2.Application of dizziness handicap inventory in evaluation of benign paroxysmal positional vertigo

Thirty patients with objective benign paroxysmal positional vertigo (BPPV) (objective group) and 30 patients with subjective BPPV (subjective group) visiting Department of Neurology from January 2014 to June 2015 were enrolled in the study.There were no significant differences in semicircular canal,gender,age,related factors of secondary onset (P > 0.05).Patients were evaluated with dizziness handicap inventory (DHI) before and after the maneuver therapy.The average DHI subscale scores before treatment of all patients were higher than those after treatment (11.8 ± 2.8 vs.2.5 ± 1.8,P < 0.05);there was 11.9 ± 3.0 vs.2.5 ± 2.1 (P < 0.05) for objective group and 11.6 ± 2.6 vs.2.5 ± 1.6 (P < 0.05) for subjective group.The scores of PI,F5,P11,P13,P25 (mean rank order value) after treatment were decreased compared to corresponding initial scores in two groups(objective group:21.30 vs.39.70,19.55 vs.41.45,19.00 vs.42.00,18.65 vs.42.35,15.90 vs.45.10,respectively,subjective group:20.93 vs.40.07,18.07 vs.42.93,19.77 vs.41.23,18.57 vs.42.43,16.47 vs.44.53,respectively;all P < 0.05).The results indicate that maneuver therapy is effective both for objective and subjective BPPV patients;and subscale of DHI can be used to evaluate the degree of illness in patients with BPPV.
3.Correlation between vasoactive intestinal peptide and expression of NKG2D, adaptor protein 10 and extracellular signal-regulated kinase in inflammatory cells of gastric adenocarcinoma

Objective To investigate the expression of NKG2D and related signaling molecules adaptor protein(DAP10) and extracellular signal-regulated kinase(ERK) in the inflammatory cells of gastric adenocarcinoma and para-carcinoma tissues,the effects of vasoactive intestinal peptide(VIP) on the expression of NKG2D,DAP10 and ERK in natural killer(NK) cells of healthy volunteers.Methods The expression of NKG2D,DAP10 and ERK in the inflammatory cells of 36 gastric adenocarcinoma and para-carcinoma tissues were detected by immunohistochemistry.The peripheral blood NK cells of healthy volunteers were isolated and purified with CDC method.The expressions of NKG2D,DAP10 and ERK in NK cells were determined by immunocytochemistry and reverse transcription-polymerase chain reaction(RT-PCR) methods before and after VIP intervention.The ttest was used for comparison between two groups and rank transformation nonparametric test (Kruskal-Wallis H test) for count data analysis.Results The expression levels of NKG2D,DAP10 and ERK in the inflammatory cells of gastric adenocarcinoma were significantly lower than those of para-carcinoma tissues (H=30.640,24.910,20.320,all P<0.01).The expression levels of NKG2D,DAP10 and ERK were much lower in poorly differentiated gastric adenocarcinoma (H =4.049,11.830,8.118),at stage Ⅲ-Ⅳ (H=4.275,11.560,7.686),and the expression levels of NKG2D,ERK were lower with lymph nodes metastasis (H=6.513,6.064,all P<0.05).The expression levels of NKG2D,DAP10 and ERK in gastric adenocarcinoma tissues and inflammatory cells were not correlated with gender,age and the location of lesions (all P>0.05).The expression levels of NKG2D,DAP10 and ERK in NK cells was decreased after VIP intervention (protein:H=12.438,4.798,13.745,mRNA:F=337.640,638.579,1055.015,all P<0.05).Conclusion Gastric adenocarcinoma may downregulate the expression of NKG2D,DAP10 and ERK in NK cells through VIP secretion,which may be involved in the immune escape of gastric cancer.
4.Subcutaneously transplanted human pancreatic cancer tissue fragments in a mouse model

Objective To establish a mouse model by transplanting subcutaneously human pancreatic cancer tissue fragments.Methods Surgically resected pancreatic cancer tissue fragments from patients were transplanted into NOD/SCID mice subcutaneously,and then the growth of the tumor and transplanting it into the next generation were observed.The growth rate,HE staining and immunohistochemistry staining of Ki67 and VEGF were compared.Results We have obtained 13 cases ofpancreatic cancer tissues and 6 cases of biopsy specimens.In 5 cases transplantation was successful,in onemouse model passing to fourth generation,in 4 models to second generation.With the increase of generaions,tumor growth accelerated.HE staining showed later passage cells behavior in an identical manner as the primary cells.Immunohistochemistry staining showed that expressions of Ki67 and VEGF are increasing.Conclusions Through transplanting human pancreatic cancer tissue fragments directly,we have constructed mouse model of pancreatic cancer successfully.With the passage of subculture,the malignant degree and invasiveness may increase.
5.Management of inoperable advanced pancreatic head carcinoma without gastric outlet obstruction: an analysis of 441 patients

Objective To explore the optimal management strategies for unresectable advanced pancreatic head carcinoma without preoperative gastric outlet obstruction(GOO).Methods Clinical data of 441 cases of advanced pancreatic head carcinoma without GOO undergoing surgery from Jan 2001 to Dec 2010 were analyzed retrospectively.Results Among the 441 cases of advanced pancreatic head carcinoma without GOO,101 patients received simple Roux-en-Y cholecystojejunostomy (group A),133 patients received simple Roux-en-Y choledochojejunostomy (group B),83 patients received Roux-en-Y cholecystojejunostomy combined gastrojejunostomy(group C) and the other 124 patients received Roux-en-Y choledochojejunostomy combined gastrojejunostomy (group D).The postoperative recurrent obstructive jaundice rates were 7.9% and 6.0% in group A and C,respectively; the postoperative de novo GOO rates were 8.9% and 8.3% in group A and B,respectively; there were no differences in median survivals among the four groups (F =1.933,P =0.123).Conclusions Choledochojejunostomy is effective for the reduction of recurrent obstructive jaundice for advanced pancreatic head carcinoma patients without GOO,combined prophylactic gastrojejunostomy during surgical biliary drainage could decrease the rate of postoperative GO0.Cholecystojejunostomy could be only applied for patients with poor health or when choledochojejunostomy is a taboo.
7.The influence of rHuEPO on inflammatory factors in the process of renal interstitial fibrosis and its possible mechanism

Objective Clinical treatment can delay the development of renal interstitial fibrosis , but it can not reverse renal dysfuntion.The article was to discuss the influence of recombinant human erythropoietin ( rHuEPO ) on inflammatory factors in the process of renal interstitial fibrosis and its possible mechanism . Methods The vitro cultured HK-2 cells were randomized into 7 groups:the blank control group , rHuEPO control group ( addition of 20U/mL rHuEPO), albumin stimulation group (addition of 5mg/mL albumin), 5mg/mL rHuEPO intervention group (5mg/mL albumin +5U/mL rHuEPO), 10 U/mL rHuEPO intervention group (5mg/mL albumin +10 U/mL rHuEPO), 20U/mL rHuEPO intervention group (5mg/mL albumin +20U/mL rHuEPO), and Rho inhibi-taion group (addition of 5mg/mL albumin 30min after 10μmol/L Y27632), 24 h acting time for each group.We observed the changes of cell morphology in each group .Reverse transcription polymerase chain reaction ( RT-PCR) was used to evaluate the mRNA levels of RhoA, ROCK1 and IL-6 , and ELISA was applied to measure the levels of supernatant TNF-αand IL-6 protein. Results The form of pebbles or paving stone was observed in blank control group and rHuEPO intervention groups , a long and thin spindle change with the appearance of fibre cells in albumin stimulation group , the transformation to pebbles in 5, 10, 20 mg/mL rHuEPO intervention groups , the form of oval and slightly increased intercellular space in Rho inhibitaion group .Compared with the blank control group , the expressions of RhoA mRNA, ROCK1 mRNA and IL-6 mRNA significantly increased in the albumin stimulation group (P<0.05), while significantly reduced in 5, 10, 20 mg/mL rHuEPO intervention groups (P<0.05), which was in negative relation with the rHuEPO concentrations .Compared with the albumin stimulation group , the expressions of ROCK 1 mRNA and IL-6 mRNA reduced in Rho inhibtation group (P<0.05), while there was no significant difference as to the expression of RhoA mRNA .ELISA results showed:compared with blank control group , the expressions of supernatant TNF-α([452.32 ±33.23] ng/L vs [1347.54 ±41.52] ng/L), IL-6 protein([884.62 ±0.73] pg/L vs [95.12 ±0.32]pg/LP<0.05) increased significantly.Compared with albumin stim-ulation group, the expressions of TNF-αin 5, 10, 20 mg/mL rHuEPO intervention groups and Rho inhibitation group reduced signifi-cantly([1003.32 ±3.42] ng/L, [821.32 ±21.32] ng/L, [590.15 ±7.68] ng/L, [488.13 ±65.03] ng/L vs [1 347.54 ± 41.52]ng/L,P<0.05), while the expressions of IL-6 mRNA reduced accordingly in 5, 10, 20 mg/mL rHuEPO intervention groups and Rho inhibitation group reduced significantly ([656.68 ±0.55] pg/L, [422.35 ±0.22] pg/L, [217.32 ±0.35] pg/L, [309.49 ±0.21] pg/L vs [884.62 ±0.73]pg/L,P<0.05).Moreover, there was significant statistical difference among 5, 10, 20 mg/mL rHuEPO intervention groups(P<0.05). Conclusion RHuEPO can inhibit the transdifferentiation process of HK-2 cells in-duced by albumin by suppressing inflammation factors , and the mechanism may be involved in RhoA/ROCK signaling pathway .
8.Effect of hydroxyethyl starch on early stage of abdominal pressure and inflammatory cytokines in severe acute pancreatitis patients with intra-abdominal hypertension

Objective To investigate the effect of hydroxyethyl starch (HES) 130/0.4 on intraabdominal hypertension (IAH) and inflammatory cytokines in early stage of severe acute pancreatitis (SAP).Methods Clinical data of 55 case of SAP with IAH from Nov 2007 to Oct 2013 in Pancreas Surgery Department of Wuhan Union Hospital were analyzed retrospectively.All patients received conventional treatment without operation.According to the method of fluid resuscitation,patients were divided into treatment group (n =24) and control group (n =31).In treatment group,patients received ringer solution plus 6% HES 130/0.4 for fluid resuscitation,and patients in control group received only ringer solution for fluid resuscitation.The IAP level,APACHE Ⅱ score and serum inflammatory cytokine from day 1 to 8 were measured.Results The baseline data between the two groups were comparable.The IAP level was significantly lower in treatment group than that in control group from day 5 to day 8 [(10.2 ±2.9),(8.8 ±2.9),(7.9 ± 2.5),(6.9±2.6)mmHg vs (11.9±2.7),(10.5±2.7),(9.5±2.4),(8.6±2.5)mmHg,1 mmHg=0.133 kPa,respectively],and the difference between the two groups was statistically significant (P <0.05).There was no significant difference in APACHE Ⅱ score between the two groups,but the decline of APACHE Ⅱ score from baseline (△APACHE Ⅱ score) was more significant in treatment group (P <0.05).The serum IL-1 and IL-8 level in treatment group at day 8 was lower than that in control group [(15.1 ± 13.7) μg/L vs (23.6 ± 13.5) μg/L,(11.2 ± 12.8) μg/L vs (23.8 ± 27.9) μg/L,respectively],and the difference between the two groups was statistically significant (P < 0.05).And the serum TNF-α level in treatment group at day 4 and day 8 was lower than that in control group [(31.9 ± 12.1) μg/L vs (43.4 ± 22.4) μg/L,(24.2 ± 12.8) μg/L vs (35.1 ± 15.3) μg/L],and the difference between the two groups was statistically significant (P < 0.05).Conclusions Early fluid resuscitation with HES 130/0.4 and ringer solution relieves IAH,reduces APACHE Ⅱ score and down-regulates IL-1,IL-8 and TNF-α level.
9.Risk factors of hepatocellular carcinoma in patients with HBV-related liver cirrhosis receiving nucleos (t) ide analogues treatment

Objective To survey the incidence of hepatocellular carcinoma (HCC) in patients with HBV-related cirrhosis receiving nucleos(t)ide analogues treatment and to assess its risk factors.Methods A total of 141 patients with HBV-related liver cirrhosis receiving nucleos(t) ide therapy from April 2008 to June 2011 were enrolled.The clinical data including virological and biochemical tests were retrospectively analyzed.Univariate and multivariate Cox proportional hazards regression model was used to identify the risk factors of HCC occurrence.Results Patients were followed up for 6.4 to 87.6 months with a median followup time of 32.5 months.During the follow-up period,15 out of 141 patients developed HCC with an average annual incidence rate of 3.8%.HCC incidence was higher in HBeAg positive cirrhosis and in those with family history of liver cancer ( RR =4.524 and 3.858,P < 0.05 ).Conclusions Patients with HBV-related cirrhosis have a high incidence rate of HCC even they recieve nucleos (t) ide analogues treatment.HBeAg positive cirrhosis and family history of liver cancer are independent risk factors for HCC.
10.Synthesis and structure-activity relationship of cycloberberine as anti-cancer agent.

A series of cycloberberine derivatives were designed, synthesized and evaluated for their anti-cancer activities in vitro. Among these analogs, compounds 6c, 6e and 6g showed strong inhibition on human HepG2 cells. They afforded a potent effect against DOX-resistant MCF-7 breast cells as well. The primary mechanism showed that cell cycle was blocked at G2/M phase of HepG2 cells treated with 6g using flow cytometry assay. It significantly inhibited the activity of DNA Top I at the concentration of 0.1 mg mL-1. Our results provided a basis for the development of this kind of compounds as novel anti-cancer agents.
