1.A Case of Pretibial Myxedema with Diffuse Toxic Goiter.

Pretibial myxedema, which consists of localized cutaneous accumulations of acid mucopolyeaccharides, occurs in a small percentage of patients with diffuse toxic goiter. Pretibial myxedema may also occur during the course of nonthyrotoxic thyroid disease. A case of pretibial myxedema with diffuse toxic goiter in 64 year-old female is reported. She has been suffered from thickened yellowish waxy pIaques on both pretibial areas and left dorsum of foot. Initial skin lesions developed about 7 months ago, and increased its size and numbers insidiously. Diagnosis was confirmed by characteristic clinical, laboratory and histopathological findings. Literatures were reviewed.

2.Clinical and Histopathologic Study of Steroid Acne.

This study was performed to investigate clinical and histopathologic features of steroid acne, which was induced by systemic administration and topical application of corticosteroids. Thirty five cases of steroid acne visited to Department of Dermatology, Hangang Sacred Heart Hospital from September, 1g79 to June, 1984 were analyzed, and the results obtained can be summarized as follows: 1. The peak age of the subjects was third decade(42.9%) with an average age of 30. 1 years, and male to female ratio was l.9: 1,2. The skin lesions had unique clinical features that showed many, uniform sized, erythematous papules and pust;ules. 3 The predilection sites of steroid acne induced by systemic steroid therapy were anterior chest(93.1%), back(44,8%), neck(31.0%), shoulder(31.0%) and face (20.7%) 4 Among thirty five cases of steroid acne, twenty cases were induced by parenteral adrninistration of dexamethasone disodium phosphate(group A), nine cases by oral administration of prednisolone(group B), and six cases by topical application of three kinds of steroid creams(group C). 5. The mean induction time after starting steroid in group A(ll. 3 days) was shorter than those in group B and C(18.9 days and l4.8 days respective)y). The mean total dosage of used steroid in group A was 191. 3mg of dexamethasone disodium phosphate and that in group B was 515. 7mg of prednisolone. On histopathologic findings of twenty two skin biopsy specimens of the three groups, perivascular inflammatory reaction was the most common finding followed by intra-and peri-follicular inflammatory reaction, dermal vascular dilatation, necrosis of follicular epithelium, comedo, intraand periollicular abscess and rupture of follicle.

4.A Study of Pinprick Test with Food Allergens in Urticaria.

BACKGROUND: Prick test, a kind of skin test, is most frequently used in clinical procedure because of its simplicity and safety of application and better correlation with clinical history history as well as specificity and highty reproducibility. OBJECTIVE: Studies into the result and clinical significance of the pinprick test with relationship to food allergens as the contributing factor of urticaria. METHODS: We have examined the skin test with extracts of 45 food allergens(Bencard Ltd. In England) in three groups. 10 acute urticaria patients. 30 chronic urticaria patients, and normal controls. RESULTS: Of the 40 patients with urticaria, 23(57.5%) revealed positive reaction to one or more food allergens in comparison with 9(45.0%) of the control, ad there was no statistically significant difference(p>0.05). Positive reaction to food allergens in 15 food correlates was 11(73.3%), Which was higher than 11(44.4%) in 25 patients with no relation to food. and there was a statisticals difference (p>0.05). Food allergen groups which showed frequent positive reaction were fishes(35.0%), shell fishes(30.0%), cereals(27.5%), fruit(20.0%), vegetabies(17.5%), egg & milk(10.0%), and meats(7.5%) in that order. CONCLUSION: These results suggest that food allergen test might not be useful in evaluation the contributing factor of urticaria.

5.A Case of Pityriasis Lichenoides et Varioliformis Acuta.

We report herein a case of pityriasis lichenoides et variloiformis acuta in a 48- year-old man, who has wide spread erythematous macule, papule, vesicle, hemorrhagic vesicle and reddish-brown crust on the neck, trunk, axillae and both extremities. He was treated successfully with tetracycline 2. 0 gm a day for 4 weeks.

6.A Case of Scrotal Eczema with Calcified Nodules.

Cutaneous leishmaniasis (oriental sore) is usually a self-limited infection of the skin caused by the protozoan Leishmania tropica. The disease is endemic to the Mediterranean, Asia, Africa, and the Middle East. It has been seen in this country among many Korean technical experts and labourers working in the endemic areas of the disease. Our patient had acquired cutaneous leishmaniasis in Saudi Arabia and it had remained active for six months. He had been treated with antimony and metronidazole but failed because of severe side effects. And then we treated the patient witb cryosurgery and the skin lesions were followed by resolution with cosmetically acceptable scar in 4 months. The brief review of literature on the treatment of cutaneous leishmaniasis was undertaken.

8.Primary Cardiac Angiosarcoma: A case report and rewiew of literture.

We report a case of primary cardiac angiosarcoma in a 35 year-old woman. She presented with dyspnea, facial edema and neck vein distension during 1 month. The oval round large mass(8x4 cm) filling the right atrium infiltrated into the entire thickness of the lateral wall and extended to the vena cava and ventricle. The interatrial septum and pericardium seemed to be intact. Despite the surgical excision and adjuvant chemo-and radiotherapy, the patient died 8 months later due to tumor recurrence. The histologic findings of tumor varied from benign looking capillary proliferation mimicking granulation tissue to sarcoma composed of spindle cells. But anastomosing vascular channels lined by anaplastic polygonal cells and intracytoplasmic lumen containing red blood cells were characteristic findings. These tumor cells showed positive reaction to endothelial cell antigen and factor VIII-related antigen. The intercellular junctional complex and vascular channels formed by cytoplasmic process of tumor cells were identified by the ultrastructural study.

9.Eosinophilic Granuloma of the Adult Spine: Report of a Case.

Eosinophilic granuloma of bone well-documented in the literature. This lytic bone disease most commonly occurs in children, but a fraction of the cases does occurs in adult. When the disease occurs in adults, the bone most often affected include the skull, ribs, mandible, clavicle, pelvis, and long bone of the extremities. We report the case of a 31-year old woman with isolated eosinophilic granuloma of the thoracic spine. She had a 1-month history of progressive back pain that radiated to the right anterior chest. Authors performed resection of vertebral body lesion and soft tissue lesion and anterior fusion from T7 to T9. Immunochemical staining showed a positive S100 protein stain. EM demonstrated characteristic Birbeck granule. Postoperatively, the patient underwent a 5-day course of RT to the T6-T10 spine with 200cGY fractions (total dose 1000cGY). Postoperatively, the patient's back and chest pain had disappeared. There is no evidence of local recurrence or distant metastasis at 18 months follow-up. Though rare, eosinophilic granuloma of the adult spine could be considered in the differential diagnosis of osteolytic lesions.

10.Echocardiographic Doppler Mitral Valve Flow Velocity In Hypertension.

This investigation was performed to evaluate Doppler mitral valve flow velocity profiles in hypertensive heart disease for the detection of the left ventricular diastolic dysfunction which preceded the systolic dysfunction. Doppler derived parameters included; the duration of the early diastolic flow velocity peak(DF duration), the downward slope of the early diastolic flow velocity peak (EF slope) and the ratio between the height of the early and the late diastolic flow velocity peak (E-E'/A-A'). Doppler flow velocity profiles were measured in 19 normotensive subjects and 28 hypertensive patients. We found decreased EF slope in hypertensive patients (3.6+/-1.4./sec) in comparison with normotensive subjects (5.2+/-2.4m/sec) (p<0.01). In hypertensive patients, E-E'/A-A' value was significantly lower (0.8+/-0.2) than that of normotensive subjects (1.6+/-0.4)(p<0.001). DF duration was similar in both groups. In conclusion, we found that Doppler mitral valve flow velocity profiles, EF slope and E-E'/A-A', were decreased in hypertensive patients, which may be due to the left ventricular diastolic dysfunction.
