Heterografts*

Proliferative fasciitis is a benign pseudosarcomatous mesenchymal lesion occurring in the subcutis. The lesion occurs clinically as a tumorous mass that develops within a rather short time in elderly patients. We recently experiecened a case of fine needle aspiration cytology of proliferative fasciitis in the left anterior chest wall of a 72 year-old male patient. The smear revealed two types of cells. One was the large and mostly oval cell with one or two nuclei lying at the periphery of the cell body and abundant basophilic cytoplasm like the ganglion cell. The nuclei were round to oval, had vesicular chromatin and contained prominent nucleoli. The other was the spindle shaped fibroblast with an oval nucleus. The differential diagnosis includes a true tumor such as ganglioneuroma, rhabdomyosarcoma, and liposarcoma and therefore fine needle aspiration cytology is very much indicated in order to exclude these possibilities.

PURPOSE: Hepatitis B virus infection is prevalent in Korea. Active immunization of Hepatitis B has been proceeded extensively for the last decade. This study was performed to estimate the negative rate of anti-HBs after immunization and to evaluate the effect of revaccination among Korean children between 1995 and 1997, as well as to determine a proper revaccination schedule. METHODS: Anti-HBs was measured with enzyme immunoassay. Hepatitis B revaccination was performed on nonresponders, those who were previously vaccinated and whose anti-HBs titers were under 10mIU/mL. Group A was revaccinated one dose of the same amount of the previous vaccine. Group B was given three doses of the same amount of the vaccine. Group C was given one double dose of the vaccine. The seroconversion rate and geometrical mean titer were measured one month later. RESULTS: Negative anti-HBs rate was 14.5% in children who had been vaccinated. The seroconversion rate was high in all revaccination groups(86.7-94.7%). Three doses of revaccination had no significant difference from one dose of revaccination. There was no positive effect of double dose of revaccination in comparison with the usual dose. CONCLUSION: In our study, one dose of hepatitis B vaccine is as effective as three doses or double dose of hepatitis B vaccine for the nonresponsive children. We recommend administering one dose of hepatitis B vaccine to those children whose three anti-HBs titer is under 10mIU/mL and examining of the anti-HBs titer one month later after revaccination.

Dysplasia Epiphysealis Hemimelica is a rare developmental disorder of epiphyseal growth characterized by asymmetrical cartilagenous overgrowth. This disorder was first described with the “la tarsomegalie” by Mouchet and Belot in 1926 but this name, Dysolasia Epiphysealis Hemimelica, was first proposed by Fairbank in 1956. This disorder is usually limited to either the medial or lateral half of a single extremity in childhood. It is asymptomatic until the protruding epiphyseal mass interferes joint function. The character istic abnormalities are deformities, restricted motion and pain. The most frequent finding at the initial examination is an irregular, often multicentric radiopacity, and when the lesion is matured it looks irregular, and frequently there is a lobulated osseous mass protruding from the epiphysis of tarsal and carpal bones. Diagnosis by the roentgenogram requires primarily knowledge of this condition. The lesion is often microscopically indistinguishable from an osteochondroma. A case of Dysplasia Epiphysealis Hemimelica in a 17 year old male, showing typical roentgenographic appearances, is reported together with a brief review of literature in this paper.

This presentation is a case of osteomalacia, the cause of which was a nutritional deficiency. The patient was a 42-year-old female who had pseudofractures of ribs, ulnae, pelvic raml, the right second metacarpai bone and both femoral necks. Following treatment by medication combined with internal fixation of the femoral necks, the result was good.

The clinical observation was performed on 32 patients of acetabular fracture, who had been admitted and treated at Department of Orthopedic Surgery, College of Medicine, Busan National University from January 1980 to December 1983. The results obtained were as follows. 1. The prevalent age ranged from 20 to 50 years, which comprised 71.9% and the ratio between males and females was 2.6: l. 2. The causes of injury were 16 cases of traffic accident, which comprised the highest incidence, 10 falling down and 6 direct blow. 3. According to Letournels classification, 21 cases (65.6%) were simple fractures, llcases(34.4%) were associated fractures. 4. Total patients associated injuries of another part of body were 24 cases#{75.0%) among them pelvic bone fracture was the most common fractured case and cerebral concussion was the most common soft tissue injury. 5. 2 types of treatment were performed, one was 18 conservative treatment and the other was 14 surgical treatment which was performed by open reduction and internal fixation. 6. The satisfactory result of conservative treatment was 61.6% and surgical treatment was 85.7. 7. The complications were 7 cases(21.9) of degenerative arthritis, ectopic ossification in 2cases (6.3%), avascular necrosis, superficial infection, and secondary displacement of fracture site in 1 case(3.1%) respectively. 8. The anatomical reduction and internal fixation was an important factor in treatment of acetabular fracture, especially in severe displaced articular fracture.

Eccrine epithelioma is an exceedingly rare cutaneous tumor ancit is believed to represent the maligant counterpart of the eccrine dermal syringoma. It occirs as an infiltrating basaloid growth, usually on the scalp or head, that frequently recur, apparrntly because of inadequate excision. We report a case of this tumor in a 60-year-old female, who showed a well demarcated, tender nodule on her left parietal scalp which had been present for three years. Histopat.hologically, the tumor was composed of strands of darkly staining epithelial cell embedded in a dense fihrcus stroma with ductal differentiation and cyst formation.
