horn, bear bile are listed as national key protected animals, so their clinical application is limited, the current solution is mainly based on the ideas and methods of similar pharmacological effects, close genetic relationships, artificial breeding, and artificial synthesis to find and develop alternatives for endangered animal medicinal materials. Although artificially cultured bear bile and musk, and artificially synthesized tiger bone, bezoar and musk can solve the shortage of endangered animal medicines to a certain extent, there are still some problems such as difficult breakthroughs in breeding technology and incomplete recognition in the substitute industry. According to this, based on summarizing the existing substitutes for endangered animal medicines, our group proposed the concept of homology, homogeneity and equivalent of substitutes, and constructed a new idea to develop and evaluate substitutes by combining frontier biotechnology with multi-omics detection, so as to provide some support for protecting rare and endangered animals and solving the shortage of endangered animal medicines.
