1.Approaches for developing a system model for health evaluation based on body constitutions of traditional Chinese medicine and order parameters.

From the point of view of systems science, human body can be considered as a complex system, and the human health system is a subsystem of it. Systems science conducts investigation in a holistic manner. As a theoretical method, it deals with the operation and evolution of systems from the macroscopic perspective, so this theory is similar to phenomenological theory of traditional Chinese medicine (TCM) in methodology. Naturally, numerous theories of systems science can be used in research of the human health systems of TCM. In this paper, the authors introduced synergetics, a theory of modern systems science, and its slaving principle, and in particular, analyzed the concept of order parameters related to the slaving principle and the relationship between body constitutions of TCM and order parameters. The body constitution of TCM can be treated as a slow variable in the human health systems. By using synergetics, the authors established a model of the human health system based on body constitutions of TCM. As an application of the model, the authors illustrated the argumentations in the theory of constitution being separable, the theory of a relationship between constitution and disease, and the theory of a recuperable constitution. To some extent, this work has made links between the TCM theory of body constitution and modern systems science, and it will offer a new thought for modeling the human health system.
2.Therapy of patients with tentorial herniation due to post-traumatic acute diffuse brain swelling around operations

Objective To summarize and discuss the therapy of patients with tentorial herniation due to post-traumatic acute diffuse brain swelling around operations to improve the therapeutic efficacy.Methods A retrospective study was accomplished on 70 patients with tentorial herniation due to post-traumatic acute diffuse brain swellin around operations.Results According to Glasgow outcome scale,44 of 70 cases got favorable outcome,including 37 cases good recovery and 7 cases moderate deficit.The 26 of 70 cases got unfavorable outcome,including 6 cases severe deficit,5 cases persistent vegetable status and 15 cases dead.Conclusions This comprehensive therapy improves the efficacy of patients with tentorial herniation due to post-traumatic acute diffuse brain swelling around operations,which is worth spreading and applying.
3.Analysis of Normal Variation of Electrocardiograph for 6800 Adults

To explore some variation about normal electrocardiograph (ECG). Methods 6800 people aged 17~40 who hadhealth physical examination in our hospital from 1993~2011 were reviewed. Results The ECG of 4640 (68.24%) people are normal, 136(2%) were abnormal. The rest 2024 (29.76%) people probably had some ECG variation as follows: shorter PR interval (PR<0.12 s); PⅡ upsidedown; heart rate (HR): 40~60 beats/min, HR: 100~110 beats/min; incomplete right bundle branch block (ICRBBB); left ventricular hypertrophy(LVH); abnormal Q; RV1~RV5 amplitude were not increasing; SⅠSⅡSⅢ; STV3~V6 elevation>0.3~0.5 mV; Heart electrical axis is somewhatdeviation and heart position is somewhat shift; occasional superventricular premature beats or ventricular premature beats, etc. Theywere finally diagnosed as normal healthy people after 3~6 months follow-up with ultrasonic cardiogram (UCG), Holter and chest X rays,etc. Conclusion These abnormal ECG beyond normal ECG diagnosis standards are variation of normal ECG in health people.
6.The suitable uptake value threshold of 18F-FDG PET/CT image on gross tumor volume delineation of nasopharyngeal carcinoma

Objective To define a suitable threshold setting for gross tumor volume (GTV)when using 18F-fluoro-deoxyglucose positron emission tomography and computed tomogram (PET/CT) for radiotherapy planning in Nasopharyngeal carcinoma(NPC).Methods Sixteen NPC patients respectively received PET/CT and MRI scan before their radiation treatment.All of the images were transferred to the radiotherapy planning system (TPS).MRI/CT-based primary GTV was defined as GTVf.Biological target volumes (BTVs) were derived from PET/CT-based GTVs of primary tumors.The BTVs were defined as the volumes when adjusting different percentage of the maximal standardized uptake value (SUVmax).GTVfs were compared with BTVs.The suitable threshold level (sTL) could be determined when BTV value and its morphology using a certain threshold level were observed to be the fittest GTVf.The suitable standardized uptake value (sSUV) was calculated as the sTL multiplied by the SUVmax.Results Our result demonstrated no single sTL or sSUV method could achieve an optimized volumetric match with the GTVf.The sTL was [20.93%±6.51%(15%-40%)],whereas the sSUV was [2.27±0.48(1.56-3.25)].The sTL was inversely correlated with the SUVmax sTL =-0.144ln(SUVmax) + 0.5548 (R2 =0.85,F =78.57,P<0.01.The sSUV showed a linear correlation with the SUVmax sSUV =0.104(SUVmax) + 1.0398,(R2=0.75,F=41.88,P<0.01).The sTL was not associated with the value of GTVf.Conclusions In PET/CT-based BTV for NPC,SUVmax threshold method is feasible.sTL is not a fixed value,which is correlated with the SUVmax instead of the value of tumor.
7.Choice of the treatment of palliative therapy of jaundice in cholangiocarcinom

Objective To compare the efficacy and indications between the biliary bypass laparotomy surgery.and the two different kinds of biliary stent insertion surgery in the palliative alleviating jaundice of cholangiocarcinoma.Methods From March 2008 to March 2013,69 patients treated with palliative alleviating jaundice therapy of cholangiocarcinoma were included,who were all came from the Third Affiliated Hospital of Guangzhou Medical University.Including 17 patients who treated with the open biliary bypass surgery and 52 patients who treated with interventional therapy.We analysed the differences between these cases of biliary drainage operations in the recent jaundice reduction rate,average stay,mortality rate,the incidence of related complications,et al.Results All these different drainage ways had good effect in alleviating jaundice,incidence of alleviating jaundice have no obvious difference (P > 0.05).Compared to the open biliary bypass surgery,interventional therapy had obvious advantages in the average stay and postoperative survival (P < 0.05).The pancreatitis rate was lower in Percutaneous Group than that in Endoscopic Group (P < 0.05).incidence of biliary tract infection and biliary tract bleeding have no obvious difference (P > 0.05).There were no significant differences between the success rates of in biliary stent insertion operation in patients with each model cholangiocarcinoma (P > 0.05).Conclusions Among the therapies of the palliative alleviating jaundice of cholangiocarcinoma,the internal biliary drainage of biliary stent insertion operation was superior to the treatment of the biliary bypass laparotomy.As to biliary stent insertion operations,endoscopic biliary stenting surgery should be the preferred choice.
8.Clinical observation of pregabalin in the treatment of painful diabetic peripheral neuropathy

Objective To observe the efficacy of pregabalin in the treatment of painful diabeticperipheral neuropathy.Methods In a 8-week study,50 patients were randomly assigned into two groups:the therapy group and the control group.The patients in the therapy group were treated with pregabalin 150 mg/d,and the patients in the control group were treated with carbamazepine 200 mg/d.The dose of pregabalin was increased to 300 mg/d till pain was alleviated in the therapy group,and the dose of carbamazepine was increased to 600 mg/d till pain was alleviated in the control group.The therapeutic effect and VAS score in the two groups were observed and evaluated.Results After 1 and 8 weeks' treatment,the therapeutic effect of therapy group were significantly superior to the control group ( At 1 week:U =2.028,P =0.046 ; At 8 week:U =3.540,P <0.001 ).After 1 and 8 weeks' treatment,the VAS scores of therapy group were significantly lower than the control group ( At 1 week:5.13 ± 1.76 vs.6.74 ± 1.52,P < 0.05 ; At 8 week:1.13 ± 0.45 vs 3.27 ± 1.04,P < 0.05).There were 5 patients (20%) and 10 patients (40%) had adverse reaction in the therapy and the control groups respectively.Conclusion Pregabalin is effective and safe for the treatment of painful diabetic peripheral neuropathy.
9.Particle gun-mediated bone morphogenetic protein-2 gene transfection for treatment of chronic bone defects

BACKGROUND:Both in vitro and in vivo studies have confirmed that, bone morphogenetic protein (BMP) regulates the differentiation of osteoblasts and chondroblasts, induces heterotopic bone formation, promotes fracture healing, and controls the morphology of skeleton in mammals.
OBJECTIVE:To treat chronic bone defects using particle gun containing BMP2 gene eukaryotic expression plasmid via local injection.
METHODS:A total of 72 healthy New Zealand white rabbits were applied to establish chronic bone defect model in the rabbit radius. According to the length of bone defect, the rabbits were divided into three groups:1.5 cm group, 2.0 cm group, 2.5 cm group. Each group was further randomly assigned into two subgroups:treatment group (BMP-2 gene transfection) and control group (natural y healing). X-ray examinations were performed at 1, 3, 8 and 9 weeks after transfection, and soft tissue between the bone defects was harvested to detect BMP-2 using western blot analysis;and radius specimens were taken for gross observation at the same time points, to evaluate the healing.
RESULTS AND CONCLUSION:(1) Gross specimen observation:bone cal us formation in treatment group was general y more than that in control group. (2) Lane-Sandhu X-ray score in treatment group was significantly higher than that in control group at 1, 3, 8, 9 weeks after transfection (P<0.05). (3) BMP-2 concentration in treatment group was significantly higher than that in control group at each time point (P<0.05). The local transfer of particle gun-mediated BMP-2 gene is an effective therapy of chronic bone defect.
10.Exploration on the selection of positive materials in active systemic anaphylaxis in guinea pigs

ObjectiveTheaimofthisstudywastoprovideabetterpositivecontrolforallergictestbycomparing the allergic effect of two kinds of positive materials , human albumin and ovalbumin , on active systemic anaphylaxis in guinea pig.Methods Guinea pigs were randomly divided into 14 groups, and were given human albumin , ovalbumin (2, 10, 100 mg/animal), or 0.9%sodium chloride injection as test substances , to assess the symptoms and incidence of systemic allergic responses induced by different sensitizing substances in different challenge doses and different challenge intervals.Results In the range of 2 to 100 mg/animal, the guinea pigs showed a 100%incidence rate of positive allergic reaction to human albumin and ovalbumin , the severity of anaphylactic symptoms was increasing along with the increase of sensitizing doses and challenge doses , and the allergic reaction was more strong induced by the same dose of ovalbumin than human albumin .Conclusions Our findings indicate that in the active systemic anaphylaxis test in guinea pigs , we recommend ovalbumin as the positive control in a dose of 2 mg/animal.
