1.Association between A Body Shape Index (ABSI) and sarcopenia among older adults in China: evidence from the CHARLS

Objective:To explore the association between A Body Shape Index (ABSI) and sarcopenia in Chinese older adults.Methods:Totally 1 728 participants in waves 1 and 3 of the China Health and Retirement Longitudinal Survey (CHARLS) were enrolled in this cohort-based cross-sectional analysis. ABSI was calculated by using anthropometric measurements from CHARLS. According to the Asian Working Group for Sarcopenia (AWGS) 2019 consensus update on sarcopenia, low muscle strength and low muscle mass were used as the diagnostic criteria for sarcopenia. Logistic regression and restricted cubic spline (RCS) were used to assess the relationship between ABSI and sarcopenia, and subgroup and interaction analyses were conducted to evaluate the influence of other factors on this association.Results:High ABSI showed a significant association with the risk of sarcopenia among older adults in China, and RCS analysis demonstrated a positive linear correlation between ABSI and sarcopenia. Additionally, the relationship of ABSI with sarcopenia was significantly modified by educational level and depressive status.Conclusion:ABSI, a novel anthropometric measure, serves as an effective threshold for assessing sarcopenia risk in the Chinese older population, providing a basis for developing clinical interventions and public health strategies for the prevention and early identification of sarcopenia.
2.Feasibility of deep learning technique based on CT radiomics in improving the diagnostic accuracy for pulmonary nodules

Objective:To investigate the feasibility of deep learning based on computed tomography(CT)radiomics in improving diagnostic accuracy for pulmonary nodules.Methods:A total of 500 patients with pulmonary nodules who admitted to our hospital from January 2023 to January 2024 were selected as study subjects,and they were randomly divided into a training set(350 patients)and a test set(150 patients)as 7:3 ratio.All patients underwent CT examination,and pathological diagnosis was used as gold standard to record pulmonary nodules that were judged by clinical judgment.The radiomics features were screened from the CT images of the patients,and these features were used to construct multiple machine learning models.The predictive value of different models in diagnosing pulmonary nodules was analyzed through confusion matrices and receiver operating characteristic(ROC)curve.Results:A total of 1,594 radiomics features,including 1,195 texture features(74.97%)that was the largest ratio,334 first-order histograms(20.95%),and 65 second-order histograms(4.08%),were extracted in this study.After least absolute shrinkage and selection operator(LASSO)regression analysis and ten-fold cross-validation processing,a total of six radiomics features were screened out.The screened radiomics features were incorporated respectively into four assembled models with machine learning,including ResNet50,DenseNet121,Inception_V3 and VGG19.The constructed models were evaluated respectively using the training set and the test set.The results showed that the assembled model had the highest accuracies in both training set and the test set(96.57%and 95.33%),which area under curve(AUC)values were 0.934 and 0.923,and specificities were 81.64%and 80.52%,and sensitivities were 90.25%and 88.71%,respectively.The results of consistency test indicated that the assembled model had the best classification consistency(Kappa=0.856,P<0.001)in the constructed diagnostic model for pulmonary nodule,which was the best-performing model.Conclusion:The deep learning technique based on CT radiomics has a certain feasibility in improving the diagnostic accuracy for pulmonary nodules,and the machine learning model that is included in this study has favorable predictive value in diagnosing pulmonary nodules.In them,the assembled model that is constructed on the basis of ResNet50,DenseNet121,Inception_V3,and VGG19 has better classification ability.
3.Relationship of serum neuron-specific enolase and neuropeptide Y with sleep disorders in elderly stroke patients

Objective To explore the relationship of changes in serum neuron-specific enolase(NSE),neuropeptide Y(NPY)and hypocretin(Hcrt)levels with sleep disorders in the elderly stroke patients.Methods A total of 105 elderly stroke patients admitted in our department from February 2021 to February 2024 were retrospectively enrolled in this study.According to the re-sults of Pittsburgh sleep quality index(PSQI)evaluation,they were divided into sleep disorder group(64 cases)and non-sleep disorder group(41 cases).The serum NSE,NPY and Hcrt levels at admission were measured,their sleep status was evaluated with PSQI questionnaire,and the levels and clinical data were compared between groups.Multivariate logistic regression analysis was applied to analyze the influencing factors of sleep disorders in elderly stroke patients,and ROC curve analysis was performed.Results The NSE level was significantly higher,and the le-vels of NPY and Hcrt were obviously lower in the sleep disorder group than the non-sleep disor-der group(P<0.01).Multivariate logistic regression analysis suggested that NSE,NPY,Hcrt,age,NIHSS score and history of stroke were independent influencing factor for sleep disorders in elderly stroke patients(P<0.05,P<0.01).ROC curve analysis indicated that the AUC value of NSE,NPY and Hcrt in predicting the occurrence of sleep disorders was 0.797,0.754 and 0.915,re-spectively.Conclusion Serum NSE,NPY and Hcrt are closely associated with sleep disorders in elderly stroke patients,and are of important value in predicting the occurrence of sleep disorders.
4.Evaluation of hemolysis quality of specimens by fully automated blood collection robot

Objective:To compare the differences in specimen results between the intelligent robotic phlebotomy group and the manual venipuncture group, and to evaluate the clinical applicability of the autonomous blood collection system.Methods:From January 20 to October 28, 2022, 154 volunteers at Zhongshan Hospital, Fudan University underwent paired blood collections (robotic and manual) within 5 minutes. The collected samples were analyzed for: hemolysis index (HI), alanine transaminase (ALT), aspartate transaminase (AST), L-γ-glutamyltransferase (γ-GT), lactate dehydrogenase (LDH), urea nitrogen (UREA), creatinine (CRE), uric acid (UA), glucose (GLU), total cholesterol (TC), triglyceride (TG), natrium (Na), kalium (K), chlorine (Cl), creatine kinase (CK), CK-MB, CK-MM, and neuron-specific enolase (NSE). Statistical analyses used t-tests and Wilcoxon signed-rank tests.Results:The results of two different blood collection methods revealed that the HI values of 154 specimens in the intelligent robot blood collection group were all less than 20SI, while 7 specimens (4.54%) in the manual blood collection group had HI values exceeding 20SI; In the comparison of 17 biochemical and immunological markers, there were statistically significant differences between groups in 8 items including γ-GT[20.00(15.00, 37.75)U/L vs. 19.00 (14.00, 36.25)U/L, Z=2.497, P<0.05], LDH[165.5 (147.0, 183.0)U/L vs. 173.0 (155.0, 193.0)U/L, Z=8.629, P<0.05], TC[(5.002±0.856)mmol/L vs.(5.031±0.870) mmol/L, t=-3.006, P<0.05], K[4.1 (4.0, 4.3)mmol/L vs. 4.3 (4.1, 4.4)mmol/L, Z=5.592, P<0.05], CK[97.00 (73.00, 133.00)U/L vs. 99.00 (74.75, 136.25)U/L, Z=3.490, P<0.05], CK-MB[13 (11, 15)U/L vs. 14 (12, 16)U/L, Z=6.581, P<0.05], CK-MM[84.00 (60.00, 119.00)U/L vs. 83.50 (58.75, 118.00)U/L, Z=3.790, P<0.05], and NSE[10.600 (9.500, 11.700)ng/ml vs. 11.950 (10.475, 13.725)ng/ml, Z=8.151, P<0.05]. Conclusions:In the collection of serum samples, intelligent blood collection robots can achieve standardization and normalization of specimen collection volume and mixing in the pre-analysis stage. The hemolysis related indicators of the collected specimens are lower than those of the manual collection group, and can be used for the collection of clinical serological specimens.
5.Banxia Xiexin Decoction suppresses malignant phenotypes of colon cancer cells via PARG/PARP1/NF-κB signaling pathway.

This study aims to delve into the influences and underlying mechanisms of Banxia Xiexin Decoction(BXD) on the proliferation, apoptosis, invasion, and migration of colon cancer cells. Firstly, the components of BXD in blood were identified by UPLC-MS/MS, and subsequently the content of these components were determined by HPLC. Then, different concentrations of BXD were used to treat both the normal intestinal epithelial cells(NCM460) and the colon cancer cells(HT29 and HCT116). The cell viability and apoptosis were examined by the cell counting kit-8(CCK-8) and flow cytometry, respectively. Western blot was employed to determine the expression of the apoptosis regulators B-cell lymphoma-2(Bcl-2) and Bcl-2-associated X(Bax). The cell wound healing assay and Transwell assay were employed to measure the cell migration and invasion, respectively. Additionally, Western blot was employed to determine the expression levels of epithelial-mesenchymal transition(EMT)-associated proteins, including epithelial cadherin(E-cadherin), neural cadherin(N-cadherin), and vimentin. The protein and mRNA levels of the factors in the poly(ADP-ribose) glycohydrolase(PARG)/poly(ADP-ribose) polymerase 1(PARP1)/nuclear factor kappa-B p65(NF-κB p65) signaling pathway were determined by Western blot and RT-qPCR, respectively. The results demonstrated that following BXD intervention, the proliferation of HT29 and HCT116 cells was significantly reduced. Furthermore, BXD promoted the apoptosis, enhanced the expression of Bcl-2, and suppressed the expression of Bax in colon cancer cells. At the same time, BXD suppressed the cell migration and invasion and augmented the expression of E-cadherin while diminishing the expression of N-cadherin and vimentin. In addition, BXD down-regulated the protein and mRNA levels of PARG, PARP1, and NF-κB p65. In conclusion, BXD may inhibit the malignant phenotypes of colon cancer cells by mediating the PARG/PARP1/NF-κB signaling pathway.

6.Medication rules and mechanisms of treating chronic renal failure by Jinling medical school based on data mining, network pharmacology, and experimental validation.

This study aims to explore the medication rules and mechanisms of treating chronic renal failure(CRF) by Jinling medical school based on data mining, network pharmacology, and experimental validation systematically and deeply. Firstly, the study selected the papers published by the inherited clinicians in Jinling medical school in Chinese journals using the subject headings named "traditional Chinese medicine(TCM) + chronic renal failure", "TCM + chronic renal inefficiency", or "TCM + consumptive disease" in China National Knowledge Infrastructure, Wanfang, and VIP Chinese Science and Technology Periodical Database and screened TCM formulas for treating CRF according to inclusion and exclusion criteria. The study analyzed the frequency of use of single TCM and the four properties, five tastes, channel tropism, and efficacy of TCM used with high frequency and performed association rule and clustering analysis, respectively. As a result, a total of 215 TCM formulas and 235 different single TCM were screened, respectively. The TCM used with high frequency included Astragali Radix, Rhei Radix et Rhizoma, Salviae Miltiorrhizae Radix et Rhizoma, Poria, and Atractylodis Macrocephalae Rhizoma(top 5). The single TCM characterized by "cold properties, sweet flavor, and restoring spleen channel" and the TCM with the efficacy of tonifying deficiency had the highest frequency of use, respectively. Then, the TCM with the rules of "blood-activating and stasis-removing" and "diuretic and dampness-penetrating" appeared. In addition, the core combination of TCM [(Hexin Formula, HXF)] included "Astragali Radix, Rhei Radix et Rhizoma, Poria, Salviae Miltiorrhizae Radix, and Angelicae Sinensis Radix". The network pharmacology analysis showed that HXF had 91 active compounds and 250 corresponding protein targets including prostaglandin-endoperoxide synthase 2(PTGS2), PTGS1, sodium voltage-gated channel alpha subunit 5(SCN5A), cholinergic receptor muscarinic 1(CHRM1), and heat shock protein 90 alpha family class A member 1(HSP90AA1)(top 5). Gene Ontology(GO) function analysis revealed that the core targets of HXF predominantly affected biological processes, cellular components, and molecular functions such as positive regulation of transcription by ribonucleic acid polymerase Ⅱ and DNA template transcription, formation of cytosol, nucleus, and plasma membrane, and identical protein binding and enzyme binding. Kyoto Encyclopedia of Genes and Genomes(KEGG) analysis revealed that CRF-related genes were involved in a variety of signaling pathways and cellular metabolic pathways, primarily involving "phosphatidylinositol 3-kinase(PI3K)-protein kinase B(Akt) pathway" and "advanced glycation end products-receptor for advanced glycation end products". Molecular docking results showed that the active components in HXF such as isomucronulatol 7-O-glucoside, betulinic acid, sitosterol, and przewaquinone B might be crucial in the treatment of CRF. Finally, a modified rat model with renal failure induced by adenine was used, and the in vivo experimental confirmation was performed based on the above-mentioned predictions. The results verify that HXF can regulate mitochondrial autophagy in the kidneys and the PI3K-Akt-mammalian target of rapamycin(mTOR) signaling pathway activation at upstream, so as to alleviate renal tubulointerstitial fibrosis and then delay the progression of CRF.

7.Preliminary efficacy observation of 3D printed functional spinal external fixation brace combined with McKenzie therapy in the treatment of lumbar disc herniation.

OBJECTIVE:
To observe the clinical efficacy of 3D printing spinal external fixator combined with McKenzie therapy for patients with lumbar dics herniation (LDH).
METHODS:
Sixty patients with LDH between January 2022 and January 2023 were enrolled. Among them, 30 patients were given McKinsey training. According to different treatment methods, all patients were divided into McKenzie group and McKenzie + 3D printing group, 30 patients in each group. The McKenzie group provided McKenzie therapy. The McKenzie + 3D printing group were treated with 3D printing spinal external fixation brace on the basis of McKenzie therapy. Patients in both groups were between 25 and 60 years of age and had their first illness. In the McKenzie group, there were 19 males and 11 females, with an average age of (48.57±5.86) years old, and the disease duration was (7.03 ±2.39) months. The McKenzie + 3D printing group, there were 21 males and 9 females, with an average age of (48.80±5.92) years old, and the disease duration was(7.30±2.56) months. Pain was evaluated using the visual analogue scale (VAS), and lumbar spine function was assessed using the Oswestry disability index (ODI) and the Japanese Orthopaedic Association (JOA) score. VAS, ODI and JOA scores were compared between two groups before treatment and at 1, 3, 6, 9 and 12 months after treatment.
RESULTS:
All patients were followed up for 12 months. The VAS for the McKenzie combined with 3D printing group before treatment and at 1, 3, 6, 9, and 12 months post-treatment were(6.533±0.860), (5.133±1.008), (3.933±0.868), (2.900±0.759), (2.067±0.640), (1.433±0.504), respectively. In the McKenzie group, the corresponding scores were (6.467±0.860), (5.067±1.048), (4.600±0.968), (3.533±1.008), (2.567±0.728), (1.967±0.809), respectively. The ODI of the McKenzie group before treatment and at 1, 3, 6, 9, and 12 months post-treatment were (41.033±6.810)%, (37.933±6.209)%, (35.467±6.962)%, (27.567±10.081)%, (20.800±7.531)%, (13.533±5.158)%, respectively. For the McKenzie combined with 3D printing group, the corresponding ODI were(38.033±5.605)%, (33.000±6.192)%, (28.767±7.045)%, (22.200±5.517)%, (17.700±4.836)%, (11.900±2.771)%, respectively. The JOA scores of the McKenzie combined with 3D printing group before treatment and at 1, 3, 6, 9, and 12 months post-treatment were(8.900±2.074), (13.133±2.330), (15.700±3.583), (20.400±3.480), (22.267±3.084), (24.833±2.640), respectively. In the McKenzie group, the corresponding scores were(9.200±2.091), (12.267±2.406), (15.333±3.198), (18.467±2.240), (20.133±2.751), (22.467±2.849), respectively. Before the initiation of treatment, no statistically significant differences were observed in the VAS, ODI, and JOA scores between two groups (P>0.05). At 3, 6, 9, and 12 months post-treatment, the VAS in the McKenzie combined with 3D printing group was significantly lower than that in the McKenzie group, and the difference was statistically significant (P<0.05). The comparison of ODI between two groups at 1, 3, 6, 9, and 12 months post-treatment revealed statistically significant differences (P<0.05). At 6, 9, and 12 months post-treatment, the JOA score in the McKenzie combined with 3D printing group was significantly higher than that in the McKenzie-only group, and the difference was statistically significant (P<0.05).
CONCLUSION
The combination of 3D printed functional spinal external fixation brace with McKenzie therapy can significantly improve and maintain lumbar function in patients with LDH.

8.Immunophenotypic Characteristics of Bone Marrow Granulocytes and Their Clinical Significance in Patients with Multiple Myeloma.

OBJECTIVE:
To explore the immunophenotypic characteristics of bone marrow granulocytes (G) and their clinical significance in patients with multiple myeloma (MM).
METHODS:
The granulocyte immunophenotypes of bone marrow in 70 MM patients (MM group) and 40 anemia patients (control group) were detected by flow cytometry, and its correlation with clinical characteristics was further analyzed. Univariate and multivariate regression analysis were used to screen factors that affected prognosis.
RESULTS:
The CD56+G%, CD13+G%, CD22+G% and CD117+G% in MM group were higher than those in the control group (all P <0.05). CD56+G% and CD117+G% in CR+VGPR group were significantly lower than those in PR+MR+PD group (both P <0.05). The CD10+G% in RISS Ⅲ stage and Ca2+ ≥2.65 mmol/L groups were increased (both P <0.05). The CD56+G% in elevated lactate dehydrogenase, β2-microglobulin≥5.5 mg/L and hemoglobin <85 g/L groups were increased (all P <0.05), while the CD117+G% in high-risk cytogenetic positive group was decreased (P <0.05). The expression rate of CD molecules on granulocytes was divided into low (L) and high (H) groups according to the median value. The overall survival (OS) of the LCD56+G%, LCD13+G% and LCD22+G% groups was significantly prolonged (all P <0.05). CD13+G% and CD22+G% were independent risk factors for OS in MM patients (HR=0.443, 0.410, both P <0.05).
CONCLUSION
The CD56+G%, CD10+G% and CD117+G% are closely correlated with clinical features in MM patients, while CD13+G% and CD22+G% are closely correlated with prognosis. Detection of CD molecules expression on granulocytes may be used to evaluate prognosis and guide treatment.

9.Mechanisms and Efficacy of Chinese Herbal Medicines in Benign Prostatic Hyperplasia.

Benign prostatic hyperplasia (BPH) is one of the most common diseases in elderly men, the incidence of which gradually increases with age and leads to lower urinary tract symptoms (LUTS), which seriously affects the quality of life of patients. Chinese herbal medicines (CHMs) are widely used for the treatment of BPH in China and some other countries. To explore the molecular mechanisms of CHMs for BPH, we conducted a review based on peer-reviewed English-language publications in PubMed and Web of Science databases from inception to December 31, 2023. This article primarily reviewed 32 papers on the use of CHMs and its active compounds in the treatment of BPH, covering animal and cell experiments, and identified relevant mechanisms of action. The results suggest that the mechanisms of action of CHMs in treating BPH may involve the regulation of sex hormones, downregulation of cell growth factors, anti-inflammatory and antioxidative effects, inhibition of cell proliferation, and promotion of apoptosis. CHMs also exhibit α-blocker-like effects, with the potential to relax urethral smooth muscle and alleviate LUTS. Additionally, we also reviewed 4 clinical trials and meta-analyses of CHMs for the treatment of BPH patients, which provided initial evidence of the safety and effectiveness of CHMs treatment. CHMs treatment for BPH shows advantages as a multi-component, multi-target, and multi-pathway therapy, which can mitigate the severity of the disease, improve LUTS, and may become a reliable treatment option in the future.

The key link to colorectal cancer progression is that epigenetic derangements promote reprogramming of glucose metabolism.Based on many studies on the relationship between spleen being the origin of acquired constitution and epigenetics,spleen qi dispersing essence and glucose metabolism,combined with the extensive experience of traditional Chinese medicine in the treatment of colorectal cancer and the accumulation of researches on glucose metabolism,it is believed that the"epigenetic-reprogramming of glucose metabolism"pathology of colorectal cancer is the microscopic essence of the key pathogenesis of"dysfunction of spleen transforming essences".The"promoting spleen and dispersing essences"method means that"promoting spleen"to reverse epigenetic derangements and"dispersing essences"to regulate reprogramming of glucose metabolism was put forward to treat colorectal cancer.Look forward to enrich the scientific connotation of the pathogenesis of"dysfunction of spleen transforming essences",new ideas and potential empirical basis for traditional Chinese medicine intervention in colorectal cancer were provided.
