1.A Case of Meibomian Gland Adenocarcinoma.

The clinical resemblance of carcinoma of the Meibomian gland to the relatively frequent chaiazion should be kept in mind by anyone performing eye lid surgery. A 46-year-oId Korean house wife was first visited here in May 2nd '69 with complaints of localized swelling, ulceration and dull pain on the left upper lid. She had been taken the operation for chaiazion on the same side of the left upper eye lid a year ago. Examination revealed a reddened hard, tender lesion elevating the skin on the medial one third of the left upper lid with ulceration at its center. The tarsal conjunctiva was noted yellowish patches with ulceraion extended to its margin. Local resection with full thickness containing normal tissue around he lesion of upper lid was performed and graft was made in lower lid and rotatory transplantation was given, and pathologic examination revealed adeno-carcinoma of Meibimian gland. There was no evidence of recurrence and metastasis to remote site a year after surgery.

2.Parapatellar Complications after ACL Reconstruction Using Bone-Patellar Tendon-Bone Autograft.

PURPOSE: To evaluate clinical and radiological results and to analyse the parapatellar complications after endoscopic anterior cruciate ligament reconstruction using central one-third bone-patellar tendon-bone autografts. MATERIALS AND METHODS: 66 cases among 158 consecutive cases from Feb 1990 to May 1996 were reviewed and evaluated with regard to patellofemoral pain, crepitus, quadriceps atrophy, graft donor site complication, Lysholm knee scoring system and radiological assessment. The average period of follow up was 23 months (range, 12-57 months) and the average age at operation was 31 years old (range, 18-58 years). RESULTS: The average Lysholmn knee score improved from 57.5 points preoperatively to 91.3 points at follow up. The instrumented anterior laxity test showed that excellent anterior stability was regained in most patients. There were many cases of parapatellar complications, 7 cases (10.6%) of anterior knee pain, 21 cases (32.8%) of crepitus, 43 cases (65.1%) of quadriceps weakness, 29 cases (45.3%) of graft donor site paresthesia and 15 cases (23.4%) of pain on kneeling. CONCLUSIONS: Although endoscopic ACL reconstruction using central one-third bone-patellar tendon-bone autografts had good clinical results, many parapatellar complications were noted. In order to prevent these complications, different reconstruction techniques and graft materials should be considered

3.Arthroscopic Treatment for Acute or Subacute Patellar Dislocation.

PURPOSE: The acute or subacute patellar dislocation is frequently neglected. Arthoscopy has provided a new method of both diagnosis and treatment of the patellofemoral instability. In our series, we performed proximal realignment including lateral retinacular release and medial capsular imbrication under arthroscopic alone on the patients who had normal skeletal alignment with Q angle less than 20 degree. The clinical and radiographic results of the procedure was assessed. MATERIAL AND METHODS: 21 knees in 20 patients were evaluated at an average 36 months following arthroscopic procedures alone. The average patient's age was 25 years old and the mean follow-up period was 18 months. The patients had been treated surgically after conservative management for mean three weeks from initial trauma. Medial patellar osteochondral fractures and loose bodies in 7 knees of 6 patients and osteochondral defect of lateral femoral condyle in 1 knee of 1 patient were confirmed radialologically and intraoperatively. We performed additional treatments for the associated disease. The radiologic results of Insall-Salvati index, congruence angle for subluxation and lateral patellofemoral angle (LPFA) for tilt on preoperative, postoperative and last follow-up and clinical outcome were assessed, according to Crosby and Insall's criteria. RESULTS: Congruence angle(+10.8 to -0.5), LPFA(+7.2 to -7.3) and Insall-Salvati index(0.82 to 1.05) were radiologically normalized. Clinically, results were excellent to good results in 19 knees(90.5%). CONCLUSION: The arthroscopic proximal realignment was done by minimal surgical exposure and was effective on relief of the patellofemoral pain after acute or subacute dislocation, it also proved the less pain, rapid rehabilitation and ability to diagnose the other intraarticular disorders.

5. Quantitative determination of phenyl propanoides from barks of Syringa dilatata grown in DPR of Korea

Objective: To isolate phenyl propanoides from the barks of Syringa dilate, identify their structures, and determine the contents by reliable HPLC. Methods: Phenyl propanoides were extracted with methanol, and isolated by silica gel chromatography. The structures were elucidated by spectroscopic analysis including NMR, MS, UV, and IR. Shimpack CLC C18 (150 mm × 6 mm, 5 μm) column chromatography was employed with gradient elute of 1% acetic acid and methanol, and the column temperature was set at 30℃. The flow rate was 1.0 mL/min and the detection wavelength was set at 267 nm. Results: Two phenyl propanoides were isolated from the barks of S. dilata grown in DPR of Korea and identified as syringin (1) and coniferin (2). The linear range of syringin was 0.2-6.0 mg/mL, the correlation co-efficient was 0.999 8, and the average recoveries were 99.8%, 100.6%, and 100.9%. The results showed that the content in the barks was 8 folds of inner parts. There was no difference in locations and the content of syringin increased in winter, while decreased in summer. Conclusion: The elucidation of phenyl propanoides existence in S. dilata could be a fundamental for the search of new herbal materials. The method is simple, rapid, and reliable. It can be used to determine the content of syringin in herbal medicines.
6.Aortic Root Reconstruction for Aortic Insufficiency Developed after Fontan Operation.

Aortic regurgitation in the pediatrics who had previous cardiac surgery is increased with their improved longterm survival rate and their complexity of heart disease. So the need of aortic valve surgery in pediatrics is also on the increase. A 10-year old boy was admitted for progressive cyanosis and dyspnea on exertion(DOE). The patient had been underwent lateral tunnel Fontan operation before. Echocardiography and cardiac catheterization study revealed hepatic vein drained to pulmonary atrium via intrahepatic collaterals, moderate atrioventricular regurgitation, and severe aortic regurgitation due to aortic root dilation. We report a case who had aortic root reconstruction, valvuloplasty of the atrioventricular valve, and hepatic vein ligation successfully. Cyanosis and DOE was dramatically improved after the operation

7.Cystic Thymic Diseases: CT Manifestations.

PURPOSE: To describe CT findings and differential points of cystic thymic lesions. MATERIAL AND METHOD: We evaluated retrospectively total 19 masses with well marginated cystic lesions at thymic area on CT scans. They were 10 teratomas, 3 congenital thymic cysts, 2 multilocular thymic cysts(associated with thymoma and myasthenia gravis in each), 2 cysts Assciated with thymic Hodgkin's lymphomas an ectopic parathyroid cyst, and an infected thymic cyst. The radiological abnormalities evaluated were thickness of the wall, presence or abscene of septa, mural nodule, solid component, calcification and fat component. RESULTS: All three cases of congenital thymic cysts and an ectopic parathyroid cyst appeared as thin-walled unilocular cyst with homogeneous internal density and without identifiable solid component. In multilocular thymic cyst, there were thick wall and solid components(n=2), thick internal septa and calcifications(n=l). The cysts of teratomas manifested thick walls(n=9), internal septa(n=4), calcifications(n=6), fat components(n=4), and solid components(n=4). Cysts in Hodgkin's diseases appeared as multilocular or unilocular and had thick wall and septa without calcification. infected thymic cyst presented with multilocular cystic mass with identifiable wall and septa, calcification, and solid components. CONCLUSION: The thymic diseases with cystic lesion include teratomas, congenital thymic cysts, multilocular thymic cysts, parathyroid cyst, .and Hodgkin's disease. Congenital thymic cyst and ectopic parathyroid cyst are thin-walled unilocular cystic lesions. Cystic lesions associated with teratoma, Hodgkin's disease, and multilocular thymic cyst are thick-walled cystic lesions with or without solid component.

8.Cerebral White Matter Lesions and Apolipoprotein E polymorphism.

BACKGROUND: Cerebral white matter lesions(WMLs), such as leukoaraiosis, may be related to damage from cerebral ischemia and may also be associated with the degenerative process. The apolipoprotein E (APOE) 4 allele is a risk factor for degenerative diseases, such as Alzheimer`s disease, and ischemic brain damage through acceleration of atherosclerosis. No study has been performed regarding WMLs and APOE genotype in Korea. We investigated the association between WMLs and APOE among Koreans. METHODS: Brain MRI was performed in 225 subjects(ages 61 to 85 years) without neuropsychiatric disease randomly selected from the Ansan Health Cohort Study. WMLs observed on 225 MRI scans were rated in terms of severity by 2 raters. All study subjects underwent APOE genotyping. RESULTS: WMLs were observed in 109(48.4%) of subjects. In the subjects with WMLs, the distribution of APOE genotypes was 0.9% for epsilon 2/epsilon 2, 11.0% for epsilon2/epsilon3, 1.8% for epsilon2/epsilon4, 61.5% for epsilon3/epsilon3, 22.9% for epsilon3/epsilon4, and 1.8% for epsilon4/epsilon4, respectively. The distribution of APOE genotypes did not differ between subjects with and without WMLs. CONCLUSION: These data suggest that there is no association between WMLs and APOE genotypes in Koreans.

9.Normal Retinal Vascular Pattern: Arteriovenous Pairing and A/V ratio.

All physicans regard the fundus as an important guide to prognosis in the systemic disease which permits the retinal changes, especially hypertension. There are fallacies in the ophthalmoscopic examination of as essing arteriolar calibre by the arteriovenous ratio. The selection of comparable vessels by ophthalmoscopy for the estimation of the arteriovenous ratioshould be fully appreciated, and no other reference base is available unless measurments are made on retinal photographs or by means of an ophthalmoscope fitted with a special graticule. Studies were made by a random selection of 30 normal persons (60 fundi) in 2nd decades. This series of fundus color photographs incorporated a field extending to 3-5 disc diameters from the disc margin. This paper is a series on the retinal vessels which was concerned with the arteriovenous comparable pairing and its ratio. The results were: 1. Total comparable pairs ware 142 pairs in the 60 fundi. 2. In the 60 fundi comparable pairs were found in 50, and in 118 out of 240 quadrants(48 percent). 3. Comparable pairs were found more frequently in the temporal half of fundi. 4. The mean A/V ratio was 0.703.
