Carotid stenosis is an important modifiable risk factor in ischemic stroke.The com prehensive diagnosis of carotid stenosis,including the comprehensive understanding of stenosis degree,plaque stability,and head and neck vessels,which has important guiding significance for the prevention and treatment of ischemic stroke.This article reviews the diagnostic values of digital subtraction angiography,sonography,magnetic resonance angiography and CT angiorgaphy in carotid stenosis.

In order to supply a gap of PACS which is unidirectional,frame transmission and lag,a demonstration and teaching system which can be used as the tech support of the academic exchanging,teaching,studying,and emulating on spot is developed.By this,the medical audio and video signals and images including the video of the operation process,the images from endoscopes,electron microscopes,and the pathological images etc.in the clinical buildings and in the terminal units including research and teaching(training and exchanging) centers,lecture theaters and the director's offices etc.can be transmitted synchronously and bi-directionally in real time.

Objective:To establish the emergency medical equipment rental center, not only can reduce the repeated purchase for medical equipment, but also can achieve resource sharing, reduce equipment maintenance cost, and improve utilization efficiency.Methods: To establish emergency medical equipment rental center, which adopted lease mode and centralized management mode to use equipment. Department of medical engineering managed this center and this department is responsible to ensure every clinical department can use equipment by lease method.Results: The center has optimized the resource allocation of hospital, obviously reduced equipment input cost, improved utilization rate of medical facility since it were established. This mode provided reliable service for clinical departments.Conclusion: The established rental center of emergency medical equipment has changed the traditional management mode to a new feasible mode which can improve centralized management for medical devices, reduce waste for medical resource and achieve resource sharing.

[Summary] Type 2 diabetic rat model accompanied by insulin resistance was established by high fat/high glucose diet and streptozotocin.Following metformin treatment for 4 weeks,realtime PCR and Western blot were used to detect the expressions of SIRT3 mRNA and protein in skeletal muscle tissue,respectively.The results showed that insulin sensitivity,superoxide dismutase (SOD),and glutathione (GSH) levels were significantly reduced in diabetic group compared with the control group (P＜0.05),while methane dicarboxylic aldehyde (MDA) level was increased (P＜0.05),along with the decreased expressions of SIRT3 mRNA and protein in skeletal muscles (P＜0.01).After metformin treatment,the insulin sensitivity,SOD,GSH,and SIRT3 mRNA and protein levels were significantly increased (P＜ 0.05 or P＜ 0.01),while MDA level was decreased (P ＜ 0.05),suggesting that metformin may ameliorate insulin resistance via upregulating SIRT3 expression in skeletal muscles of diabetic rats.

Objective To investigate the expression of NKG2D and related signaling molecules adaptor protein(DAP10) and extracellular signal-regulated kinase(ERK) in the inflammatory cells of gastric adenocarcinoma and para-carcinoma tissues,the effects of vasoactive intestinal peptide(VIP) on the expression of NKG2D,DAP10 and ERK in natural killer(NK) cells of healthy volunteers.Methods The expression of NKG2D,DAP10 and ERK in the inflammatory cells of 36 gastric adenocarcinoma and para-carcinoma tissues were detected by immunohistochemistry.The peripheral blood NK cells of healthy volunteers were isolated and purified with CDC method.The expressions of NKG2D,DAP10 and ERK in NK cells were determined by immunocytochemistry and reverse transcription-polymerase chain reaction(RT-PCR) methods before and after VIP intervention.The ttest was used for comparison between two groups and rank transformation nonparametric test (Kruskal-Wallis H test) for count data analysis.Results The expression levels of NKG2D,DAP10 and ERK in the inflammatory cells of gastric adenocarcinoma were significantly lower than those of para-carcinoma tissues (H=30.640,24.910,20.320,all P＜0.01).The expression levels of NKG2D,DAP10 and ERK were much lower in poorly differentiated gastric adenocarcinoma (H =4.049,11.830,8.118),at stage Ⅲ-Ⅳ (H=4.275,11.560,7.686),and the expression levels of NKG2D,ERK were lower with lymph nodes metastasis (H=6.513,6.064,all P＜0.05).The expression levels of NKG2D,DAP10 and ERK in gastric adenocarcinoma tissues and inflammatory cells were not correlated with gender,age and the location of lesions (all P＞0.05).The expression levels of NKG2D,DAP10 and ERK in NK cells was decreased after VIP intervention (protein:H=12.438,4.798,13.745,mRNA:F=337.640,638.579,1055.015,all P＜0.05).Conclusion Gastric adenocarcinoma may downregulate the expression of NKG2D,DAP10 and ERK in NK cells through VIP secretion,which may be involved in the immune escape of gastric cancer.

The DNA demethylase genes are widespread in plants. Four DNA demethylase genes (LJDME1, LJDME2, LJDME3 and LJDME4) were obtained from transcriptome dataset of Lonicera japonica Thunb by using bioinformatics methods and the proteins' physicochemical properties they encoded were predicted. The phylogenetic tree showed that the four DNA demethylase genes and Arabidopsis thaliana DME had a close relationship. The result of gene expression model showed that four DNA demethylase genes were different between species. The expression levels of LJDME1 and LJDME2 were even more higher in Lonicera japonica var. chinensis than those in L. japonica. LJDME] and LJDME2 maybe regulate the active compounds of L. japonica. This study aims to lay a foundation for further understanding of the function of DNA demethylase genes in L. japonica.

Thirty patients with objective benign paroxysmal positional vertigo (BPPV) (objective group) and 30 patients with subjective BPPV (subjective group) visiting Department of Neurology from January 2014 to June 2015 were enrolled in the study.There were no significant differences in semicircular canal,gender,age,related factors of secondary onset (P ＞ 0.05).Patients were evaluated with dizziness handicap inventory (DHI) before and after the maneuver therapy.The average DHI subscale scores before treatment of all patients were higher than those after treatment (11.8 ± 2.8 vs.2.5 ± 1.8,P ＜ 0.05);there was 11.9 ± 3.0 vs.2.5 ± 2.1 (P ＜ 0.05) for objective group and 11.6 ± 2.6 vs.2.5 ± 1.6 (P ＜ 0.05) for subjective group.The scores of PI,F5,P11,P13,P25 (mean rank order value) after treatment were decreased compared to corresponding initial scores in two groups(objective group:21.30 vs.39.70,19.55 vs.41.45,19.00 vs.42.00,18.65 vs.42.35,15.90 vs.45.10,respectively,subjective group:20.93 vs.40.07,18.07 vs.42.93,19.77 vs.41.23,18.57 vs.42.43,16.47 vs.44.53,respectively;all P ＜ 0.05).The results indicate that maneuver therapy is effective both for objective and subjective BPPV patients;and subscale of DHI can be used to evaluate the degree of illness in patients with BPPV.

Objective By comparing clinical efficacy of percutaneous vertebroplasty with high viscosity bone cement and percutaneous kyphoplasty with conventional low viscosity bone cement for osteoporotic vertebral compression fractures,to explore the clinical value of treatment of osteoporotic vertebral compression fractures with high viscosity bone cement.Methods In this prospective study,100 patients with one-level OVCF and older than 70 years were diagnosed and treated in our hospital from June 2014 to December 2015,which were randomly divided into 2 groups (50 patients in each),applied unilateral percutaneous vertebroplasty with high viscosity bone cement and bilateral percutaneous kyphoplasty with conventional low viscosity bone cement,respectively.Collected data of basic clinical informations (including age,gender,body mass index,etc.),preoperative and postoperative visual analog scales (VAS),Oswestry disability questionnaire (ODI),operative time,intraoperative X-ray times,the amount of bone cement injection,leakage of bone cement,anterior vertebral height and kyphosis (Cobb angle) of preoperative and postoperative,and statistical analysis.Results There were significant improvement of both VAS and ODI scores of the postoperative 1 day,3 months and 12 months compared with preoperative,but there was no significant difference between two groups;The operative time,intraoperative fluoroscopy times,bone cement injection amount and intraoperative bone cement leakage of high viscosity bone cement PVP group were significantly less than those of low viscosity bone cement PKP group (P＜0.05);the anterior vertebral height and kyphosis improved,and there was a significant difference between preoperative and postoperative of low viscosity bone cement PKP group (P＜0.05).Conclusion The efficacies of unilateral PVP with high viscosity bone cement and bilateral PKP with conventional low viscosity bone cement for osteoporotic vertebral compression fractures are the same,which can relieve pain and improve function of life significantly.However,the high viscosity bone cement PVP is simpler surgical procedure,with shorter operative time,less intraoperative radiation,less bone cement leakage rate.The domestic bone cement injection device can significantly reduce the cost of surgery,it worth to be widely used for clinical application.

The Expert Consensus on the Diagnosis and Therapy of myelodysplastic syndrome (MDS) (2014) will be interpreted in this paper focusing on whether it is scientific and reasonable.Some advises and views will be put forward,hoping that it will be useful to improve the diagnostic and therapeutic ability on MDS for clinicians in our country.
