1.The Effect of H1 and H2 Antihistaminics on Dermographism.

This study was designed to evaluate the therapeutic effect of H2 and H2 antihistaminics on dermographism. Twenty four patients with dermographism were allocated on a random to one of the following 3 treatment regimens. A) chlorphenirarnine and cimetidine (H1+H1) B) chlorpheniramine alone (H>) C) cimetidine alone (H) The patients cutaneous response to the dermographometer was assessed at weekly intervals for four weeks. The results were as follows: 1. The combinded treatment with chlorpheniramine and cimetidine was significantly more effective in inhibiting wheal and flare than the cimetidine alone (wheal p<0.01, flare p<0.02). 2. Following comparisions were not statistically significant: chlorpheniramine+ cimetidine vs chlorpheniramine (p>0. 05), chlorpbeniramine alone vs cimetidine alone(p>0.05). 3. Main side effects were drowsiness (3 cases) and mild gastric disturbance (2 cases) on hlorpheniramine alone.

2.A Case of Tansient Hyperphosphatasemia.

We examined a male infant with transient increase in alkaline phosphatase(ALP) activity. The 25-month-old infant was admitted to a local hospital because of pneumonia of unknown etiology. Initial laboratory investigation revealed a serum ALP of 11,260U/L, which was comparable to that of bone isoenzyme on the electrophoresis pattern, There was no evidence of hepatitis, skeletal or intestinal diseases. The hyperphosphatasemia disappeared 3 months later. We report a case of transient hyperphosphatasemia diagnosed inci- dentally in the course of management of pneumonia. 3.A Clinical Observation on Systemic Lupus Erythematosus.

BACKGROUND: Systemic lupus erythematosus(SLE) is a clinical syndrome of unknown causes characterized by inflammat.ion and multisystem involvement. Since Hargravas describe the LE cell in 1948, a profound change has occured in the concept of SLE. Many studies of the large series have been reported in the West, but few collective studies have been reported in Korea. OBJECTIVE: The goal of this study was to elucidate the clinical and laboratory characteristics of SLE in Korean people. METHODS: We carried out retrospective study of 64 patients, who were diagnosed as SLE on the base of the 1982 revised bacteria for the SLE by American Collepe of Rheumatology(formerly, American Rheumatism Association), at Korea University Hospital from 1982 to 1991. The results were compared with those of previous studies in Korea and western countries. RESULTS: 1. In 64 patients with SLE, female patients outnumbered male by 6.1 to 1 and the mean age of onset was 28.6 years. 2. The initial manifestations were arthritis or arthralgia, 25%; fatigue, malaise, weight, loss, 21.8%; facial rash or blush, 12.5%, fever, 12.5%. 3. The major clinical manifastations were hematologic(90.6 %), muccutaneous(89%), musculoskeleta (79.7%), and renal(73.4%) involvemant. 4. The ANA test were positive in 83.9% of the patients, and homoenous pattern was most common. There was no significant difference of organ involvement according to the nuclear patterns of ANA, but cardiopulmonary, renal involvement, hypocomplement,emia and positive anti-ds DNA were less frequent in ANA neghative group. 5. In 73.7% of the patients, the titers of anti-ds DNA antibody was elevated. The amount of anti-ds DNA Ab in serum did not always correlate with disease activity. 6. Photosensitivity, arthritis or arthralgia and CNS manifestatioris were less common, but hematologic findings, including anenia and leukopenia were more common in Korean than western reports. CONCLUSION: Patients with SLE are heterogenous in the natural h story of their disease, which is not an uncommon disease in Korea. Continued efforts to clarify the immunopathologic basis for SLE should eventually lead to more effective treatment.

6.A Study on 657 Skin Tumors.

A study was made on 657 cases of skin tumors, which were obtained mainly as surgical pathology specimens during the period of 12 years and 4 months from January, 1960 to April, 1972, at Departments of Dermatology and Clinical Pethology, Catholic Medical Center in Seoul, and the results were as follow; 1) Of the 657 cases of skin tumors, 470 cases (71. 5%) were benign tumors, 26 casea (4.0%) premalignant lesions and 161 cases (24. 5%) malignant tumors. 2) The ratio of male vs. female was 1: 1 for benign tumor, 1: 1.9 for premalignant lesion and 1. 9: 1 for malignant tumor. 3) Among the benign tumors were 144 cases of cysticercosis, 68 cases of epidermal inclusion cyst, 46 cases of hemangioma and 45 cases of lipoma. 4) Arnong the premalignant lesions were 13 cases of leukoplakia, 5 cases of Nevus sebaceous (Jadassohn) and 4 cases of Bowen's disease. 5) The average age of malignant tumor was 53. 2 years for squamous cell carcinoma, 57. 2 years for basal cell carcinoma, and 35. 6 years for malignant melanoma. 6) The sites of predilection of the squamous cell carcinomas were penis (30. 6%), face (27. 4%) and upper and lower extremities (12.9%). The site of predilection of the basal cell carcinomas was face (88. 5%)., 7) The ratio of squamous cell carcinoma vs. basal cell carcinoma was 2. 4: 1. 8) Among the metastatic cancers of the skin the more frequent primary sites were stomach (3 cases), lung (2 cases) and liver (2 cases).

9.Expression Patterns of Cytokeratin and Involucrin in the Epidermis of the Artificial Skin Reconstructed by Cultured Cells.

To investigate differentiation and growth process of keratinocytes in organotypic cultured skin, we carried out immunohistochemical studies for cytokeratin (CK) 10, 14, 16, 17 and involucrin in the cultured skin. In normal skin CK14 and CK10 were detected in the basal and all suprabasal layer, respectively, whereas in artificial skin CK14 was detected up to the middle of spinous layer but CK10 expressed from the middle of spinous layer. The detection of involucrin in normal skin was from the upper spinous layer but found from lower spinous layer in the artificial skin. Both CK16 and CK17 did not expressed in in vivo skin but expressed weakly in the spinous layer of artificial skin. It is therefore concluded that the characteristics of basal cell were maintained in the several, lower layers of the sartificial skin. The growth and differentiation steps of the skin were similar to those of in vivo although differences were seen in the expression level.

10.Intratemporal facial nerve neuromas and their mimics; CT and MR findings.

CT and MR findings of nine cases with intratemporal facial nerve nouromas were described and compared with CT findings of 3 cases with facial nerve palsy and facial nerve canal erosion which may mimic facial nerve neuroma. The tympanic segment of the facial nerve was involved in 8 cases, mastoid segment in 7 cases and labyrinthine segment in 5 cases. The lesions were easily diagnosed with high resolution CT with bone algorythms by showing the expansion of bony structures along the course of the facial nerves. In 4 cases with large vertical segment tumors. Extensive destruction of mastoid air cells and external auditory canals posed difficulty in making a diagnosis. Two out of 5 cases with labyrinthine segment imvolvement were presented as middle cranial fossa masses. MRI with enhancement was performed in 4 cases and was useful in characterizing the lesion as a tumor with its superior sensitivity to enhancement. Three cases of facial neuroma-mimicking lesion including post-inflammatory perineural thickening. Perineural extension from parotid adenoidcystic carcinoma, and congenita: cholesteatoma showed irrgular erosion or mild expansion of the facial nerve canal which may be helpful for differential diagnosis from neuromas.
