2.A Clinical Observation of Girdlestone Operation

We clinically reviewed and analysed 19 patients treated as Girdlestone operation with or without Batchelor osteotomy at the Department of Orthopedic Surgery, Busan National University Hospital from April 1979 to June 1981. The results were as follows: 1. The underlying disease were infected pinning and nailing of 8 cases, pyogenic hip of 4 cases, failed total hip arthroplasty of 4 cases, tuberculous hip of 2 cases and femur neck fracture of 1 case. 2. Satisfactory results were obtained in 7 or 8 cases treated as secondary Girdlestone operation over 50 years of age. 3. Postoperative shortening ogleg length was 3.2cm in average (from 1.7cm to 5.0cm), which was average 2.5cm at 12 weeks traction and average 3.7cm at 6 weeks traction. 4. Postoperative average range of motion was; flexion to 80 degrees and abduction to 30 degrees. There was statistical significancy in activity of daily living of hip function at 12 weeks traction in postoperative score. 5. 6 cases out of 11 cases without pain in postoperative condition showed severe bony contacts and ir- regularities of proximal portion of the operated femur radiologically.

3.A Case of NCAM-positive Nasal Type T/NK-Cell Lymphoma.

We herein report a case of nasal type T/natural killer(NK)-cell lymphoma(TNKCL). This lymphoma is characterized by the expression of CD2, CD43 and NCAM(CD56) antigen, an aggressive clinical course, frequent extranodal spreading, a strong association with Epstein-Barr virus(EBV), and the absence of T-cell receptor(TCR) gene rearrangement. NCAM antigen is known to be a possible determinant of extranodal dissemination of peripheral T-cell lymphoma(PTCL). The patient is a 70-year-old male with skin lesion on his forearm. Histopathological and immunohistochemical studies were diagnostic of EBV-associated TNKCL. Untill now, he has failed to respond to anticancer therapy.

4.A Case of Non-T,Non-B Primary Cutaneous Lymphoblastic Lymphoma.

We herein present a case of a 2-year-old girl with non-Hodgkin's lymphoma(NHL) of the lymphoblastic type involving cutaneous sites at the time of diagnosis. The histological finding was typical of lymphoblastic lymphoma. However, immunophenotypically, this lymphoma was not of the T-cell or B-cell type, although the vast majority of lymphoblastic lymphomas involving the skin are usually of the pre-B cell or T-ce11 type. Until now, there have been few reports of non-T, non-B primary cutaneous lymphoblastic lymphoma expressing surface CD10 and CD56 antigens as in this case.

5.A Case of Secondary Cutaneous Diffuse Large B-cell Lymphoma.

We herein report a case of secondary cutaneous diffuse large B-cell lymphoma(DLBCL) occurring in a 66-year-old woman. The skin lesions were erythematous infiltrative nodules on the right inguinal area. Histologically, the skin lesion disclosed DLBCL mainly composed of immunoblasts. Concurrently, she showed lymph node involvement. Initially, however, we could not define the conclusive temporal sequences between nodal lesions and skin lesions. Finally, additional further studies revealed this case as secondary cutaneous 8-cell lymphoma, and she was managed with systemic chemotherapy.

Peripheral T-cel1 lymphoma(PTCL) encompasses histopathologically and clinically various spectra of cutaneous T-cell lymphoma(CTCL). In this report, we describe two cases of PTCL showing diRerent clinical courses according to EBV(Epstein-Barr virus) positivity. The chnical course of case 1 with EBV-associated PTCL was rapidby fatal and refractory to intensive chcmotherapy. However, in case 2, EBV genomes were not found in her lesional tissues and she showed an indolent clinical course withoaat systemic symptoms. Accordingly, serological and immunohistochemical investigations for EBV might be mandatory in cutaneous PTCL to evaluate clinical prognosis.

8.Use of Alternative Medicine by Patients with Psoriasis.

BACKGROUND: Psoriasis is a disabling and intractable disease influencing the psychosocial life of the patients. Patients who are frustrated with orthodox medicine may explore alternative therapies. OBJECTIVE: This study was done to research on the actual condition of the use of alternative medicine in the patients with psoriasis. METHODS: A self-administered questionnaire was constructed, and was given to the patients to answer our questions. RESULTS: Among 128 patients with psoriasis, 61 (47.7%) reported previous use of one or more forms of alternative medicine. The absence of satisfactory long-term effects of physician-provided therapy was the main reason for patients trying alternative medicine. Persons without skin s disease and the mass media were the main sources of information on alternative medicine. CONCLUSION: Alternative therapies were widely utilized by subjects participating in this study, and dermatologists need to be aware of alternative treatments employed by their patients.

9.Diagnostic Value of the Measurement of the Pre-vertebral Soft Tissue in Patients with Cervical Spine Injury.

STUDY DESIGN: We analysed the pre-vertebral soft tissue measurements in cervical spine trauma patients. OBJECTIVES: We tried to determine the sensitivity and specificity of soft tissue measurements in patients with radiographically proven cervical spine fracture and to investigate the diagnostic value of the measurements. SUMMARY OF LITERATURE REVIEW: Widening of the pre-vertebral soft tissue is a well recognized sign in injury of the cervical spine. But the diagnostic value of this widening is still controversial. MATERIALS AND METHODS: Width of the prevertebral soft tissue on lateral cervical spine radiographs was measured for 52 patients with a history of cervical spine trauma and for 53 patients complaining of cervical discomfort without a history of trauma. As a normal Korean adult standard, 7mm at C2 level, 8mm at C3 level, and 16mm at C6 level were considered the upper limit. Width of the prevertebral soft tissue was measured as a distance between antero-inferior margin of the body of cervical spine and the most posterior border of the larynx and trachea. RESULTS: A C2 prevertebral soft tissue measurement of more than 7mm had a sensitivity of 35% and a specificity of 86%. A C3 measurement of more than 8mm had a sensitivity of 35% and a specificity of 72%. A C6 measurement of more than 16mm had a sensitivity of 30% and a specificity of 80%. CONCLUSIONS: The prevertebral soft tissue measurement seems to be of limited diagnostic value. Further study may be required to determine more acceptable limits of normal cervical spine measurement in Korean population.

10.Cryotherpy of warts with liquid nitrogen.

BACKGROUND: Despite the widespread use of liquid nitrogen cryoth.rapy on warts, little is known about its therapeutic effects. OBJECTIVE: This study was conducted to evaluate the therapeutic effect of liqiud nitrogen cryotherapy on warts. MEHTODS: One-hundred and fifteen patients with 142 cases of vira warts were studied. The cryosurgery set we used was CS-76 table-top liquid nitrogen spray unit and it was applied directly on the lesion, the freezing front extending 2 to 4mm onto the uninviblvid skin. RESULT: There was an overall cure rate of 69.7% and the averager umber of freezings required for a cure was 2.6 amongst all patients cured by 5 or less freezings at ntervals of 2 or 3 weeks. CONCLUSION: The liquid nitrogen cryotherapy on warts results in a relatively high cure rate with safety, simplicity, rapid healirig, continustion of normal function and a tivities, and restoration of normal appearance.
