2.Ultrasound guided reduction of childhood intussusception

Following on from diagnosis using ultrasound, hydrostatic reduction using saline enema under ultrasoundguidance was tried in 20 cases of childhood intussusception. The conclusions were as follows: 1. Success rates ofsaline enema under ultrasound guidance during a frist-8 month period(P1) and a second 10 month period(P2) were55.6%(5/9) and 54.5%(6/11) respectively. Average succes rate was 55%(11/20). 2. During the periods of P1 and P2,no case was reduced by barium enema in 6 cases of failed reduction with saline enema. 3. During the period of P2, 5 cases of successful redution with saline enema were confirmed by clinical follow-up without barium enema. 4.During the periods of P1 and P2, 9 cases of failed reduction with saline enema were operated, resulting in 6 casesof segmental resection and 3 cases of manual reduction. 5. The obvious advantages of this method are: 1. Noradiation hazard. 2. No fear of barium peritonitis. 3. Detection of leading point. 6. With above results, thismethod could completely replace barium enema. And ultrasonography should be the initial study in the evaluation ofintussusception in children and then if necessary, saline enema under ultrasound guidance should be done.

Authors retrospectively analyzed the CT findings of cystic tumors of the mediastinum in surgically confirmed13 cases at Seoul National University Hospital during the recent 3 years from August 1980 to August 1983. 6.EFFECTS OF THE ION BEAM ASSISTED DEPOSITION OF HYDROXYAPATITE ON OSSEOINTEGRATION OF THE ENDOSSEOUS IMPLANTS IN RABBIT TIBIAE.

A large increase in the use of thin film coating of hydroxyapatite(HA) in implant dentistry is driven by the desire to take advantage of the excellent biocompatibility and high strength of HA coating. The purpose of this study was to evaluate the effects of HA-coated implants by Ion Beam Assisted Deposition(IBAD) method in comparison to the sand-blasted and machined surfaces. Osteoblast culture test, removal torque test and histomorphometric analysis were performed and the following results obtained; 1. Examination of the osteoblast cultures displayed no difference in the secretion of alkaline phosphatase between the various specimen, but IBAD with pure HA specimen showed low alkaline phosphatase secretion(p<0.05). 2. Removal torque tests showed HA-coated implants by IBAD method to be similar in high value to the implants with sand-blasted surface than the implants with machined surface. And the ovariectomized group showed low mechanical test value than the normal group(p<0.05). 3. Histomorphometrical comparisons were performed on undecalcified ground sections. HA-coated implants by IBAD method demonstrated the highest mean bone-to-metal contact ratio on all threads and 3-best consecutive threads, and the implants with sand-blasted surface and implants with machined surface was in the next consecutive order(p<0.05). HA-coated implants showed slightly higher bone-to- metal contact ratio than sand-blasted implants, but no statistically significant difference was obtained between the two materials. The ovariectomized group showed lower value of bone-to-metal contact ratio than the normal group, but no statistically significant difference was obtained between the two groups. 4. Evaluation of bone volume on all threads and 3-best consecutive threads showed no statistically significant difference among the different surface treatment groups, but showed lower bone volume in ovariectomized group than in the normal group(p<0.05). According to these results, thin film coated implants with HA showed high bone contact ratio, bone volume and removal torque strength in the short term, but long term observation is needed.

8.Radiofrequency Tissue Ablation with Cooled-Tip Electrodes:An Experimental Study in a Bovine Liver Model on Variables Influencing Lesion Size.

PURPOSE: The purpose of this study was to determine the influence of various factors on the extent of thermal coagulation necrosis after radiofrequency (RF) tissue ablation using a cooled-tip electrode in bovine liver. MATERIALS AND METHODS: RF ablation was induced by a monopolar 500 KHz-RF generator (CC-1; Radionics,Burlington, Mass., U.S.A.) and an 18-G cooled-tip with single or clustered electrodes. The ablation protocol involveda combination of varying current, ablation time, power output, gradual or abrupt increase of this out-put, and pulsed radiofrequency techniques. The maximum diameter of all thermal lesions which showed a color change was measured perpendicular to the electrode axis by two observers who reached their decisions by consensus. Twenty representative lesions were pathologically examined. RESULTS: With increasing current lesion diameter also increased, but above 1500 mA no further increase was induced. Extending the ablation time to 9 minutes for a single electrode and 15 minutes for a clustered electrode increased lesion diameter until a steady state was reached. Higher power levels caused larger lesions, but above 100 W no increase was observed. Ample exposure time coupled with a stepwise increase in power level induced a lesion larger than that resulting from an abrupt increase. Continuous pulsed RF with a high current led to increased coagulation necrosis diameter. CONCLUSION: These experimental findings may be useful thermotherapy. The data suggest that all involved factors significantly affect lesion size: if the factors are better understood, cancer thermotherapy can be better controlled.

9.The level of buccal gingival margin around single and two adjacent implant restorations: a preliminary result.

STATEMENT OF PROBLEM: Little information is available about the buccal gingival level of multiple implant restorations. PURPOSE: This study was aimed to evaluate the relationship between width and height of buccal soft tissue around single and 2 adjacent implant restorations. MATERIAL AND METHODS: Four implant restoration groups (first and second molars, single second molars, posterior single restorations between teeth, and anterior single restorations between teeth) were randomly chosen from one dental institute. Each group comprised of 6 patients. After 6 months of function, silicone impressions were taken and stone models were fabricated for each restoration group. The stone models were cut in bucco-lingual direction at the most apical point of buccal gingival margin. The height and width of buccal supra-implant soft tissue were measured. One way ANOVA and Tukey HSD post hoc tests were performed to analyze the data obtained (P < .05). RESULTS: The most unfavorable width-height ratio was noted for the group, which was comprised of the second molar in the multiple adjacent (first and second molar) implant-supported restorations. The group also resulted in the shorter height of buccal supra-implant mucosa rather than that of anterior single implant restorations between natural teeth. CONCLUSION: To achieve a favorable level of buccal gingival margin, greater thickness of buccal supra-implant mucosa is required for the implant restorations without a neighboring natural tooth compared to the implant restorations next to a natural tooth.

10.A Clinical Study of Treatment of Unstable Ankle Fracture

Although the method to obtain a good results of treatment is still controversial in ankle fracture, most authors agree with open reduction and internal fixation for unstable ankle fracture. They also emphasize the importance of accurate reduction and rigid fixation of fractured lateral malleolus. Recently, the good results of early weight bearing and joint motion has been recognized. Therefore the cast immobilization for long duration is eliminated after surgery by many authorities. In addition to positive role of early ankle motion and weight bearing, we could expect the advantage of subsidence of stiffness, osteoporosis, and early returning to social activity. The seventy-eight unstable ankle fractures treated at Severance Hospital, Yonsei University College of medicine with open reduction and internal fixation were analyzed in clinical and radiological aspects. The following results were obtained. 1. In unstable ankle fracture, the good functional results were obtained with early joint motion and weight bearing after accurate reduction and rigid fixation. 2. The accurate reduction and rigid fixation for lateral malleolar fracture was the most significant factor in contributing to good results. 3. Syndesmotic ligament should be examined on each exploration of fractured fibular. In spite of the immediate weight bearing, the transfixion screw has remained in place without loosening or breakage. 4. The reduction of medial malleolar fracture was relatively easy and seems not to affect the results.
