3.Progress in regulation of adenosine system on sleep

The growing evidence suggests that adenosine system can regulate sleep-wake through the cooperation between inhibition of promoting wake system and excitement of promoting sleep system.Changes in adenosine level and adenosine receptor activity by pharmacological and genetic methods can affect sleep time and structure.However,the studies on whether the changes exist in the adenosine system in the patients with chronic insomnia disorder remain rare.In order to arouse the close attention to the clinical sleep study of adenosine system,in this paper,we present an overview of the current research progress regarding adenosine in general knowledge (adenosine's production,metabolism,peripheral distribution,circadian rhythm),the regulation of sleep and its mechanism,and the differences in regulation of sleep for various adenosine receptors.In the end,we propose some research directions of adenosine system under chronic insomnia after pointing out the existing issues currently.
4.Study on the correlation between serum IL-17 level and sleep-emotion disorder in type 2 diabetes mellitus patients complicated with peripheral neuropathy

Objective To evaluate the sleep and mood of patients with type 2 diabetes and peripheral neuropathy , and to explore whether the results were related to the level of interleukin-17 ( IL -17 ) . Methods 130 cases of type 2 diabetes diagnosed in Department of Endocrinology , First Affiliated Hospital of Medical University of Anhui from September 2015 to July 2016 were selected ,46 diabetes mellitus patients without peripheral neuropathy ( NDPN group ) and 84 patients with peripheral neuropathy ( DPN group ) were selected in the study.According to Toronto score (TCSS),DPN patients were divided into mild DPN group (DPN1 group,54 cases), medium and severe DPN group (DPN2 group,30 cases).The severity of insomnia and depressive symptoms were assessed by Pittsburgh Sleep Quality Index (PSQI) and 17-term Hamilton's Depression Scale (HAMD-17).The serum levels of IL-17 were measured .Results The scores of PSQI and HAMD-17 in the DPN group were signifi-cantly higher than those in the NDPN group,The score was (9.74 ±2.87),(10.85 ±2.20),and (12.03 ±2.71)respectively,the differences was statistically significant ( F =7.43, P =0.001 ), and the scores of PSQI and HAMD-17 in the DPN2 group were significantly higher than those in the DPN 1 group,the scores were (6.80 ± 2.74),(7.52 ±2.49),and (8.97 ±3.22) respectively,the differences was statistically significant (F=5.73,P=0.004 ) .The level of IL-17 in the DPN group was significantly higher than that in the NDPN group ,the differences was statistically significant(F=5.27,P=0.006).There was no statistically significant difference in IL -17 between the DPN1 group and the DPN2 group.IL-17 level of patients with PSQI >10 was significantly higher than that of patients with PSQI≤10 (t=5.52,P=0.021).The level of IL-17 in patients with HAMD-17≥7 and patients with HAMD-17 <7 had no statisticallysignificant difference .Conclusion Sleep and mood is closely related to the condition of patients with type 2 diabetes and peripheral neuropathy .Sleep condition may affect the level of IL -17 and the development of neuropathy .
5.Implantation metastasis of breast cancer in vacuum-assisted breast biopsy needle tract and the impact of neoadjuvant chemotherapy.

OBJECTIVETo study the incidence of implantation metastasis of breast cancer in vacuum-assisted breast biopsy (VABB) needle tract in Chinese patients and evaluate the effect of neoadjuvant chemotherapy on needle tract metastasis following VABB.
METHODSThe breast cancer patients with established diagnosis by VABB were divided into two groups to receive open surgery or neoadjuvant chemotherapy prior to open surgery. The incidence of needle tract metastasis, disease-free survival (DFS) and overall survival (OS) were compared between the two groups.
RESULTSA total of 214 patients were enrolled, among whom 94 directly underwent surgeries and 120 had neoadjuvant chemotherapy before surgery. The two groups showed no significant differences in the incidence of needle tract metastasis (3.2% vs 0.8%, P=0.206), DFS (P=0.221), or OS (P=0.531).
CONCLUSIONThe incidence of needle tract metastasis is low after VABB, and neoadjuvant chemotherapy does not increase this risk.

6.Association of white matter abnormality with facial emotion cognitive in the first-episode schizo-phrenic patients with never-medicated

Objective To explore the structure changes of white matter in the first-episode schizo-phrenic patients with never-medicated(FESZ)and the relationship between facial emotion perception and white matter(WM)integrity.Methods Sixty-three schizophrenic patients and thirty health control subjects underwent diffusion tensor imaging(DTI)scans.Voxel-based analysis was used to compared fractional ani-sotropy(FA)map between the two groups.Correlations were analyzed with pearson relative analysis between impaired facial emotion perception tested by facial emotion categorization(FEC)and severity of symptoms measured by the Positive and Negative Syndrome Scale(PANSS).Results (1)Compared with controls, FESZ patients showed overall decreased FA in WM of the body of left ventral frontal lobe((MNI(x,y,z):-18,26,-4;t=4.43)),right supramarginal gyrus((MNI(x,y,z):32,-50,26;t=4.27)),left middle oc-cipital gyral((MNI(x,y,z):-26,-60,0;t=4.89)),right middle occipital gyral((MNI(x,y,z):28,-70, 14;t=4.18)),left fusiform gyrus((MNI(x,y,z):-40,-50,0;t=3.92)),left cerebellum anterior lobe ((MNI(x,y,z):-32,-56,-28;t=4.57)),right parahippocampa gyrus1((MNI(x,y,z):32,-10,-14;t=4.16)),right parahippocampa gyrus2((MNI(x,y,z):16,-6,-14;t=4.56)),left anterior cingulate ((MNI(x,y,z):-2,4,-6;t=4.41)),left extra-nuclear((MNI(x,y,z):-2,-10,-6;t=4.44)),right thalamus((MNI(x,y,z):10,-10,2;t=4.20)),left thalamus((MNI(x,y,z):-22,-28,12;t=4.01)), and right caudate((MNI(x,y,z):14,12,8;t=4.87)).(2)Compared with controls,the patients with schizo-phrenia showed a higher shift point and a steeper slope than control subjects in FEC.Correlational analysis re-vealed that the negative correlations were found between the slope and negative factor(r=-0.298,P=0.036),between positive factor and the FA value in WM of the right middle occipital gyral(r=-0.322,P=0.023)and the left middle occipital gyral(r=-0.288,P=0.043),and between the FA value in the left cere-bellum anterior lobe and shift point(r=-0.393,P=0.005),but the positive correlation was found between disorganized/concrete factor and the FA value in the right parahippocampal gyrus(r=0.429,P=0.002).Con-clusions There are extensive microstructural abnormalities in WM of patients with FESZ.Disrupted WM in-tegrity in these regions may constitute a potential neural pathological basis for the facial emotion perception impairments in schizophrenia.
7. Changed serum levels of neurotrophic factors and their correlations with sleep and cognition in patients with chronic insomnia disorder

Objective:
To explore serum levels of brain-derived neurotrophic factor (BDNF) and glial-derived neurotrophic factor (GDNF), and whether changes of BDNF and GDNF are correlated with sleep quality and cognitive function in patients with chronic insomnia disorder (CID).
Methods:
Fifty-seven CID patients in the Department of Sleep Disorders, Chaohu Hospital of Anhui Medical University and 30 healthy controls were enrolled from May 2017 to July 2018. Pittsburgh Sleep Quality Index (PSQI) was used to assess the degree of insomnia severity (some CID patients were monitored by overnight polysomnography). Montreal Cognitive Assessment (MoCA) scale and Nine-Box Maze were used to assess general cognitive function and specific memory function, respectively. The serum levels of BDNF and GDNF were detected using ELISA.
Results:
