1.A Radiological Observation on the Normal Hip in Legg-Calve-Perthes disease

A study on the deformity of the hip in Legg-Calve-Perthes disease has been confined to the involved hip. Recently the morphological change of uninvolved hip has been investigated with histological and radiological means, by several authors. Their results, however, were inconclusive and there were much differences as to the pattern and incidence of abnormality. To elucidate and gain further insight into this differences, the authors have performed radiological observation on the opposite hip in 126 unilateral Legg-Calve-Perthes disease and have compared with that of normal 94 children. The results were as follows: 1. Surface irregularity; Surface irregularity was observed in both the disease group and the control group. The appearance rate in disease group (12.7%) was about two times that of control group (6.4%). 2. Thickness of articular cartilage: A change of thickness of articular cartilage was not observed. 3. Epiphyseal index; A statistically significant decrease in the disease group was observed with average epiphyseal index of 0.44±0.065 in the disease group and 0.50±0.095 in the control group respectively. 4. In conclusion, the authors could confirm the presence of more abnormality in the opposite hip of unilateral Legg-Calve-Perthes disease than previous recognition and became to have question on the reliability of the opposite hip as a normal reference in measuring the various quotients.

5.Radiological Findings of Vascular Involvement in Behcet's Disease.

Angiographic findings were analysed in 11 cases of Behcet'sdisease which was diagnosed clinically in Seoul national University Hospital from July 1982 to June 1983. Arterial involvement was noted in 5 cases and venous involvement in 6 cases respectively. Arterial aneurysm and occlusion were arteriographic findings. Arterial aneurysm was noted at large arteries in all 5 cases and multifocal involvement was noted in 4 cases. Characteristic finding of arterial involvement was protruding saccular aneurysm. Venous involvement was thrombotic occlusion of deep veins in iliac, femoral and tibial areas in all 6 cases. Behcet's disease should be differentiated whenever these arterial or venous angiographic findings are revealed in a young adult.

Bacterial endocarditis has been well recognized as an important complication of congenital heart disease, such as ventricular septal defect, patent ductus arteriosus or pulmonary stenosis. The incidence of right sided bacterial endocarditis is lesser than left sided bacterial endocarditis. Also, pulmonic valve vegetation has been thought to be relatively uncommon. So in a patient with fever and evidence of recurrent pulmonary infarction, changing heart murmurs and scattered pneumonic infiltrates, one should direct attention to the heart as a possible source of the infection. Echocardiography with M-mode, 2-D and Doppler mode represents the only noninvasive technic available for detecting vegetations in bacterial endocarditis. In fact, the technic is more sensitive in identifying these lesions than angiography. We experienced a case of ventricular septal defect with bacterial endocarditis, pulmonic valve vegetation and multiple pulmonary embolism diagnosed with Echocardiogram and lung scan, and confirmed by operation. Patch repair of ventricular septal defect, resection of pulmonic valve and vegetation and artificial valve formation with pericardium were done.

7.A Case of Cutaneous Mycobacterium marinum Infection.

Mycobacterium marinum is a photochromogenic and slowly growing acid-fast organism classified in Runyon's group I of atypical mycobacteria. Infection with M. marinum occurs when the organism is inoculated usually in association with exposure to contaminated water and the disease is characteristically limited to the skin but deep soft tissue may be involved. We report a case of M. marinum infection in a 43-year-old man with erythematous verrucous plaque on his right knee, which was caused by a shell wound on a beach 3 months ago. The skin biopsy specimen showed pseudoepitheliomatous hyperplasia and several small epithelioid granulomas in the dermis. Culture of the tissue specimen revealed slowly growing whitish colonies in the dark, which changed yellowish after exposure to light. The microorganism was identified as M. marinum by biochemical tests, and was confirmed by the DNA-DNA hybridization method. The skin lesion disappeared after 3 moaths of treatment with minocycline, 200mg daily.

8.Three-dimensional finite element analysis of stress distribution and preload of different connection types implant with initial clamping.

STATEMENT OF PROBLEM: One of common problems associated with single teeth dental implant prosthetic is the loosening of screws that retain the implants. PURPOSE: The maintenance of screw joint stability is considered a function of the preload achieved in the screw when the suggested initial tightening torque is applied. The purpose of this study was to investigate acquired preload after initial clamping torque for estimating screw joint stability. MATERIAL AND METHODS: A comparative study on the effect of initial clamping of two types of implant systems with different connections was conducted. Three dimensional non-linear finite element analysis is adopted to compare the characteristics of screw preloads and stress distributions between two different types of implant systems composed with abutment, screw, and fixture under the same loading and boundary conditions. RESULTS: 1. When the initial clamping torque of 32Ncm was applied to the implant systems, all types of implants generated the maximum effective stress at the first helix region of screw. 2. Morse taper connection types of implants generate lower stress distributions compared to those by butt joint connection types of implants due to large contact surface between abutment and fixture. 3. The internal types of implant systems with friction grip type implant systems have higher resistance to screw loosening than that of the external types of implant systems since the internal types of implant systems generated larger preload than that generated by the external types for the same tightening moments.

9.Immunohistochemical Expression of p53 Protein and P-glycoprotein and its Prognostic Significance in Osteosarcoma.

PURPOSE: The principal cause of treatment failure in osteosarcoma is drug resistance. Multidrug resistance (MDR), mediated by P-glycoprotein (P-gp), is an in vitro phenomenon, resulting in becoming cross-resistant to structurally unrelated drugs. P-gp expression was related to drug resistance, prognosis and recurrence in osteosarcoma. p53 gene regulates genomic stability. Mutant p53 protein causes loss of tumor suppression. Co-expression of mutant p53 protein and P-gp was related to short survival, malignancy and drug resistance in malignant tumors. Wild type p53 protein in SaOS-2 cell line down-regulates the transcription of mdr1 gene. The purpose of this study was to evaluate the expression of mutant p53 protein and P-gp in osteosarcoma and its prognostic significance. MATERIALS AND METHODS: Fifty-two archival pathologic tissues were examined with immunohisto-chemistry. Positive expression was defined if more than 10% of tumor cells were stained. RESULTS: Expression rate of mutant p53 protein was 63% and rate of P-gp was 35%. Correlation between mutant p53 protein and P-gp expression was significant (P=0.0008). Co-expression of mutant p53 protein and P-gp was the strongest prognostic factor for shorter survival by multivariate analysis (P=0.0004). CONCLUSIONS: Nuclear accumulation of mutant p53 protein was significantly related to P-gp expression in osteosarcoma, Co-expression of mutant p53 protein and P-gp is an independent unfavorable prognostic factor in osteosarcoma

10.Pure Varus Injury to the Knee Joint.

A 30-year-old male was involved in a car accident. Radiographs revealed a depressed marginal fracture of the medial tibial plateau and an avulsion fracture of the fibular head. Magnetic resonance imaging showed avulsion fracture of Gerdy's tubercle, injury to the posterior cruciate ligament (PCL), posterior horn of the medial meniscus, and the attachments of the lateral collateral ligament and the biceps femoris tendon. The depressed fracture of the medial tibial plateau was elevated and stabilized using a cannulated screw and washer. The injured lateral and posterolateral corner (PLC) structures were repaired and augmented by PLC reconstruction. However, the avulsion fracture of Gerdy's tubercle was not fixed because it was minimally displaced and the torn PCL was also not repaired or reconstructed. We present a unique case of pure varus injury to the knee joint. This case contributes to our understanding of the mechanism of knee injury and provides insight regarding appropriate treatment plans for this type of injury.
