1.Correlation between the expression of high mobility group box 1 and receptor for advanced glycation end products and the onset of pre-eclampsia

Objective To evaluate different expressions of high mobility group box 1(HMGB1)and receptor for advanced glycation end products(RAGE)in placentas and their relationship with preeclampsia.Methods Fifteen early-onset pre-eclaraptic women(early-onset pre-eclampsia group),22 late-onset pre-eclamptic women(late-onset pre-eclampsia group)and 12 normotensive women(control group)in the third trimester were recruited at the Shengjing Hospital of China Medical University from March 2006 to March 2007.The localization and levels of HMGB1 and RAGE in placentas of the three groups were detected by the strept avidin biotin-peroxidose method.Results (1)Immunoreactivities to HMGB1:positive immnnostaining for HMGB1 was observed in trophoblast,macrophages,decidual cells,vascular muscle cells,endothelial cells and placental mesenchymal cells in the placentas from the pre-eclamptic women,while a low level of immunoreactivities was observed in the placentas from healthy pregnancies;the staining was observed within both the nuclei and the cytoplasm,mainly in the cytoplasm.The cytotrophoblast,especially the nuclei was extensively positive for HMGB1 in early-onset pre-eclampsia. (2)Immunoreactivities to RAGE:positive immunostaining for HMGB1 was observed in syncytiotrophoblast,macrophages and endothelial cells in the placentas from the preeclamptic women,while a low level of immunoreactivities was observed in the placentas from healthy pregnancies:the staining was in the cytoplasm and(or)cell membrane.The trophoblast was extensively positive for RAGE in early-onset pre-eclampsia.(3)Positive rate of HMGB1 expression:the expression of HMGB1 in early-onset group(73%,11/15)and late-onset group(64%,14/22)was significantly higher than that in normal group(17%,2/12;P<0.05),but no significant difference was found in early-onset group and late-onset group(P>0.05).(4)Positive rate of RAGE expression:the expression of RAGE in early-onset group(80%,12/15)and late-onset group (82%,18/22)was significantly higher than that in normal group(25%,3/12;P<0.05),but no significant difference was found in early-onset group and late-onset group(P>0.05).Conclusions The increased expression of HMGB1 and RACE in the placenta may play an important role in the pathogenesis of pre-eclampsis.The different locations may be associated with the occurrence of different onset types of pre-eclampsia.
2.Clinical significance of KiSS-1 and matrix metalloproteinase-9 expression in trophoblasts of women with preeclampsia and their relation to perinatal outcome of neonates

Objective To investigate the role of KiSS-1 and matrix metalloproteinase(MMP)9 in trophoblasts in the pathogenesis of preeclampsia and their relation to perinatal outcome of neonates. Methods RT-PCR and western blot analyses were used to detect the MMP-9 and KiSS-1 expression levels in trophoblast of 40 patients with preeclampsia (15 cases of mild and 25 cases of severe preeclampsia)(preeclampsia group) and 20 cases of term pregnancy (normal pregnancy group) and their correlations with symptoms and perinatal outcome of neonates were analyzed. Results (1) The KiSS-1mRNA and metastin expression levels in trophoblasts of preeclampsia group were 1.73?0.24 (A value) and (78.4?8.0) ?g/ 100 ?g total protein separately,those of mild preeclampsia were (1.50?0.15) and (72.4?6.9) ?g/ 100 ?g total protein , and severe preeclampsia were (1.87?0.20) and (83.52?3.57) ?g/100 ?g total protein , which were all significantly higher than those of normal pregnancy group [1.24?0.25, P
3.Effect of basic fibroblast growth factor on the gene expression of epidermal growth factor receptor by periodontal ligament cells in culture.

OBJECTIVETo study the effect of basic fibroblast growth factor (bFGF) on the gene expression of epidermal growth factor receptor (EGFR) by periodontal ligament cells (PDLC) in culture and discuss the effect of bFGF in cell differentiation and periodontal regeneration.
METHODSHuman PDLC were cultured in vitro and cell clone was obtained by the method of limiting dilution. PDLC were stimulated by bFGF, and then gene expression of EGFR was assessed by reverse transcription-polymerase chain reaction (RT-PCR).
RESULTSThe mRNA expression of EGFR was promoted by bFGF and the effect was dose-dependent.
CONCLUSIONThe promotion effect of the mRNA expression of EGFR in PDLC may be one of important factors which participate in the healing process of periodontitis and provide partly theoretical basis of cell differentiation and periodontal regeneration.

4.Mutiple-slice spiral CT diagnosis of atypical intraabdominal hernia.

OBJECTIVETo discuss the value of multiple-slice spiral CT diagnosis of atypical intraabdominal hernia.
METHODSThe clinical and CT findings of 16 cases of atypical intraabdominal hernia confirmed surgically were retrospectively analysed.
RESULTSIn all the 16 cases, the contents of hernia were small bowels and the mesentery. Nine cases were caused by the adhesion after abdominal operations or infection, 6 by the mesenteric foramen, and 1 by the gap of pelvic peritoneum. The main CT findings were as follows:(1)the obstructed small bowels gathered abnormally and showed cluster shape(9 cases); the walls of the bowels thickened with edema and showed "target" sign with exudate in the neighboring spaces(5 cases); other part of the abdominal cavity became empty for lack of small bowels(4 cases).(2) Abnormal arrangement of the branches of mesenteric vessel, which appeared gathered or pulled or rigid and displacement of the main branches to left or right(12 cases). Thickened mesenteric vessel (4 cases): torsion of mesentery with "whirlpool" sign (3 cases). (3) With the help of multiplanar reformation, 5 cases showed the evidence of hernia rings. In the ring area, there were gathered or pulled or rigid and radiating mesenteric vessel and the dilated or effusion bowels in cluster arrangement forming "parachute" and "bundle of balloons" sign.
CONCLUSIONCT manifestations of atypical intraabdominal hernia has some specific characteristics, which is of important value for clinical diagnosis and treatment.

5.Heterotopic complex membrane induces bone formation.

OBJECTIVEThe composite membrane was made by recombinant human bone morphogenetic protein-2 (rhBMP-2), collagen (Co), polylactic-co-glycolic acid (PLGA). To observe the ectopic bone formation and evaluate the capability of rhBMP-2/Co/PLGA complex membrane.
METHODS48 male Kunming adult mice were divided into two groups randomly: Co/PLGA complex membrane group and rhBMP-2/Co/PLGA complex membrane group. After etherized, an incision was made on the outer muscle and two groups of complex membranes were implanted into the right side hind limb muscle pouch. 8 mice from each group were sacrificed at 7, 14, 28 d. The hind limbs were removed, examined by soft X-ray, HE staining and light microscope examination.
RESULTSWounds healed well, no infections and rejections were observed. In the Co/PLGA group, partial implanted membrane degradated and absorbed at 7 d. The membrane collagen fiber appeared loose at 14 d. The membrane lost its intact to disrupted at 28 d. No ectopic bone formation was found. In the rhBMP-2/Co/PLGA group, ectopic bone formation was found by eye, soft X-ray and histological examinations. The rhBMP-2/Co/PLGA complex membrane degraded slower than Co/PLGA complex membrane.
CONCLUSIONCo/PLGA can provide a carrier for rhBMP-2 to have stronger effect of biological activity.

6.Papillon-Lefevre syndrome: a case report.

Papillon-Lefevre syndrome (PLS) is an extremely rare inherited disease as an autosomal recessive trait. The disorder is characterized by diffuse palmoplantar keratoderma and premature loss of both deciduous and permanent teeth. This paper described a case of PLS with classic clinical features and briefly reviewed the relevant literature.

OBJECTIVETo study the effect of basic fibroblast growth factor on the mRNA expression of beta1 integrin subunit by periodontal ligament fibroblasts in culture; to discuss the effect of basic fibroblast growth factor in periodontal regeneration.
METHODSHuman periodontal ligament fibroblasts were cultured and stimulated by basic fibroblast growth factor (0.1, 1.0, 10.0 ng x mL(-1)) for 24, 48, 72 h respectively, and then mRNA expression of beta1 integrin subunit was assessed by fluorescent quantitative real-time reverse transcription polymerase chain reaction.
RESULTSBasic fibroblast growth factor enhanced the mRNA expression of beta1 integrin subunit, and there was optimal effect when the concentration of basic fibroblast growth factor was 1.0 ng x mL(-1) at 24, 48, 72 h respectively; the mRNA expression of beta1 integrin subunit at 72 h was higher than that at 24, 48 h.
CONCLUSIONBasic fibroblast growth factor can strengthen human periodontal ligament fibroblasts' adhesion and may be one of important factors which participate in the periodontal regeneration.

8.Finite element study of maxillary Le Fort-I osteotomy with rigid internal fixation.

OBJECTIVETo study the biomechanical characteristic of maxillary Le fort- I osteotomy with rigid internal fixation (RIF) , so as to choose best fixation method.
METHODSThe 3-dimensional finite element models of maxillary Le Fort-I osteotomy with 9 kinds of RIF methods were established. Then the models were divided into three groups to calculate the stress distribution of the maxilla and the displacement of bone segment under 3 kinds of occlusion condition. The fixation stability of the different RIF methods was evaluated.
RESULTSUnder the incisor occlusion condition, the stress of the cranio maxillary complex transmits mainly along the nasal-maxillary buttress. Under the premolar and molar occlusion condition, the stress transmits along the alveolar process first, then turns to the nasal-maxillary and zygomatic-maxillary buttress. The focused stress position of the internal fixation system is at the connection between the screws and the plate and at the plate near the osteotomy line. Under the premolar occlusion condition, the displacement of bone segment with different RIF methods was (in a decreasing order) 0.396509 mm (with bio-absorbable plate), 0.148393 mm (with micro-plate ), 0.078436 mm (with mini-plate) in group 1; 0.188791 mm (fixing at the nasal-maxillary buttress), 0.121718 mm (fixing at the zygomatic-maxillary buttress), 0.078436 mm (fixing at the both buttress) in group 2; 0.091023 mm (with straight plate), 0.078436 mm (with L shape plate), 0.072450 mm (with Y shape plate), 0.065617 mm (with T shape plate) in group 3.
CONCLUSIONSThe fixation stability of using the bio-absorbable plate in Le Fort-I osteotomy is less stable than using the titanium plate. Fixing at the zygomatic-maxillary buttress is more stable than at the naso-maxillary buttress. The fixation stability is different by using different shapes of plates.

9.Establishment and primary analysis of the three-dimensional finite element model of mandibular bilateral sagittal split ramus osteotomy with rigid internal fixation.

OBJECTIVEEstablish the three-dimensional finite element models of mandibular bilateral sagittal split ramus osteotomy (BSSRO) with rigid internal fixation (RIF), for further study of BSSRO.
METHODSCT scanned technology and the finite element software (ANSYS) were used to establish the original three-dimensional model of mandible, modify the model to animate the BSSRO, then establish the model of RIF, last mesh the model to establish the finite element model of BSSRO with RIF. Apply 100 N occlusion force at the central incisor; calculate the stress distribution of the mandible and the RIF.
RESULTSThree-dimensional finite element models of BSSRO with RIF were established, such as miniplate model, bicortical fixation screw model. When biting with the incisor and fixed with upper plate, the stress of the medial screw position of the distal and medial segment of mandible is high. When fixed with bicortical fixation screw, the highest stress position located at the internal surface of the medial screw' s position of the distal segment of mandible.
CONCLUSIONSThe mentioned methods proved feasible in establishing the finite element models of BSSRO with RIF . The models can be applied to the study of BSSRO with RIF.

10.A clinical report of 58 cases of split liver transplantation

Objective To summarize the clinical experience in 58 cases of split liver transplantation (SLT).Methods A retrospective analysis was conducted on 58 cases of SLT during June 2006 to January 2011.There were 13 cases performed at the first phase (2006.6-2008.12),and 45 cases at the second phase (2009.1 2011.1). The survival rate of patients,recovery of liver function,re-transplantation rate,incidence of vascular complications and biliary complications were observed,and the causes of death were analyzed.Results The median follow-up time of all the patients was 11.4 months (0-48 months).The 1- and 2-year cumulative survival rate was 77.4% and 68.3% respectively,re-transplantation rate was 6.9%,the incidence of vessel complications was 13.8%,and biliary complication rate was 32.1%.Fifteen cases died,including 8 deaths which were related to surgical complications.Conclusion With the donor split technology improvements and refinements in partial liver transplantation, the survival rate of SLT recipients is significantly increased,but selection of recipients is still the key factor that impacts survival rate of recipients receiving SLT.SLT can expand the resource of liver donors,and adequate selection of recipients can obtain better results.
