1.Clinical Observation on the Cryptorchism.

A clinical observation was made on 53 patients of cryptorchism who had been admitted to the Department of Urology, Kangnam Sacred Heart Hospital during the period from January, 1980 to January, 1983. The following results were obtained: 1. Only 22 patients (41.5%) visited hospital in preschool age (7 years old) and 50 patients (94.3%) before the age of 25. 2. 44 patients (83%) had unilateral involvement and right side (56.6%) was more frequently affected. Bilaterally was noted in 9 patients (17%). 3. 59 testes were surgically explored, 2 cases were confirmed as monorchism. Of the 59 testes, 47 (79.7%) were located in the inguinal canal, 8 (13.6%) in the prepubic region and 2(3.4%) in the abdomen. 4. Among the 59 explorations, 50 cases (84.8%) were found .the hernial sac. Other complication and associated diseases were contralateral hydrocele, ventral curvature of penis, prosis, entropion, intestinal obstruction, triology of Fallot, acute glomerulonephritis and seminoma. 5. Orchiopexy was performed on 53 testes (93%), 4 cases were performed the orchiectomy because of testicular atrophy and excessive short spermatic cord.

4.International Comparison of the Non-benefits Management Policies for Public and Private Health Insurance

In the process of promoting policies to strengthen health insurance coverage, the relationship between public health insurance and private health insurance, along with the management of non-benefit, is also emphasized as a policy issue. First, the concept and scope of non-benefit were comparatively analyzed by country. Second, the interaction between the public and private health insurance was classified as ‘large or small,’ and the government’s regulation and management policy on private health insurance was classified as ‘strong or weak.’ Korea has relatively smaller benefits covered by public health insurance, higher copayment expenses, and more areas and scope of non-benefits. In countries where the interaction between public and private health insurance is small, private health insurance-related policies are weak. And in countries with large interactions had public-private partnerships and the government’s management policies were also strong. On the other hand, Korea has a large interaction, but the actual structure of cooperation between public and private insurance and management policies were weak. Because the non-benefit sector in Korea is relatively wide, it is difficult to manage compared to other countries where the concept of non-benefit is limited. In addition, the health authorities rarely perform the role of supervision over private health insurance, and they have so few linkages and cooperation for public-private insurance. Therefore, practical policy enforcement is necessary to achieve the easing of the burden of national medical expenses through linkage and cooperation of public-private health insurance with reference to relevant other countries’ cases.
5.Correction of facial depression using precisely-shaped silicone implants.

We introduce a new silicone implanting method to correct facial depression caused by trauma or congenital anomaly. It resulted in good facial aesthetics in patients with facial depression. From August 1997 to January 1999, 5 patient (3 males and 2 females), in ages from 19 to 34 years, underwent silicone implantation for facial depression using our method. Three males were injured by traffic accident and had facial depression with or without bone defect. Two females had congenital forhead depression and requested aesthetic contouring. An impression of the face was taken with alginate backed with quick setting plaster. When set, a further stone plaster model was taken from the alginate-plaster, which was removed after fixation. Wax was then carved into the patient's stone model to fill the depression. Another stone cover was taken to cover this was-stone model. Then wax was melted away and medical implant grade room temperature vulcanized silicone adhesive was squeezed into the empty space. After cured, this precisely-shaped silicone imlant was inserted into the patient. We did not experience major complicaion such as infection, implant extrusion during 4 to 18 months' follow-up periods. All patients were satisfied with the results. The clinical result of this technique would be useful for accurate and excellent aesthetic implantation.

6.Prevalence of otitis media and allied diseases in Korea--results of a nation-wide survey, 1991.

A nation-wide survey was conducted to estimate the prevalence of otitis media and allied diseases through physical examination by otolaryngologic doctors from July to October, 1991. A sample study population was selected from a panel to be representative of the Korean population by multistage clustered and stratified random sampling method based on the National Census Data, 1990. Total number of subjects undertaken were 9,321, Which were drawn from 2,899 households residing in 60 different areas throughout the country. The overall prevalence of otitis media was 2.85% (3.10% of males and 2.61% of females). Prevalence of each subtype was as follows; acute otitis media, 0.02%; middle ear effusion, 0.60%; chronic otitis media, 2.19%, respectively. The prevalence of otitis media did not vary by sex, nor by urban-rural difference. There was an increasing tendency of the prevalence of otitis media by age. However, the prevalence of both middle ear effusion and acute otitis media showed a decreasing pattern by age. Particularly noteworthy was a significant geographic variation in the prevalence of otitis media by provinces. The prevalence rate of septal deviation was 21.93% and the presence of septal deviation was significantly related to that of otitis media (p < 0.05). The prevalence rates of nasal allergic symptoms, nasal polyposis and evident cleft palate were 1.14%, 1.00% and 0.07%, respectively. The presence of otitis media was significantly correlated with both nasal allergic symptoms (P < 0.1) and evident cleft palate (P < 0.05), but not with that of nasal polyposis.

7.The Effect of 5-FU and Radiation on A549 Cells In Vitro.

Effects of ionizing radiation alone and combined with chemotherapy on tumor growth and it's clonal specificity Monitored by changes in distribution of chromosome number were studies in A549 cell line originated from human adenocarcinoma of the lung. Radiation (300 rad, 600 rad and 900 rad) were delivered with or without 5-FU. Forty eight hours later, 57.5% of growth inhibition of cell was Seen in cells treated with 5-FU concentration of 0.47g/ml for 24 hr exposure. Cell survival carves after radiation with and without 5-FU were made. Chromosomal analysis of cells in metaphase in control, and in cells treated with 300 rad of radiation, or 0.47g/ml of 5-FU treatment, and combined treatment of cloth were 77ne to examine the changes in ploidy and number of chromosome. Radiation combined with 5-FU enhanced growth inhibition of A549 cells. However, no evidence of synergegic effects in growth inhibition was observed in the cells treated with the combination therapy. Pattern of chromosomal distribution of survived cells were shifted from hyperploidy to hypoploidy by single dose of radiation(300 rad). As radiation dose increased a large number of hypoploidy cells were observed. Following treatment of cells with 5-FU, chomosomal distribution of survived cells were also shifted to hypodiploidy, which were seen in cells treated with radiation. The cell treated with 5-FU and followed by radiation within 24 hrs had cell with increased number of hypodiploidy cells. Almost same type of chromosomal changes were reproduced in cells treated with combined treatment with radiation and 5-FU. Minor differences were that cells with fewer number of chromosome were more frequent in cells treated with combined therapy. Further increase in cells of hypoploidy(93%) having 1~10 chromosome were induced by additional radiation. Therefore, the enhanced therapeutic effect of 5-FU combined with radiation of A549 cells appeared to be additive rather than synergistic.

8.A case of Aicardi syndrome with cleft lip and palate.

A case of Aicardi syndrome with cleft lip and palate was experienced at the Department of Pediatrics, College of Medicine, Hallym University, and the patient's infantile spasm was treated with ACTH. In previous studies, four cases of Aicardi syndrome accompanied by cleft lip and palate were reported. We present the fifth case of Aicardi syndrome with cleft lip and palate in the world. Thus, facial clefts may be found as an occasional manifestation of Aicardi syndrome.

9.Laparoscopy Through the Open Ipsilateral Sac to Evaluate Contralateral Patent Processus Vaginalis in Children with Clinical Unilateral Inguinal Hernias.

PURPOSE: Laparoscopy through the open ipsilateral hernial sac to evaluate the contralateral side in boys with unilateral inguinal hernia has been criticized because of some risk and cost. Laparoscopic evaluation of the contralateral side was modified to avoid abdominal trocar insertion by using the open ipsilateral hernia sac for instrumentation. This study was performed to determine the usability, safety, and effectiveness of this technique. MATERIALS AND METHODS: Forty-two boys with unilateral inguinal hernia and hydrocele were evaluated using open hernial sac for diagnostic laparoscopy of the contralateral side at the time of inguinal herniorrhaphy. RESULTS: The mean age was 3.8 years. The presenting hernia was right-sided in 29 (69.0%) cases and left-sided in 13 (31.0%) cases. Based on the result of diagnostic laparoscopy 32 (76.2%) cases were negative on the contralateral side, and 10 (23.8%) were positive. After the contralateral exploration of the positive groups, 9 (90.0%) showed contralateral patent processus vaginalis. After undergone the high ligation of the patent processus vaginalis at the internal inguinal ring. CONCLUSIONS: Laparoscopic evaluation of the contralateral side via the open ipsilateral hernial sac is feasible, quick, safe, and cost effective. It requires no additional incision. Long-term follow up is required to determine the ultimate effectiveness of this method.

10.Clinical Observation of the Effect of Sacral Block with Phenol in Hyperreflexic Bladder Patient.

Percutaneous chemical neurolysis of sacral nerve with phenol was performed on 11 cases of hyperreflexic bladder to augment bladder capacity and to make intermittent clean catheterization more feasible. Urodynamic evaluations were done before and after chemical neurolysis. Followup ranged from 10 months to 16 months(mean 13 months). Mean bladder capacity increased significantly after chemical neurolysis ( from 157.5ml to 340ml). No significant changes in bowel or injection sites were noted. Continence was improved in 100%, with 7 patients remaining completely dry and voiding with intermittent self-catheterization. The long term results were necessary but we considered the chemolysis of sacral nerve was available modality for hyperreflexic bladder patients, who did not respond to anticholinergic medication, before decide the more aggressive alternatives, such as augmentation cystoplasty or urinary diversion.
