In Korea where night soil used as fertilizer for crops and vegetables they may be contaminated simultaneously with infective stage of ascaris, hookworm and other helminthes. In this circumstance, the ascaris eggs and hookworm larvae may adhere to certain kinds of leafy vegetables(Chyu, 1957) and may remain viable even after pickling in brine or in other substances (Han and Ko, 1952; Soh, 1960). It has been shown that Heliogen(Thitasut, 1961) will kill the parasite eggs and larvae, and Sodium nitrite (Kozai, 1960; Kim and Soh 1964) and Thiabendazole (Kutsmi, 1964 and 1965; Kim et al., 1966) also had ovicidal action. The present study has been planned to study the ovicidal and larvicidal actions of several chemicals in Korean pickle juice(Kimchi) and the results are summarized as follows: Heliogen solution(iodine 100 ppm) did not destroy the ascaris and hookworm eggs within 30 minutes at the temperature of 15-30 C, but the hookworm larvae were killed within 3 minutes under the same conditions. Sodium nitrite which was added to pickle juice at the rate of 0.1 percent destroyed more than 90 percent of ascaris fresh eggs within 10 days and the embryonated eggs were destroyed within 2 days. Hookworm eggs were destroyed within 3 days and larvae were killed within 7 hours. Thiabendazole which was added to pickle juice at the rate of 0.1-0.025 percent inhibited the infectivity of ascaris embryonated eggs within 24 hours and hookworm eggs were destroyed in the same concentrations of the reagent. Hydrogen ion concentration of pickle juice which was mixed with Sodium nitrite (0.1 percent) was not changed for 10 days. 1.5 mg of Sodium nitrite or Thiabendazole produced no toxicity in kidney and intestine of mice but slight pathological changes in the liver of the same animal.

We report a case of myiasis on the left submandibular area after radiation therapy for squamous cell carcinoma. A 77-year-old man visited our hospital, 7 weeks after complicating a course of radiation therapy, had a 7cm diameter necrotic ulcer on the left submandible. The margin of the lesion were undermined by numerous interlinking passages containing 20 maggots. The maggots were removed by simple extraction and were identified as Calliphoridae Lucilia. The probable origin of the myiasis in this case was attributed to poor environmental condition and general weakness of the patient. Oppotunistic cutaneous myiasis can be prevented by protecting open wounds from flies, wound should not be left without a covering capable of excluding flies, especially in the summer month.

PURPOSE: To evaluate the efficacy of arthroscopic microfracture in patients with focal full-thickness cartilage defects in the osteoarthritic knee. MATERIALS AND METHODS: Seventy-six patients were enrolled in this study. They were divided into group I (n=38) who underwent microfracture plus meniscectomy and group II (n=38) who underwent only meniscectomy. Clinical and radiological evaluations were performed. RESULTS: At the time of the three-year follow-up, a total of five failures (6.6%) were reported: four patients in group I and one in group II. The two groups showed no significant difference in the Lysholm score, the Tegner activity score and the visual analog pain scale score at three years after surgery. However, at the time of the three-month follow-up, group II showed significantly more improvement in the Tegner activity and the visual analog pain score compared with group I. CONCLUSIONS: In the osteoarthritic knee, additional microfracture did not confer any additional benefit to meniscectomy.

In Korea, Clonorchis sinensis infection is still highly prevalent because case detection in the field is difficult and the detected cases used to be incompletely cured due to treatment failure. The present study tried to control clonorchiasis in an endemic village by repeated treatments with praziquantel every 6 months and to evaluate sonography as a diagnostic measure. By stool examinations, the egg positive rate in the endemic village was 22.7%, but it decreased to 19.6% at 6 months, 15.1% at 12 months. 12.2% at 18 months, 6.3% at 24 months, 11.4% at 30 months, and 6.3% at 42 months after the beginning of repeated praziquantel administration. The sonography showed 61 (49.6%) positive cases of 123 screened residents: among egg-positives the sonography positive rate was 52.2% and among egg-negatives it was still 49%. The rate among cured cases was 64.3% after 6 months, 50.0% after 12 months, 50.0% after 18 months, and 66.7% after 24 months. In a non-endemic village, 64 residents were found egg-negative by fecal examination, but 20 (31.3%) of them were positive by sonography. The present findings indicate that control of clonorchiasis in an endemic village by repeated praziquantel treatment for 42 months is still insufficient and sonography is of little value for diagnosis of clonorchiasis.

BACKGROUND: The median effective concentration, EC(50), for the supraglottic airway (predicted plasma concentration equilibrated with the effect site at which supraglottic airway insertion is successful in 50% of patients) of propofol in children was not known. The aim of this study was to determine the EC(50) with a target-controlled infusion of propofol to insert a classic laryngeal mask airway (LMA) and laryngeal tube (LT) in pediatric patients. METHODS: Pediatric patients scheduled for urologic, orthopedic, or plastic surgery lasting less than 2 hr under general anesthesia were enrolled in this study. The patients were divided into two groups in a randomized, double blind manner. After applying a standard cardiorespiratory monitor, anesthesia was carried out with a target-controlled infusion (TCI) using the stelpump program. The initial target concentration of propofol was 5microgram/ml, and in accordance with Dixon's up-and-down, the propofol concentration for consecutive patients in each group was varied with increments or decrements of 0.2microgram/ml based on the previous patient insertion result. RESULTS: The EC(50) of propofol required to insert a LMA and LT was 5.5+/-0.2microgram/ml and 5.6+/-0.2microgram/ml, respectively. CONCLUSIONS: The EC50 of propofol is similar for LMA and LT.

PURPOSE: The purpose of this study was to evaluate the effect of the temporal changes in brain lesions onserial MR images during the course of Leigh syndrome. MATERIALS AND METHODS: We retrospectively reviewed 11 MRimages in four patients diagnosed as suffering from Leigh syndrome on the basis of clinical features, MRIfindings, and biochemical data. Follow-up and earlier, MR images were compared and temporal changes in lesionswere analyzed, with particular attention to location, size, signal intensity, and contrast enhancement. RESULTS: Initial MRI showed that in order of frequency, the following were involved : bilateral putamina (4/4), caudatenuclei (2/4), the brain stem (2/4), medial thalamic nuclei (1/4), and the cerebral cortex (1/4). In two patients,the size of acute putaminal lesions, as seen on follow-up MRI, decreased in the short term (within two weeks); inone patient, strong contrast enhancement of the lesions was observed twelve days after initial MRI. Long termfollow-up MRI, over 7 - 19 months, showed newly developed lesions (2/4) and atrophy (2/4) or obliteration ofprevious lesions (3/4) in the basal ganglia, thalami, and brain stem. CONCLUSION: Serial MRI demonstratedtemporal changes in brain lesions during the course of Leigh syndrome. On follow-up MRI, the appearance ofbilateral lesions in basal ganglia and the brain stem, not present on initial MRI, may be helpful corroborativeevidence to support a diagnosis of this syndrome.

The diagnosis of acute inflammation of the placenta, represented as acute chorioamnionitis, is important in that it is associated with a poor clinical outcome for both the mother and the fetus, including major perinatal morbidities such as sepsis, respiratory distress syndrome, and CNS damage. However, current medical trends in Korea seem to overlook the significance of a histopathological diagnosis of acute placental inflammation, mainly due to the indifferences of clinicians and pathologists. Since late 1993, histopathological examinations have been performed on preterm placentas at Seoul National University. These examinations have demonstrated acute placental inflammation in a significant number of cases. In the present study the incidence of acute placental inflammation was analyzed in 521 placentas which were submitted for pathological examinations in 1995. Examinations were performed to provide basic information on the incidence and profile of acute placental inflammation in this hospital and, thereby, to emphasize the significance of histopathological examinations of the placenta in the routine surgical pathology service. Among the 521 placentas, acute inflammation was found in 194 cases (37.2%). In preterm placentas acute inflammation was found in 39.6% of the cases (67/169), while 36.1% (127/352) of term placentas showed acute inflammation. Taking the delivery mode into account, 26.3% (49/186) of the placentas delivered by cesarean section showed acute inflammation, while 43.3% (145/335) of the transvaginally delivered placentas showed inflammation. The present analysis demonstrates the existence of acute inflammation in a significant proportion of placentas with different clinical settings. The importance of a histopathological examination in routine hospital practice should be emphasized.
