1.Evaluation of the body fluid mode on Sysmex XN-2000 for detecting of tumor cells in ascites and pleural effusion

Objective To evaluate the capability for detecting of tumor cells in ascites and pleural effusion by Sysmex XN‐2000 hematology analyzer .Methods Determination of 84 samples of ascites and pleural effusion specimens with Sysmex XN‐2000 hema‐tology analyzer humoral model to analyze the study parameters highly fluorescent cells absolute value (HF‐BF # ) and percentage (HF‐BF% ) of ascites and pleural effusions screening of tumor cells ,and related data for statistical analysis .Results In 84 cases of pleural effusion and ascites samples ,cytology found 14 cases of malignant cells ,cancer cells are not found in 70 cases .14 cases of malignant cases ,HF‐BF # average of 169 /μL(0-2 001) ,HF‐BF% with an average of 29 .7% (0% -261% );70 cases of benign cases ,HF‐BF # an average of 39 /μL(0-524) ,HF‐BF% with an average of 4 .2% (0% -27 .8% ) ,both groups were statistical differences in the mean significance(P<0 .05) .ROC curve analysis with high absolute value and percentage of fluorescent cells to tumor cells detection function ,HF‐BF # area under the curve(AUC) was 0 .493 ,the area(AUC)% HF‐BF under the curve was 0 .222 .Conclusion Sysmex XN‐2000 Pattern of humoral parameters of HF‐BF # and HF‐BF% ,although the tumor cells may pro‐vide some screening information ,but ineffective ,so we believe that in our daily work can′t be over‐reliance on the instrument HF‐BF# ,HF‐BF% and other parameters should be performed on every sample smear staining ,thus improving the detection rate of ma‐lignant cells .

Bacterial translocation and endotoximia occurs in consequence of increased intestinal permeability after trauma. Glutamine can decrease intestinal permeability and improve the intestina mucosal barrier.
3.Effect and safety of losartan and captopril for the treatment of essential hypertension: A systematic review

Objective To assess the efficacy and safety of losartan and captopril for the treatment of essential hypertension.Methods The database was retrieved form China Journal Full-text Database(1994-2008.10),Chinese Biomedicine Database(1978-2008.10),Chinese Scientific Journals Full-text Database(1989-2008.10),WanFang database online(1982-2008.10),PubMed(1966-2008.10),Cochrane Library(Issue 4,2008),EMBASE(1900-2008.10),and SCI(1974-2008.10).Randomized control trials(RCTs) of losartan and captopril for essential hypertension were included.The methodological quality of included studies was assessed independently by two authors.The quality of the included studies was evaluated by Handbook 4.2.6 recommend standard.Data were analyzed by RevMan 4.2.10 from the Cochrane Collaboration.Results Eleven RCTs(including 1458 patients) met the inclusion criteria.① Treatment for 4 weeks: There was a significant lowering of clinic systolic blood pressure(SBP) in losartan group compared with that in captopril group [WMD=0.59,95%CI(0.22~0.55),P=0.002];while no statistically significant difference existed between the two groups in clinic diastolic blood pressure(DBP) [WMD=-0.08,95%CI(-1.11~0.94),P=0.87].② Treatment for 8 weeks: no significant difference was found between losartan group and captopril group in the lowering of clinic SBP [WMD=0.26,95%CI(-0.08~0.61),P=0.14] and DBP [WMD=0.13,95%CI(-0.28~0.54),P=0.54].③ Treatment for 12 weeks: no statistically significant difference existed between the two groups in clinic SBP [WMD=1.75,95%CI(-0.22~3.72),P=0.08] and DBP [WMD=1.15,95%CI(-2.81~5.11),P=0.57].④ The side effect in losartan group was lower than that in captopril group [OR=0.55,95%CI(0.42~0.73),P=0.00].Conclusions Based on the review,losartan is more effective and safe in lowering SBP compared with captopril.Further high-quality randomized controlled trials are needed to provide reliable evidence on the treatment of patients with essential hypertension.
4.Change in early postoperative intestinal permeability and its relation with systemic inflammatory response

Objectives:To study the effects of glutamine on the change of intestinal permeability and investigate its relationship with systemic inflammatory response in abdominal postoperative patients. Methods: A prospective, randomized, double blind and controlled trial was taken. 20 patients who underwent abdominal surgery were randomized into two groups: Gln group(orally administrated glutamine, 30 g/d, for 7 day, n =10) and placebo group(orally administrated placebo, 30 g/d, for 7 day, n =10). Temperatures and heart rates of all patients were daily recorded during administration. The white blood cell counts and biochemical variables were measured before operation and days 4 and 7 after administration. Serum concentrations of glutamine, endotoxin, diamine oxidase and malondialdehyde and urine lactulose/mannito(L/M) ratio were measured before and the 7th day after administration. Results:The patients in the two groups were comparable prior to the administration. Serum Gln concentration was significantly decreased in placebo group and increased in Gln group after 7 days of administration. Urine L/M ratio was significantly increased in placebo group and decreased in Gln group. The serum concentrations of endotoxin, diamine oxidase and malondialdehyde were significantly decreased in Gln group when compared with those in placebo group. Temperatures, heart rates and WBC counts were significantly lower in Gln group than in placebo group. Conclusions:The gut is one of the sources of the systemic inflammatory response in abdominal postoperative patients and the glutamine can decrease the intestinal permeability,maintain the intestinal barrier and attenuate the systemic inflammatory response in the early period of postoperative patients.
5.Perceptions on the risk communication strategy during the 2013 avian influenza A/H7N9 outbreak in humans in China: a focus group study

Objective: To identify the general public’s perceptions of the overall risk communication strategy carried out by Chinese public health agencies during the first wave of avian influenza A(H7N9) outbreak in humans in 2013.
Methods: Participants were recruited from communities in Beijing, Lanzhou and Hangzhou, China in May and June 2013 by convenience sampling. Demographics and other relevant information were collected using a self-administered questionnaire. Focus group interviews were conducted using a set of nine pre-developed questions and a tested moderator guide. The interviews were audio recorded and were transcribed verbatim. The constant comparative method was used to identify trends and themes.
Results: A total of nine focus group interviews, with 94 participants recruited from nine communities, were conducted. Most participants received H7N9 information via television and the Internet. A majority of the participants appreciated the transparency and timeliness of the information released by the government. They expressed a sense of trust in the recommended public health advice and followed most of them. The participants suggested that the government release more information about clinical treatment outcomes, have more specific health recommendations that are practical to their settings and expand the use of new media channels for risk communication.
Conclusion: The public perceived the overall risk communication strategy by the Chinese public health agencies as effective, though the moderator had a governmental agency title that might have biased the results. There is a need to expand the use of social media for risk communication in the future.

7.Expression of proinsulin and insulin in insulinoma and normal pancreas

Objective To investigate the expression of proinsulin, insulin, C-peptide in insulinoma and normal pancreas and their roles. Methods Thirty-eight cases of insulinoma and 20 cases of normal pancreas from Sep. 2006 to Dec. 2009 in our hospital were selected. Immunohistochemistry was used to determine the expression of proinsulin, insulin, C-peptide. Results Proinsulin, insulin, C-peptide was expressed in insulinoma and normal pancreas. Proinsulin, C-peptide were strongly expressed in 100%insulinoma, while they were weekly expressed in 55% ~60% normal pancreas; insulin was expressed as + + +in 79% insulinoma, while it was expressed as + + + + in 85% normal pancreas. Proinsulin, C-peptide positive cell accounted for 80% ~ 100% of 38 insulinoma, and insulin positive cell accounted for 50% ~ 70%,the proinsulin/insulin ratio > 1 accounted for 78.9%; while the corresponding values were 20% ~ 80%, 60% ~90%, 10.0% in normal pancreas, and the difference was statistically significant ( P < 0.05 ). Conclusions The proinsulin/insulin ratio > 1 is 78.9% in insulinoma, and detection of proinsulin may help to diagnose insulinoma.
9.Sequence Analysis of 4.0 kb Region Upstream of the Chitinase Gene of Spodoptera litura Nucleopolyhedrovirus

The region of about 4.0 kb upstream of Spodoptera litura nucleopolyhedrovirus (SpltNPV) chiA gene was sequenced, in which six open reading frames(ORF1~6) were found. These ORFs are 156, 297, 540, 369, 1281, and 228 nucleotides long, encoding the proteins of 51, 98, 179, 122, 426, and 75 amino acids with the molecular weight of 6.15 kD, 11.46 kD, 21.70 kD, 14.69 kD, 47.59 kD, and 9.09 kD respectively. One early promoter motif CAGT in ORF1 and ORF3, two early promoter motifs CAGT in ORF2, one late promoter motif TAAG in ORF4 and two late promoter motifs TAAG in ORF5 were found in 5′noncoding regions of these ORFs. The polyadenylation signals, AATAAA, are located downstream of the translation stop codon of ORF1, ORF4 and ORF5. ORF4 is the homologous gene of AcMNPV ORF53, BmNPV ORF42, OpMNPV ORF56 and LdMNPV ORF54. Compared with all genes from baculoviruses, ORF1, ORF2 and ORF6 have no homologous genes. It is suggested that ORF1, ORF2 and ORF6 may be three novel baculovirus genes.
10.Correlation between vasoactive intestinal peptide and expression of NKG2D, adaptor protein 10 and extracellular signal-regulated kinase in inflammatory cells of gastric adenocarcinoma

Objective To investigate the expression of NKG2D and related signaling molecules adaptor protein(DAP10) and extracellular signal-regulated kinase(ERK) in the inflammatory cells of gastric adenocarcinoma and para-carcinoma tissues,the effects of vasoactive intestinal peptide(VIP) on the expression of NKG2D,DAP10 and ERK in natural killer(NK) cells of healthy volunteers.Methods The expression of NKG2D,DAP10 and ERK in the inflammatory cells of 36 gastric adenocarcinoma and para-carcinoma tissues were detected by immunohistochemistry.The peripheral blood NK cells of healthy volunteers were isolated and purified with CDC method.The expressions of NKG2D,DAP10 and ERK in NK cells were determined by immunocytochemistry and reverse transcription-polymerase chain reaction(RT-PCR) methods before and after VIP intervention.The ttest was used for comparison between two groups and rank transformation nonparametric test (Kruskal-Wallis H test) for count data analysis.Results The expression levels of NKG2D,DAP10 and ERK in the inflammatory cells of gastric adenocarcinoma were significantly lower than those of para-carcinoma tissues (H=30.640,24.910,20.320,all P<0.01).The expression levels of NKG2D,DAP10 and ERK were much lower in poorly differentiated gastric adenocarcinoma (H =4.049,11.830,8.118),at stage Ⅲ-Ⅳ (H=4.275,11.560,7.686),and the expression levels of NKG2D,ERK were lower with lymph nodes metastasis (H=6.513,6.064,all P<0.05).The expression levels of NKG2D,DAP10 and ERK in gastric adenocarcinoma tissues and inflammatory cells were not correlated with gender,age and the location of lesions (all P>0.05).The expression levels of NKG2D,DAP10 and ERK in NK cells was decreased after VIP intervention (protein:H=12.438,4.798,13.745,mRNA:F=337.640,638.579,1055.015,all P<0.05).Conclusion Gastric adenocarcinoma may downregulate the expression of NKG2D,DAP10 and ERK in NK cells through VIP secretion,which may be involved in the immune escape of gastric cancer.
