1.A Study of the Isolation of Tubercle Bacilli from the Patient's Urine with Genitourinary Tuberculosis by the Inoculation into the Korean Chipmunks.

The detection of the tubercle bacilli is absolutely necessary to make definite diagnosis of patients with genitourinary tuberculosis. Recently it has been reported that the detection rate of tubercle bacilli from the patient's urine is decreasing because of abuse of the anti-tuberculous drugs and the resistance developed in the experimental animals such as mouse and guinea pig (Marks, 1972). Therefore the necessity of the development of better diagnostic methods has been mandatory. Recently it was reported that the Korean chipmunks(Tamias sibiricus asiaticus. Gmelin) are highly susceptible to tubercle bacilli (Chang et al., 1971). The specific objection of this study is to pursue the possibility of using the chipmunks for the early diagnosis of genitourinary tuberculosis. Twenty-four hour urines of 34 patients with suspected genitourinary tuberculosis were tested for tubercle bacilli with smear, culture, and inoculation ink chipmunks and mice intraperitoneally. The results are briefly summarized as follows: 1. 18 cases out of 34 patients with suspected genitourinary tuberculosis were confirmed to be positive by any means of stained smear, culture and animal inoculation. Of 18 proved genitourinary tuberculosis. 12 cases were confirmed by smear, culture and mouse inoculation, and on the other hand. 14 cases by chipmunks inoculation. 2. In the group of chipmunks inoculation, 8 cases were found to be positive on the 10th day and 14 cages on the 20th day. 3. These findings suggest that the period required for the detection of tubercle bacilli was shorter and the positive rate was higher than that of other animals and other methods. For the clinical applications of the Korean chipmunks as a diagnostic tools, further effort is considered to be necessary.

Phacomatosis pigmentovascularis (PPV) is a rare syndrome characterized by concurrent nevus flammeus (capillary malformation) and pigmentary nevus. According to current research, the major pathophysiologic mechanism in PPV is venous dysplasia with resultant compensatory collateral channels and venous hypertension. Arterial involvement is rare. We herein report our experience on renovascular hypertension, intermittent claudication, and severe rhabdomyolysis due to diffuse stenosis of multiple arteries in a patient with PPV type IIb associated with SWS.

5.Radiological Findings of Vascular Involvement in Behcet's Disease.

Angiographic findings were analysed in 11 cases of Behcet'sdisease which was diagnosed clinically in Seoul national University Hospital from July 1982 to June 1983. Arterial involvement was noted in 5 cases and venous involvement in 6 cases respectively. Arterial aneurysm and occlusion were arteriographic findings. Arterial aneurysm was noted at large arteries in all 5 cases and multifocal involvement was noted in 4 cases. Characteristic finding of arterial involvement was protruding saccular aneurysm. Venous involvement was thrombotic occlusion of deep veins in iliac, femoral and tibial areas in all 6 cases. Behcet's disease should be differentiated whenever these arterial or venous angiographic findings are revealed in a young adult.

6.Recurrent Priapism: 3 Cases Report.

Recurrent episodes of venoocclusive priapism have been described previously in patients who have had repeated exposure to a recognized stimulus such as intracavernosal injections of vasoactive agents, or pathologic conditions such as spinal stenosis or hematological disorders. Three patients were referred for evaluation of an unusual sequoia of an initial episode of idiopathic venoocclusive priapism, occurred with a frequency ranging from several times per month to once per year and were symptomatically disabling. We reviewed the characteristics and the possible pathophysiology of this syndrome and some therapeutic options in the management of these patients.

7.Prediction of Pulmonary Arterial Pressure by Pulsed Doppler Echocardiography.

Noninvasive prediction of pulmonary arterial pressure is of paramount importance in heart disease. To estimate pulmonary arterial pressure, several echocardiographic techniques, including abnormal pulmonary valve motion, prolongation of RV preejection period/RV ejection time ratio and contrast echocardiography have been proposed. Recently Doppler echocardiography has been known to detect intracardiac blood quantitatively. For assessment of the benefit of several indices by Pulsed Doppler echocardiography for mean pulmonary arterial pressure, 22 patients(mean pulmonary pressure> or =20mmHg; 11, <20mmHg; 11) were compared with the mean pulmonary arterial pressure by cardiac catheterization. In comparison of mean pulmonary arterial pressure(MPAP); 1) Right preejection period / RV ejection time RPEP/RVET;r=0.278 2) Right preejection period / Acceleration time RPEP/AT : r=0.654 3) Acceleration time(AT) AT=-1.55(MPAP)+154.37(r=-0.763) AT=-92.99(log MPAP)+239.41(r=-0.752) AT is less than 105 msec in 9 or 11 pulmonary hypertension and one of 11 normal individual. 4) Acceleration time/ RV ejection time AT/RVET=-136.36(MPAP)+83.31(r=-0.817) AT/RVET=-0.29(log MPAP)+0.81(r=-0.803) 5) (Right preejection period+Decceleration time) / AT (RPEP+DT)/AT=9.6(MPAP)-0.16(r=0.806) (RPEP+DT)/AT=3.86(log MPAP)-2.46(r=0.789) In conclusion AT/RVET, (RPEP+DT)/AT and Acceleration time of 105 msec are valuable indices to estimate mean pulmonary arterial pressure by Pulsed Doppler echocardiogram.

8.Two Cases of Leiomyoma on Urethrovaginal Septum.

Leiomyoma of the urethrovaginal septum is rare condition. There are only 14 documented cases of leiomyoma of the female urethra and less than 250 cases of leiomyoma of the vagina in the literature. The physical and cystoscopic features are helpful in establishing the benign nature and the extent of the lesions. This report describes two cases of leiomyoma of the urethrovaginal septum that were confirmed pathologically at our Severance Hospital.

9.A Case of Granulocytic Sacoma in Esophagus.

Granulocytic sarcoma.(chloroma) is a localized tumor mass composed of immature cells of the granulocytic series infiltrating an extramedullary site. The tumor may involve anywhere in the body, especially orbit, bone, epidural space and lymph node, but there is no case involved the esophagus. In a 34-year-old male presented with sudden anset of dysphagia, esophagoscopic examination revealed concentric narrowing of the esophageal lumen wirith alight granular mucosa in the midesophagus, which hiatologically proved to b a granulocytic sarcoma. Peripheral blood and bone marrow aspiration smears showed blastic crisis of chronic myelocytic leukemia. After the systemic chemotherapy, dysphagia and mid-esophageal luminal narrowing disappeared completely.
