1.Advances in research on the diagnosis and therapy of lung cancer with bone metastases

Bone metastases in advanced lung cancer patients are common and affect the life quality seriously. So early diagnosis and appropriate treatments are important. The methods of diagnosis include X-ray, CT, MRI, ECT and PET, which are different in sensitivity and specificity. The gene microarray technique is another advance for detecting micrometastasis. The gene analysis also can help understanding the biological characteristics of the primary tumor. The treatment of bone metastases in lung cancer is various, including systemic and local therapeutic means. Surgery is often used to prevent or treat the compressions and bone fractures. Radiotherapy is one of the most effective methods of pain relief. Diphosphonate provides a new convenient and effective way of alleviative treatment. In addition, supportive and symptomatic treatments should be keep in mind.
2.Pterygium Surgery.

The author operated 34 cases of pterygia by Newton's method, wihich combined Mc Raynold's transplantation with limbal incision, in these 34 case, 4 cases were recurred, and in other 2 cases, a few vessels only found on the operated area of the cornea. Among these 6 cases, one had trachoma, 3 had chronic cat conjunctivitis. Auther thinks that the chronic inflammation such as trachoma or chronic conjunctivitis is very important factor in the recurrence of pterygia. This fact also encourages the author who believes that the inflammation is rather more important factor in the etiology of the pterygia than the degensration.

3.Marfan's Syndrome.

We are Presenting of two cases of Marfans syndrome. First acse; 22 year old boy, height 262 cm, weight 40.9 kg, typical skeleton, hands and feet. The span of the outstreched arm, from right finger to the other, is 165 cm, exceeds by 3 cm than the height. Abnormal flexibility of Joints in the both thums. Vision; O.D. 0.1 with-sph.7.0 D., O.S. 0.0 with-sph.6.0 D. Lens; typically dislocated, upward and inwad and inward iridodonesis is marked. The slit lamp examination of the zonular fiber is very attractive, In the left eye, dislocation is more severe than the other, and the zonular fiber are more elongated. The fibers are seen very clearly, Just like fine whitish silk contrast to its black background. At the 5 o'clock Position pupil margin in th left eye, one of them was revered and distorted. No significant fundus signs were seen. His parents and four brothers all have Normal visions and skeletons Normal cardiovascular system, even x-raypicture, balium in in esophagus, and normal E.C.G. picture. The second sase; 5 year old girl, Height 120 cm, weight 20.6 kg, typical skeletor, hands and feet. Vision both 0.2, 0.2 and 0.3 with -sph.9.0 D. Left eye; typical dislocated lens, upward and outward. iridodonesis is also found, no significant fundus picture. Normal cardiovascular system, even x-ray picture, balium in esophagus, and normal E.C.G. picture. Father; normal vision and skeleton. mother; typical skeleton, hands and feet, but normal vision, said her grandmother also had delicate bony structure and very poor vision, died known causes. The literature is briefly reviewed and the Post- operative complication esp. detachment of retina, emphasized.

5.Effects of p,p’-DDE on Intracellular Calcium in Sertoli Cells of Rats in vitro

Objective To study the effects of p,p’-DDE on intracellular calcium in the Sertoli cells of rats in vitro. Methods The primary culture of the Sertoli cells separated from the testicle tissues of rats was conducted, and the cells were treated with p,p’-DDE at the doses of 10, 30 and 50 ?mol/L. Fura-2/AM was used as the probe to examine the changes of intracellular calcium by two-wavelength calcium fluorescence system. Results Compared with the control, in 50 ?mol/L group of p,p’-DDE, the intracellular calcium level significantly increased. Conclusion p,p’-DDE may disrupt intracellular calcium homeostasis which will induce calcium overload in Sertoli cells in the testicle tissues of SD rats.
6.Clinical Observation of Acute Suppurative Arthritis of Hip in Children

The authors did clinical analysis of 34 cases of suppurative athritis of hip for period of three years from Jan. 1973 to Jan. 1976 at Dept. of Orthopedics, Catholic Medical Center. Through this study following results were obtained. 1. Most of the patients were under the age of 6 years including twenty-six males and eight females. 2. Lag period to treatment in 10 cases was within 3 days, 14 cases within a week: most of these series were treated within a week. 3. Frequent clinical symptom and signs were pain on hip 34 cases, fever 30 cases, limited motion of hip 24 cases and flexion contracture 8 cases. 4. Causative micro-organism was isolated in the cases of diseased hip: Staphylococcus aureus in. 24 cases, β-hemolytic streptococcus in 8, pneumococcus 1, and Gram positive bacillus 1 cases. 5. The laboratory findings included leucocytosis over 15,000 per mm in 31 cases, increased erythrocyte-sedimentation rate 15mm per hour in 33 cases. All the cases were leucocytosis with increased ESR but. 5 were only increased ESR. 6. In authors experience early incision and drainage with specific antibiotics were a choice of treatment in the acute supprative arthritis of hip 7. Result become progressibly less satisfactory in proportion in the length of time after onset at which treatment is started. a) When sufficient time has elapsed for adhesion to develop the pocket to the head thus can not be drained properly by the active movements. Indeed, it is possible, that may be actually harmful in promoting absorption from such pocket. b) In case of long standing erosion of cartilage with denuded bone areas hinder or prevent efficient active movements. c) In case of long duration, blood stream infection is an ever present danger, as well as a aggrevation of existing organic disease by the infective process.

9.Effect of Clonidine on the Blood Pressure, Heart Rate and Plasma Catecholamine Concentration during General Anesthesia.

BACKGROUND: Induction of general anesthesia with tracheal intubation and skin incision are potent stimuli that can induce increased sympathetic activity, heart rate and blood pressure. We compared the hemodynamic response and catecholamine concentration during general anesthesia with intravenous clonidine pretreatment, a centrally acting -2 adrenoceptor agonist, and without pretreatment. METHODS: Forty ASA I or II patients aged from 20 to 60 years undergoing elective surgery were randomly allocated to two groups. In group I, 5 ml of 0.9% normal saline as control administered intravenously 10 minutes before induction of anesthesia, and anesthesia was induced with thiopental sodium, succinylcholine and maintained with N2O (2.5L/min)-O2 (2.5L/min)-enflurane (1.3~1.8 vol%). In group II, 4 g/kg clonidine diluted in 5ml of normal saline administered intravenously 10 minutes before induction of anesthesia, and anesthesia was induced with thiopental sodium, succinylcholine and maintained with N2O-O2-enflurane (0.5~1.0vol%) and 2 g/kg/hr clonidine was continuously infused. We measured blood pressure, heart rate and plasma catecholamine at preinduction, 1 minute after intubation (T1), 1 (T2) and 30 minutes after skin incision (T3) and compared with group I. RESULTS: There was statistical significance in systolic and diastolic pressure at T1 between two groups. There was statistical significance in heart rate, epinephrine and norepinephrine at T1, T2 and T3 between two groups. CONCLUSION: The elevation of blood pressure, heart rate and plasma catecholamine accompanying tracheal intubation and skin incision may be prevented by administration of intravenous clonidine.
