We reviewed the whole-process management strategy according to difference of mutation state of epidermal growth factor receptor (EGFR) gene.
2.Advances in research on the diagnosis and therapy of lung cancer with bone metastases

Bone metastases in advanced lung cancer patients are common and affect the life quality seriously. So early diagnosis and appropriate treatments are important. The methods of diagnosis include X-ray, CT, MRI, ECT and PET, which are different in sensitivity and specificity. The gene microarray technique is another advance for detecting micrometastasis. The gene analysis also can help understanding the biological characteristics of the primary tumor. The treatment of bone metastases in lung cancer is various, including systemic and local therapeutic means. Surgery is often used to prevent or treat the compressions and bone fractures. Radiotherapy is one of the most effective methods of pain relief. Diphosphonate provides a new convenient and effective way of alleviative treatment. In addition, supportive and symptomatic treatments should be keep in mind.

5.Effects of perioperative hypothermia on patients’ vital signs during TURP

Objective To evaluate the effects of unwarmed and warmed irrigation fluid on patients’ vital signs during transurethral resection of the prostate (TURP),and to investigate the related factors affecting the safety of TURP. Methods A total of 100 patients were randomly assigned to receive unwarmed (21℃,n=40)or warmed (37 ℃,n=60)irrigation fluid during TURP.Pre-operatively,their age,body weight,IPSS,ratios of heart, brain, lung complications,intra-operative anesthesia modes,irrigation time, irrigation fluid amount,resected prostate weight and blood transfusion amount were not significantly different between the 2 groups. Intra-operatively,their mean arterial pressure (MAP),heart rate(HR),body temperature(T),blood oxygen saturation (SaO 2) and osmotic pressure of plasma (Osm) were observed dynamically based on the operating time. This review analyzed the effect of folic acid on pemetrexed efficacy and toxicity, especially high-does folic acid supplementation. The optimal duration for supplementation prior to the first dose of pemetrexed has not been defined. The review also explored whether total plasma homocysteine levels can be as a marker to predict and avoid toxicity from pemetrexed therapy.

Objective: To study the role of inducible nitric oxide synthase (iNOS) in activated macrophages and nitric oxide (NO) in the defence against tumors. Methods: Macrophages were obtained by alveolar lavages of mice and activated through incubation with recombinant murine INF? in vitro. P815 cells were added to the culture. Culture supernates were collected to measure the activity of iNOS and NO. Tumoricidal activity of macrophages was determined in presence and absence of the specific inhibitor of NO synthase: L NMMA. Results: NO production and activity of iNOS induced by activated macrophages were positively related with concentration of INF? in macrophage P815 coculture. 8. Diagnostic value of 320 rows spiral CT for multiple rib fractures

Objecive To evaluate the clinic value of 320 rows spiral CT in diagnosis of rib fractures. Methods Eighty-six patients with rib fractures were examined by 320 rows spiral CT. After the scanning, rib three-dimensional reconstructions were carried out. The number of ribs fractures was compared with that demonstrated by X-ray and common spiral CT. Results All patients were examined successfully in 3~5 seconds. The images quality of ribs reconstructed by the new method was excellent and exquisite. Many linear fractures and depressed fractures and costobrachial fractures which were not displayed in the images reconstructed by common spiral CT were displayed. The difference was significant statistically. Conclusion Compared with common spiral CT, 320 rows spiral is superior, with faster scanning speed, lower radiological dose and better image quality, improving the diagnostic accuracy of rib fractures to some extent. Therefore, it is an advanced and reliable diagnostic tool for rib fractures.

10.Endobronchial ultrasonography guided fine needle aspiration in diagnosis of lung cancer

Objective:To evaluate the diagnostic value of endobronchial ultrasonography(EUS)-guided fine-needle aspi-ration(FNA)in diagnosis of lung cancer.Methods:EUS-guided FNA was used in16patients suspected of lung cancer of sub-mucosal and parabronchial structures.Bronchoscopic methods failed to detect any abnormality in them.Results:Final diag-noses were made in15patients,including8cases of bronchopulmonary adenocarcinoma,4cases of bronchopulmonary squa-mous carcinoma and3cases of lymphatic metastasis of bronchral adenocarcinoma.The sensitivity of EUS-guided FNA was 93.8%. There was no severe complications associated with the use of EUS.No patients developed bleeding or pneumatotho rax.Conclusion:EUS is useful in detecting pathologic changes in the submucosal and parabronchial structures.EUS guided FNA is accurate and safe for diagnosis of lung cancer,especially in small nodes.
