4.A Case of Angiosarcoma of the Scalp.

Angiosarcoma is a rare malignant vascular tumor of endothelial cell origin. Most lesions of cutaneous angiosarcoma occur over the face and scalp. We experienced a case of angiosarcoma of the scalp in a 63-year-old man. Multiple pea-sized erythematous papules and nodules developed on the forehead 6 months ago. Thereafter, the lesions have extended to make violaceous, compressible and hemorrhagic plaques. On histologic examination, there were irregular, often anastomosing vascular channels lined by the atypical endothelial cells in the dermis.

6.Primary Cardiac Angiosarcoma: A case report and rewiew of literture.

We report a case of primary cardiac angiosarcoma in a 35 year-old woman. She presented with dyspnea, facial edema and neck vein distension during 1 month. The oval round large mass(8x4 cm) filling the right atrium infiltrated into the entire thickness of the lateral wall and extended to the vena cava and ventricle. The interatrial septum and pericardium seemed to be intact. Despite the surgical excision and adjuvant chemo-and radiotherapy, the patient died 8 months later due to tumor recurrence. The histologic findings of tumor varied from benign looking capillary proliferation mimicking granulation tissue to sarcoma composed of spindle cells. But anastomosing vascular channels lined by anaplastic polygonal cells and intracytoplasmic lumen containing red blood cells were characteristic findings. These tumor cells showed positive reaction to endothelial cell antigen and factor VIII-related antigen. The intercellular junctional complex and vascular channels formed by cytoplasmic process of tumor cells were identified by the ultrastructural study.

8.Two Cases of Hyperkeratosis of the Nipple and Areola.

We report two cases of hyperkeratosis of the nipple and areola which occurred in 22-year-old woman and 13-year-old girl. The former was suffered from mild pruritic, verrucous thickening and brownish discoloration of both nipples and areolae for 5 years. The latter had asymptomatic, verrucous thickening and dark brownish hyper- pigmentation of the right nipple and areola for 3 years. Both showed mild to marked hyperkeratosis and papillomatosis with increased pigmentation in the basal cell layer histopathologically. In both cases, the lesions were not responsive to the topical medications including steroids and keratolytic agents. Therefore we recommended surgical excision because the patients are greatly disturbed by the appearance of the lesions.

9.The Result of Radiation Therapy of superior Vena Cava Syndrome.

To access the result of radiation therapy for 8 years experiences, 21 patients who were treated with superior vena cava syndrome had been analysed according to dose fractionation and toal dose. The results are as follows; 1. In high fractionate dose group, six of eleven patients (54.5%) exhibited relief of symptoms in 1-2 days, and additional three patients of nine (81.7%) within 3-4 days, while standard fractionated dose treatment is not effective to achieve initial relief of symptoms. 2. Graded response by total dose was correlated with total dose rather than dose fractionation. 3. Overall one year survival rate with superior vana cava syndrome was 9.1% and mean survival was 4.2 months.

10.Experiences of U.K. Notionol Externol Quality Assessment Scheme for Blood Group Serology.

The blood bank of the Seoul National University Hospital has joined the U.K. National External Quality Assessment Scheme(NEQAS) for Blood Group Serology since 1992. We reported correct answers on all occasions for thirty-six samples tested for ABO, RhD typing, and direct antiglobulin test. Among 36 antibody screening and antibody,,identification tests, we missed one anti-S. Among 96 crossmatchings, we made 4 major errors and one minor error. It was interesting that all of the major errors we made were from the sample delivered in mid-summer. Since the items included in the domestic program for the external quality control of blood group serology are very limited in Korea, joining any international quality control program will be necessary for larger blood banks to assure quality of varirous blood bank tests being performed.
