1.The Influence of Bacteria Lipopolysaccharide on the Cytokine Production of Flour Mite Allergic Rhinitis Patients

Objective To explore the influence of bacteria lipopolysaccharide on the cytokine production of flour mite allergic rhinitis patients. Methods 8 cases with perennial allergic rhinitis were selected. Then the heparin anticoagulation venous blood was taken from the same patient and was added to fetal calf serum,with ultimate volume 1 ml. The two experimental groups were added 10μg LPS and 1 μg LPS respectively, forming the high LPS dose group and low LPS dose group,while the control group was added nothing. Results The contents of IL-4、IL-5、IL-10、IL-12、IFN-γwere tested by enzyme-linked immunosorbent assay. The findings were that there was significant difference in the productive contents of IL-4、IL-5、IL-10、IL-12 and no significant difference in that of IFN-γ between the high LPS dose group and the low LPS dose group. When compared in two groups,significant difference was found in the productive contents of IL-4 ,IL-10 and IL-12 between the two experimental groups and between the high LPS dose group and the control group, while no obvious difference existed between the low LPS group and control group. However, significant difference was altogether found in the productive content of IL-5 when compared one with another among the three groups,and the result was that low dose of LPS obviously promoted the production of cytokine while high dose of LPS restrained the production of cytokine; no significant difference was found in the production of IFN-γ in all comparisons between the three groups. Conclusion This research, through the experiments in vitro, proved that LPS could effectively adjust the cytokine production of perennial flour mite allergic rhinitis patients and the balance of Th1/Th2 ,whose effect depends on the dose of LPS. This research suggested that the practical future in which special cytokine target treatment will be adopted on perennial flour mite allergic rhinitis patients.
2.Pandanus tectorius derived caffeoylquinic acids inhibit lipid accumulation in HepG2 hepatoma cells through regulation of gene expression involved in lipid metabolism.

The fruit of Pandanus tectorius (PTF) has a long history of use as a folk medicine to treat hyperlipidemia in Hainan province, South China. Our previous studies have shown that the n-butanol extract of PTF is rich in caffeoylquinic acids and has an adequate therapeutic effect on dyslipidemic animals induced by high-fat diet. In this work, seven caffeoylquinic acids isolated from PTF were screened for the lipid-lowering activity in HepG2 hepatoma cells. Oil-Red O staining, microscopy and intracellular triglyceride (TG) and total cholesterol (TC) quantification showed that 3-O-caffeoylquinic acid (3-CQA), 3, 5-di-O-caffeoylquinic acid (3,5-CQA), and 3,4,5-tri-O-caffeoylquinic acid (3,4,5-CQA) significantly inhibited lipid accumulation induced by oleic acid and decreased intracellular levels of TC and TG in a dose-dependent manner. These three caffeoylquinic acids showed no significant cytotoxicity at concentrations of 1 -50 μmol x L(-1) as determined by MTT assay. Realtime quantitative PCR revealed that 3-CQA and 3, 5-CQA significantly increased the expression of lipid oxidation-related genes PPARα, CPT-1 and ACOX1 while 3-CQA, 3, 5-CQA and 3,4,5-CQA decreased the expression of lipogenic genes SREBP-1c, SREBP-2, HMGR, ACC, FAS. Overall, 3-CQA, 3, 5-CQA and 3, 4, 5-CQA may be the principal hypolipidemic components in PTF which can decrease intracellular lipid accumulation through up-regulating the expression of lipid oxidative genes and down-regulating the expression of lipogenic genes.
3.Surveillance and analysis of death causes of residents in a community of Shanghai from 2017 through 2019

Objective:To determine the mortality, composition and trend of death causes of residents in Changxing community, Chongming District, Shanghai from 2017 through 2019, and provide scientific evidence for disease prevention and control strategies. Methods:Mortality data of household registered population in this community during January 1, 2017, and December 31, 2019, were collected. Causes of death were coded according to the ICD-10 and then analyzed by descriptive epidemiological method. Results:From 2017 through 2019, crude annual mortality of household registered residents in Changxing Community was 816.32/105, 825.01/105, and 770.16/105, respectively. The average annual mortality was 803.82/105. The leading death causes were cancer, circulatory disease, respiratory disease, and injury, poisoning and other external causes. Male mortality was higher than female mortality in all years. Conclusion:Main death causes of residents in Changxing Community are chronic non-communicable diseases, followed by external causes of injuries and poisoning. Our study provides scientific evidence for the improvement of health strategies by the local health administration, which warrants targeted health education measures to reduce the morbidity and mortality of chronic diseases and improve the health.
5. In situ suturing technique for severe amputation of facial composite tissues in 14 cases

Objective:
To report the method and effect of in situ suturing technique for severe amputation of facial composite tissues.
Methods:
We in situ sutured severely amputated composite tissues of facial region in 14 cases in emergency by vessels and nerves anastomosis from May 2004 to December 2017.
Results:
All replanted composite tissues survived in 13 cases. Edge necrosis of auricle occurred in 1 case because of serious contusion and it was cured after dressing changed. The color and appearance of the replanted composite tissues were close to normal after 3 to 48 months′ follow-up.
Conclusions
In situ suturing technique in emergency is the ideal treatment for severe amputation of composite tissues of facial region.
6.Incidence of sedation-related adverse events durine microwave ablation of liver cancer under deep sedation and analysis of its risk factors

Objective To analyze the the incidence of sedation-related adverse events occurring during percutaneous microwave ablation(PMWA)under deep sedation,and to identify the risk factors for hypoxemia,hypertension and body movement response.Methods The clinical data of 506 patients with liver cancer,who received PMWA under deep sedation at the First Affiliated Hospital of Nanjing Medical University of China between December 2018 and December 2022,were retrospectively analyzed.The sedation-related adverse events including hypoxemia,hypertension and body movement response were recorded.Univariate analysis and multivariate logistic regression analysis were used to assess the risk factors for hypoxemia,hypertension and body movement response.Results After excluding 20 patients whose clinical data were incomplete,the final study sample consisted of 486 patients.After deep sedation,hypotension occurred in 5 patients(1%),and the incidences of hypoxemia,hypertension and body movement response were 13.8%,13.2%and 25.5%respectively.No patient required urgent endotracheal intubation and no cardiovascular complications occurred.The body movement response did not increase the complications of PMWA.Multivariate analysis revealed that age ≥65 years(OR=1.034,P=0.033),BMI>30 kg/m2(OR=1.865,P=0.039),and Child-Pugh grade B/C(OR=1.113,P=0.046)were the risk factors for hypoxemia.The ablation zone contacting the parietal peritoneum(OR=2.536,P<0.01),ablation zone contacting the diaphragm(OR=1.795,P=0.001),and ablation zone contacting the adrenal gland(OR=1.581,P<0.01)were the risk factors for hypertension.The ablation zone contacted the parietal peritoneum(OR=2.831,P<0.01)and ablation zone contacted the diaphragm(OR=2.213,P=0.026)were the risk factors for body movement response.Conclusion For the treatment of liver cancer,PMWA under deep sedation is clinically safe and effective.The age ≥ 65 years,BMI>30kg/m2,and Child-Pugh grade B/C are the risk factors for hypoxemia.The ablation zone contacting the parietal peritoneum,the diaphragm or the adrenal gland are the risk factors for hypertension.The ablation zone contacting the parietal peritoneum or the diaphragm are the risk factors for body movement response.(J Intervent Radiol,2023,32:1202-1206)
7. Using the forearm proximal radial inferior cubital artery perforator flap to repair the wound defects of anterior elbow

Objective:
To evaluate the design and clinical application of forearm proximal radial inferior cubital artery perforator flap.
Methods:
The clinical data of 11 patients with defects of anterior elbow from June 2013 to June 2018 were analyzed retrospectively, in which including 8 male and 3 female, mean age is 51.5 years old, ranged from 48 to 76 years old. The sizes of soft tissue defects ranged from 7.5 cm×4.5 cm to 14 cm×7 cm. Forearm proximal inferior cubital artery perforator flaps were used to repair the defects according to flap their location, size and shape, in which the largest is 15 cm×10 cm and the smallest is 7 cm×5 cm. To evaluate the effect according to flap survival, infection control, elasticity and color, appearance, the scar of the donor site, cutaneous sense, and satisfaction of patients. The elbow efficacy and function were evaluated according to Mayo criteria and the swelling degree of flap was also evaluated.
Results:
All the flaps survived in 11 cases, the incisions in donor and recipient site were primary healing. The survival of flaps is good and the appearance, color and elasticity are close to normal, on the other hand, the scar at the donor site is small after 6 to 36 months follow-up. The efficacy is satisfied in 8 cases, general in 3 cases and no dissatisfactory. The function of elbow is excellent in 4 cases, good in 6 cases and general in 1 case. Skin swelling rating in early stage is 1st degree in 6 cases, 2nd degree in 3 cases, 3rd degree in 2 cases; in later stage is 1st degree in 8 cases, 2nd degree in 2 cases, 3rd degree in 1 case.
Conclusions
The anatomic position of forearm proximal radial inferior cubital artery perforator flap is constant, the blood supply is good, and the operation method is simple and safe which could be a new supplementary method and be available for clinical selection to repair the soft tissue defects of elbow.

9.Quantification of microbial DNA in laboratory environment during DNA extraction.

Metagenomic sequencing provides a powerful tool for microbial research. However, traditional experimental DNA extraction process will inevitably mix with environmental microorganisms which float in the air. It is still unclear whether the mixed environmental microbial DNA will heavily affect the metagenomic results of samples with extremely low microbial content. In this study, we first collected environmental bacteria in the laboratory and quantified the mixed environmental microbial DNA content during DNA extraction based on a qPCR-based quantification assay. We then extracted DNA from pure water in order to determine the mixed microbial taxons during extraction under open environment. At last, we extracted total DNA from a skin sample in a Biosafety cabinet or under open laboratory environment, to assess the impact of the mixed environmental microorganisms on the metagenomic results. Our results showed that DNA extraction under open laboratory environment in Beijing region resulted in 28.9 pg contaminant, which may accout for 30% of total DNA amount from skin samples. Metagenomic analysis revealed that the main incorporated environmental taxons were Cutibacterium acnes and Escherichia coli. Tens of environmental bacteria were foisted in the skin DNA samples, which largely decreased the relative abundance of dominant species and thus deteriorated the result accuracy. Therefore, analyzing microbial composition of samples with extremely low DNA content should better performed under aseptic environment.

10.Modulative effect of Physalis alkekengi on both gut bacterial and fungal micro-ecosystem.

OBJECTIVE:
Gut microbiome is an intricate micro-ecosystem mediating the human health and drug efficacy. Physalis alkekengi (PAL) is an edible and time-honored traditional Chinese medicine. Several pharmacological effects of PAL have been verified and gut bacteria are implied in its therapeutic actions. However, the detailed modulation of PAL on gut bacterial species and on gut fungi remains largely unknown. We, therefore, designed a preliminary experiment in normal mice to reveal the modulation effect of PAL on both gut bacteria and fungi, and explore the interaction between them.
METHODS:
Herein, the aqueous extract of PAL was orally administrated to normal C57BL/6 mice for four weeks. The full-length 16S rRNA and ITS1/2 gene sequencing were explored to detect the taxa of gut bacteria and gut fungi after PAL treatment, respectively.
RESULTS:
Oral administration of PAL notably enriched anti-inflammatory bacterial species such as Duncaniella spp. and Kineothrix alysoides, whereas decreased pro-inflammatory species such as Mucispirillum schaedleri. Simultaneously, PAL increased the abundance of gut fungi Aspergillus ochraceus, Cladosporium sp. and Alternaria sp., and decreased Penicillium janthinellum. Correlation network analysis identified two co-existing microbial groups (groups 1 and 2) that were negatively associated with each other. The group 1 comprised PAL-enriched bacteria and fungi, while group 2 was mainly normal chow-enriched bacteria and fungi. In group 1, Antrodia monomitica, Aspergillus clavatus, Mortierella kuhlmanii and Sarcinomyces sp. MA 4787 were positively correlated with Bifidobacterium globosum, Romboutsia ilealis and so on. In group 2, Chaetomium subspirilliferum, Septoria orchidearum and Cephaliophora tropica were positively related to Lactobacillus spp.
CONCLUSION
Altogether, this preliminary study first demonstrated the modulation effect of PAL on both gut bacteria and gut fungi, which may shed light on the elucidation of PAL's pharmacological mechanism.
