3.Bipolar Endoprosthesis with Acetabular Bone Graft in Dysplasia of the Hip: Report of eight cases

Total hip replacement arthroplasty is one of the excellent treatment modalities of adult hip joint disease. But those who have severe dysplasia of the hip usually require augmentation of acetabular bone stock in order to carry out the total hip replacement arthroplasty and technical difficulties are often encountered in estabilishing an adequate acetabular bed for the prosthesis. From July 1988 to January 1990, we attempted to treat patients of dysplastic hip by bipolar endoprosthesis with acetabular bone graft using femoral head autograft in 8 patients and then reviewed these cases. The mean follow up period was 3 years ranging from 1 year and 10 months to 3 years and 10 months. The results obtained were as follows : 1. All the bone grafts were united well(average bone union time : 5.3 months). 2. According to Gerber and Harris' classification, there was no evidence of resorption of the graft in 1 case, and there were minor resorption in 5 cases, moderate resorption in 1 case, and major resorption in 1 case. 3. The average preoperative Harris hip score was 47.5 points and the average Harris hip score at last follow up was 79.0 points. 4. Because of the high rate of graft resorption and component migration, bipolar endoprosthesis with acetabular bone graft was not to be recommended as a reconstructive procedure for the dysplastic hip which have segmental bony deficiency in the acetabulum.

5.Hand-foot Syndrome Due to Capecitabine.

Hand-foot syndrome, also known as acral erythema, is a distinctive and relatively common toxic reaction due to some anticancer drugs. It is characterized by a painful erythema on the palms and soles during chemotherapy, which is often preceeded by paresthesia. We report two cases of hand-foot syndrome induced by capecitabine, which is a relatively brand-new oral anticancer agent. Hand-foot syndrome is one of the most common complications of capecitabine, and is on the increase. Therefore, dermatologists should be aware of it.

6.Pseudolymphoma Induced by Ear Piercing.

We present a patient who developed a pseudolymphoma after ear piercing. A patch test showed gold sensitivity. Therefore, long standing dermal exposure to gold from a pierced-type earring might have produced the patient's disease. This patient did not respond to an intralesional steroid injection and was finally treated with a complete resection of the lesion. It is proposed that every patient with a hyperplastic lesion in the ear lobe should be taken a biopsy in order to rule out the possibility of a pseudolymphoma. When a pseudolymphoma develops in the ear lobe, a complete surgical excision could be the treatment of choice, especially for the prevention of recurrence.

8.Intraperitoneal Fluid Collection: CT Characteristics in Dertermining the Causes.

PURPOSE: Abdominal CT scans in patients with intraperitoneal fluid were retrospectively studied to identify characteristic features useful for differential diagnosis of various causes. MATERIALS AND METHODS: One hundred and seventy patients with intraperitoneal fluid collection were classified as categories of hepatic disease, carcinomatosis, and infectious disease. We analyzed sites of fluid collection, the presence of peritoneal thickening, omental and mesenteric fat infiltration, and lymph node enlargment. RESULTS: Intraperitoneal fluid was present in subhepatic space, subphrenic space, paracolic gutter, mesentery, and fossa of the gallbladder in decreasing order of frequency. Fluid in the gallbladder fossa was the most frequent in hepatic diseases. The fluid collection in subhepatic and subphrenic space was less frequent in infectious diseases. Peritoneal thickening was noted in infectious diseases, and carcinomatosis. Omental fat infiltration and enlarged lymph nodes were the most frequent in carcinomatosis (58% and 44%, respectively), whereas, mesenteric fat infiltration and enlarged lymph nodes were the most common in infectious diseases (61%, and 26%, respectively). CONCLUSION: The location of peritoneal fluid collection showed some lesion specific characteristics, and CT features of fat infiltration and enlarged lymph nodes of peritoneum, omentum, and mesentery were helpful for differential diagnosis between carcinomatosis and infectious diseases.
