The purpose of this study was to develop an osteogenic, biodegradable material using polymer and BMP. It was designed to have structural function and be moldable, for the reconstruction of load bearing areas and deformities of various configurations. Bone apatite was added to Poly(L-lactide)(PLLA) and made porous for osteoconductability and ease of BMP loading. The materials, with or without BMP purified from porcine bone matrix, were evaluated in cranial bone defect models in rats for biocompatibility and bone regeneration capability. The following results were obtained: The PLLA-BMP material with BMP added to the polymer showed 30% healing of cranial bone defects in rats during the 2 weeks to 3 months period of observation. The moldable PLLA agent without BMP also showed 25% bone healing capacity. Although new bone formation was incomplete in the critical size defect of rat cranium, it can be concluded that the unique moldability of those agents makes them useful for the reconstruction of various bone defects and maxillofacial deformities.

Angiosarcoma is a rare malignant vascular tumor of endothelial cell origin. Most lesions of cutaneous angiosarcoma occur over the face and scalp. We experienced a case of angiosarcoma of the scalp in a 63-year-old man. Multiple pea-sized erythematous papules and nodules developed on the forehead 6 months ago. Thereafter, the lesions have extended to make violaceous, compressible and hemorrhagic plaques. On histologic examination, there were irregular, often anastomosing vascular channels lined by the atypical endothelial cells in the dermis.

BACKGROUND: Hand-foot-mouth disease (HFMD) is mainly caused by the infection of coxsackievirus A16. But recently several epidemics of HFMD with meningitis or myocarditis due to enterovirus 71 have been reported in Southeast Asia. It was necessary that the possibility of enterovirus 71 epidemic in Korea should be ruled out. This study was designed for the determination of causative agents of HFMD in Cheju province in the spring of 1998. METHODS: Serum specimens were collected from 45 pediatric patients with HFMD at Cheju Hankook Hospital in March and April,1998. Virus isolation was performed with RD cell culture through up to three passages. Reverse transcription-PCR and nucleotide sequencing were performed by the method of Oberste et al.(J Clin Microbiol 1999;37:1288-93). The serotypes of viral isolates were determined by BLAST program of National Center for Biotechnology Information, U.S.A. RESULTS: Virus could be isolates from 4 patients, whose age was ranged from 11 months to 3 years. All of 4 viral isolates showed about 430-bp product of RT-PCR using primers 011 and 012. The serotype showing the highest similarity with the nucleotide sequences of all of these viral isolates was coxsackievirus A16. CONCLUSIONS: The causative enteroviral agent of HFMD in Cheju province in the spring of 1998 was coxsackievirus A16. We could not detect enterovirus 71 from the patients' sera in Cheju Province in the spring of 1998.

Isovolumic phase indices of right ventricular(RV) systolic function were evaluated in 9 normal control cases(Group I) and 9 patients with mitral stenosis(Group IIa) and 8 patients with mitral stenosis and aortic regurgitation(Group IIb). RV systolic pressures of all patients in Group II(n=17) were over 40mmHg RV dp/dt max, Vmax and Vpm were measured by digitizing method and V(CE10) were measured by manual tangent method. RV isovolumic contractile functional indices were greater in Group II(n=17) compared with those in Group I(n=9) : RV dp/dt max(mmHg. Sec(-1)) 347.8+/-104.4(mean+/-SD) VS 230.5+/-65.5(p<0.05), Vmax(Sec(-1)) 30.9+/-10 VS 20.1+/-5.18(p<0.05), Vpm(Sec(-1)) 22.96+/-9.63 VS 14.8+/-4.04(p<0.05) and V(CE10)(Sec(-1)) 20.4+/-7.11 VS 12.5+/-5.31(p<0.05). But no significant differences were noted between Group IIa(n=9) and Group IIb(n=8) in these indices. In all cases(n=26), significant correlations were noted between V(CE10) and Vmax(r=0.90), V(CE10) and Vpm(r=0.85). We summurized that RV contractile function is preserved in many cases with mitral stenosis and V(CE10) may be used as an convenient index for RV contractile function.

Basal cell carcinoma is the most common skin cancer, especially on the eyelid and nose. As it rarely invades to a underlying bone or metastasizes an distant site, and is usually found at an early stage, it is regarded as a curative disease. However, basal cell carcinoma on the eyelid and nose may be resected incompletely due to efforts to preserve important structures or as a result of esthetic considerations. We experienced two cases of basal cell carcinoma with local invasion to underlying bone. One was a recurred case on the nasal area extending to the nasal bone arts medial wall of the ethmoid sinus. The lesion was widely resected and covered with a radial forearm free flap. The other was on the eyelid extending to the orbit. It was treated with orbital exenteration and resection of the involved eyelid. The defect was reconstructed with the temporalis muscle flap with split-thickness skin graft. These patients were followed up for 7 months with no evidence of recurrence. Since basal cell carcinoma can invade to the bone and metastasize to a distance site, it should be resected radically in the regions of the eyelid and nose.

We studied the effect of retinoic acid, a potent differentiation inducer, on the proliferation and invasiveness of hormone resistant prostatic cancer cell lines, PC-3 and DU-145. Cellular growth measurement by MTY assay, flow cytometry for cell cycle analysis, Papanicolaou staining for examining the change of morphologic features and in vitro invasion assay using artificial basement membrane, matrigel, were performed under various concentration of all-trans-retinoic acid. Inhibition of cellular proliferation was retinoic acid dose dependent in both cell lines. Decreased S-phase and increased G-1 phase fraction were identified with time dependent manner in both cell lines. Less prominent chromatin and nucleotide, decreased nucleus/cytoplasm ratio were shown in Papanicolaou staining after 7 days culture with 10 uM of retinoic acid. In in vitro invasion assay, PC-3 cells showed decreased netlike formation and penetration though matrigel, and DU-145 cells showed decreased colony formation with 10 uM of retinoic acid. These findings suggest that the retinoic acid could ave the possibility of clinical application in hormone resistant prostatic cancer patients as a new therapeutic modality, differentiation therapy.

An autopsy case of holoacardius hemisomus acephalus is reported. She weighed 2,190 gm and the height was 38 cm. The head and upper extremities were absent, while the vertebrae and lower extremities were relatively well developed, but severely edematous. The heart, lungs, stomach, liver, spleen, and pancreas were missing, but the lower abdominal organs including kidneys, adrenal, urinary bladder, and genital organs were present. The intestine was blind-ended at jejunal level but opened into a normal anus. The umbilical cord had two arteries and one vein.
