1.Pterygium Surgery.

The author operated 34 cases of pterygia by Newton's method, wihich combined Mc Raynold's transplantation with limbal incision, in these 34 case, 4 cases were recurred, and in other 2 cases, a few vessels only found on the operated area of the cornea. Among these 6 cases, one had trachoma, 3 had chronic cat conjunctivitis. Auther thinks that the chronic inflammation such as trachoma or chronic conjunctivitis is very important factor in the recurrence of pterygia. This fact also encourages the author who believes that the inflammation is rather more important factor in the etiology of the pterygia than the degensration.

2.Marfan's Syndrome.

We are Presenting of two cases of Marfans syndrome. First acse; 22 year old boy, height 262 cm, weight 40.9 kg, typical skeleton, hands and feet. The span of the outstreched arm, from right finger to the other, is 165 cm, exceeds by 3 cm than the height. Abnormal flexibility of Joints in the both thums. Vision; O.D. 0.1 with-sph.7.0 D., O.S. 0.0 with-sph.6.0 D. Lens; typically dislocated, upward and inwad and inward iridodonesis is marked. The slit lamp examination of the zonular fiber is very attractive, In the left eye, dislocation is more severe than the other, and the zonular fiber are more elongated. The fibers are seen very clearly, Just like fine whitish silk contrast to its black background. At the 5 o'clock Position pupil margin in th left eye, one of them was revered and distorted. No significant fundus signs were seen. His parents and four brothers all have Normal visions and skeletons Normal cardiovascular system, even x-raypicture, balium in in esophagus, and normal E.C.G. picture. The second sase; 5 year old girl, Height 120 cm, weight 20.6 kg, typical skeletor, hands and feet. Vision both 0.2, 0.2 and 0.3 with -sph.9.0 D. Left eye; typical dislocated lens, upward and outward. iridodonesis is also found, no significant fundus picture. Normal cardiovascular system, even x-ray picture, balium in esophagus, and normal E.C.G. picture. Father; normal vision and skeleton. mother; typical skeleton, hands and feet, but normal vision, said her grandmother also had delicate bony structure and very poor vision, died known causes. The literature is briefly reviewed and the Post- operative complication esp. detachment of retina, emphasized.

3.A Case of Sparganosis, Formed Giant Hematoma in Scrotum.

Sparganum infestation in human body was reported over 20 cases in Korea. Infestation of sparganosis in spermatic cord was reported a case in Korea. Three cases of sparganosis at Keu Je island in Korea was reported by Dr. Weinstein, Krawczyc and Peers 1954. We experienced a case of the sparganum infestation in scrotum. The patient, 41 year-old male who was suffering with pulmonary tuberculosis moderate advanced, visited 3rd Army Hospital urologic clinic complained with acute swelling of the scrotum in baby head size abruptly without any specific causes. severe tenderness and walking disturbance. The laboratory findings revealed no specific findings except ascaris ova in stool. Operation revealed massive hematoma formation and proliferated connective tissue where noted several infested parasites in length of 120cm, 75cm, 70cm around the tunica vaginalis in scrotum Extracted parasites revealed milkish-whitish color, flat, fragile, and tendon like glistening appearance. There is not any specific sign and symptoms by parasite infestation on the other organs. Patient had an episode of intake of raw snake as a tonic 9 years ago at Keu Je island in Korea.

Congential bile duct cysts, the so called "choledochal cysts" occur at all levels of the biliary tree and may be of saccular, diverticular, or fusiform configuration. There are 6 types of choledochal cysts proposed by todani et al based on roentgenologic and operative findings. This disease is usually diagnosed at the childhood or early adult life and predilection for orientals. Clinical manifestations are usually jaundice, right upper quadrant abdominal pain and abdominal mass. Here we are reporting a case of choledochal cyst, type IV-A diagnosed by abdominal ultrasonogram. 99mTc-DISIDA hepatobiliary scan and endoscopic retrograde cholangiopancreatogram.

8.A Case of Ascarid Chronic Pancreatitis Due to Impaction of Ascaris Lumbricoides into the pancreatic Duct.

Authors report a case with recurrent epigastric pain who revesled to have chronic pancreatitis due to impaction of Ascaris lumbricoides into the pancreatic duct. She was a 41-year-old female who favored raw meat. ERCP revealed a living round worm in the pancreatic duct which was also showed by ultrasonegraphy and computed tomography. She underwent distal pancreatectomy and developed glucose intolerance but was well controlled.

9.Colonoscopically Diagnosed Case of Behcet's Syndrome Involving Cecum.

Authors came acoss a case of Behcet's syndrome involving cecum. The patient was a 33-year-old female. Her chief complaint was right lower quadrant pain, Colonoscopy revealed several ulcerations the base of which was covered by thick whitish coat, They were surrounded by mulitiple nodular masses. Endoscopic biopsy showed noaspecific ulceration. The patient had a history of aphthous ulceration of mouth and external genitalia. So she was diagnosed to have Behcets syndrome. She underwent right hemicolectomy with tetminal ilectomy. We report this case with literiature review.

10.A Clinical Review of the Endoscopic Removal of 15 Cases of Foreign Bodies in the Upper Gastrointestinal Tract.

The clinical review was done on 15 cases with the foreign body in the upper gastrointestinal tract who visited and underwent endoscopic removal of foreign body at department of lnternal Medicine of Chonnam University Hospital and Namkwang Hospital from June 1982 to August 1987. The results are as follows: 1) The age distribution varied from 32 yeara to 83 years of age, being most prevalent at 6th decade, The ratio between male and femaie was 4:1, more prevelant in male. 2} Among 15 eases, 8 cases were lodged in the esophagus and 7 cases in the stomach. The foreign bodies in the esophagus were meat, pressure-through package, food materials and bone in order of frequency and those in the stomach were bezoar, persimmon seed, brown seaweed, tooth brush and suture materials. 3) Cambined diseases of upper gastrointetinal passage disturbance were 1 case of esophageal cancer and 3 cases of gastric outlet obstruction caueed by duodenal ulcers and gastric malignancy. Other combined diseases were schizophrenia, chronic alcoholism, diabetes mellitus and gastrie polyp. 4) Complications by foreign body were retropharyngeal abscess and phlegmonous esophagitis with esophageal ulcer.
