1.Parapatellar Complications after ACL Reconstruction Using Bone-Patellar Tendon-Bone Autograft.

PURPOSE: To evaluate clinical and radiological results and to analyse the parapatellar complications after endoscopic anterior cruciate ligament reconstruction using central one-third bone-patellar tendon-bone autografts. MATERIALS AND METHODS: 66 cases among 158 consecutive cases from Feb 1990 to May 1996 were reviewed and evaluated with regard to patellofemoral pain, crepitus, quadriceps atrophy, graft donor site complication, Lysholm knee scoring system and radiological assessment. The average period of follow up was 23 months (range, 12-57 months) and the average age at operation was 31 years old (range, 18-58 years). RESULTS: The average Lysholmn knee score improved from 57.5 points preoperatively to 91.3 points at follow up. The instrumented anterior laxity test showed that excellent anterior stability was regained in most patients. There were many cases of parapatellar complications, 7 cases (10.6%) of anterior knee pain, 21 cases (32.8%) of crepitus, 43 cases (65.1%) of quadriceps weakness, 29 cases (45.3%) of graft donor site paresthesia and 15 cases (23.4%) of pain on kneeling. CONCLUSIONS: Although endoscopic ACL reconstruction using central one-third bone-patellar tendon-bone autografts had good clinical results, many parapatellar complications were noted. In order to prevent these complications, different reconstruction techniques and graft materials should be considered

2.Comparison of SITA-standard with full threshold strategy of Humphrey field Analyzer in Glaucoma.

PURPOSE: The Swedish Interactive Thresholding Algorithm (SITA) strategies is a new diagnostic tool of Humphrey automatic perimetry and has been reported to reduce testing time without loss of useful diagnostic information. To test the usefulness of this new diagnostic method, we compared the SITA and full threshold diagnostic method, which had been used previously. METHODS: Forty six patients (46 eyes) who were followed up for glucoma or ocular hypertension were in this study. Autoperimetry was performed using both SITA and full threshold strategies. RESULTS: The SITA showed shorter testing time (7.74+/-1.25 min) than the full threshold (14.4+2.43 min). There were significant differences between the full threshold MD (-8.06+8.11 dB) and SITA MD (-7.45 +/-8.32 dB), as well as full threshold PSD (4.47+/-2.50 dB) and SITA PSD (3.67+/-2.56 dB), when compared with data of greater than -12 dB MD respectively(p<0.05). Two cases showed changed diagnosis in GHT. CONCLUSIONS: SITA is a good test method with short test time. However, changing from full threshold to the SITA method should be reconsidered due to variation in the amount of field defect depending on the damage progression and the possibility of changes in diagnosis.

4.Hydrogen Peroxide Producing Lactobacilli in Women with Cervical Neoplasia.

PURPOSE: It is well known that human papillomavirus (HPV) is the main cause of cervical neoplasia, and hydrogen peroxide-producing lactobacilli are the most important microorganisms for maintaining the balance of the vaginal ecosystem. The purpose of our study was to investigate the relationship of hydrogen peroxide-producing lactobacilli, cervical neoplasia and high-risk HPV. MATERIALS AND METHODS: We enrolled 1138 women with abnormal cervical smears or cervicograms who were referred to the department of Obstetrics and Gynecology at Chonnam National University Medical School. In all of them, 1,138 vaginal swabs were collected for the qualitative assay of hydrogen peroxide producing lactobacilli and 150 cervical swabs were used for the HPV hybrid capture II test without regard to the subjects' pregnancy status. In the non-pregnant women, 880 cervical biopsies and/or loop electrosurgical excision procedures were performed for making the histological diagnosis. RESULTS: There was no significant difference not only between the distribution of H2O2 producing lactobacilli and the cervical histology, but also between the distribution of H2O2 producing lactobacilli and the positivity for high-risk HPV. CONCLUSIONS: Both cervical neoplasia and high-risk HPV may not be influenced by the existence of hydrogen peroxide producing lactobacilli in the vagina.

5.Intraoperative Pulmonary Embolism: A case report.

A 62 year old female patient was transferred to the operating room for open reduction and internal fixation of the left femur fracture under general anesthesia. At 15min. after femur tourniquet application, there were suddenly decreased oxygen saturation, end-tidal CO2 concentration and blood pressure. We suspected a pulmonary embolism, and attempted vigorous emergency treatment and intensive care including ventilator care, vasopressor drug use, pulmonary artery pressure monitoring. At the 5th day after intensive care unit, she was transferred to general ward and she discharged without any sequelae after 17th day postoperatively.

6.Asynchronous Bilateral Renal Infarction with Atrial Fibrillation Korea.

Acute renal infarction is an uncommon disease which is often delayed or missed due to its rarity and unspecific clinical presentations. Most patients have an underlying disease or embolic event that can be detected easily by history and physical examination. The frequently noted symptoms include flank, abdominal, and back pain, as well as nausea. The pain is usually acute, severe, and sharp in nature without radiation. The most sensitive laboratory test is lactate dehydrogenase. Angiography, renal scintigraphy, IVP (intravenous pyelography), sonography and, CT (computed tomography) can be used in diagnosing renal infarction. However, CT is more preferred for the diagnosis of renal infarction because it has the advantages of easy accessability and noninvasiveness. Once diagnosed, conservative treatment with intra-arterial infusion of thrombolytics or heparin appears to be the most favorable modality. We report a 82-year-old male with asynchronous bilateral renal infarctions associated with atrial fibrillation and coronary artery disease. His clinical status improved during admission without any events.

7.A Child with an Acute Abdomen due to a Ruptured Hepatoblastoma.

A hepatoblastoma is the most common liver tumor in children between 6 months to 3 years of age. It is a rare malignancy of the liver with an incidence of 0.7 to 1 case per 1 million children under 15 years of age in Western countries. Males are more affected. Patients usually have normal liver function test, but have high serum levels of alpha-fetoprotein (AFP), which is a sensitive diagnostic marker. They usually present with abdominal distension or an asymptomatic abdominal mass. Rarely, patients with a hepatoblastoma show an acute abdominal emergency which is not typical for the disease process. The management of a ruptured hepatocellular carcinoma provides guidance for diagnosis and treatment of a hepatoblastoma because the incidence of hepatoblastoma is low. We report a child who presented abdominal pain and distension with the diagnosis of a ruptured hepatoblastoma.

8.Relationship of Air Pollution and Pediatric Respiratory Diseases in Incheon City.

PURPOSE: It is generally accepted that the air pollution can cause acute respiratory diseases. This study was undertaken to investigate the relationship between the increase in the level of air pollutants and hospital visits for pediatric respiratory diseases in Incheon city. METHODS: We retrospectively reviewed the medical records of pediatric patients who visited the outpatient department(OPD) of Inha University Hospital for respiratory diseases(such as bronchial asthma, pneumonia, bronchitis, bronchiolitis and URI) from January 1 to December 31 1997. We converted the data into an average per day that included meteorologic data of air pollution(such as O3, CO, SO2, NO2, and PM10) and weather data(such as atmospheric temperature, relative humidity) in three different places in Incheon city. We used a Poisson distribution, selecting a lowess statistics model. We also used the S-PLUS statistics program. RESULTS: Ozone(O3), CO, and SO2 were all found to have significant associations with the OPD visits for pediatric respiratory diseases. After meteorologic variables(such as CO, SO2, NO2, PM10, atmospheric temperature, and relative humidity) controlled, we determined the relative risk between the increase in the OPD visits for respiratory diseases and every 0.01 ppm increase in O3. The relative risk was 1.16 and the 95% confidence intervals were 1.12-1.20. CONCLUSION: There was a significant relationship between the incidence of pediatric respiratory diseases and the increase in the level of air pollutants, especially O3, CO, and SO2 in Incheon city. Further studies on the effects of continuous daily exposure to low or ambient levels of air pollutants are needed.

9.Effect of Hyperkalemia and Hemolysis Caused by Hyperacute Rejection on Cardiac Function in Pig to Human Ex Vivo Xenogeneic Cardiac Perfusion Model.

BACKGROUND AND OBJECTIVES: Hyperacute rejection (HAR) is a major obstacle to successful xenotransplantation of vascularized organs. This study was conducted to observe the effect of hemolysis of perfused human whole blood on pig heart function, and determine the major risk factors for preservation of xenoperfused cardiac function using ex-vivo pig to human xenogeneic cardiac perfusion model. MATERIALS AND METHODS: Harvested pig hearts were perfused with normal human whole blood (group 1), two different types of pre-treated human whole blood (group 2: immunoglobulins were depleted by plasmapheresis, group 3: pre-treated with plasmapheresis, GAS914, cobra venom factor (CVF) and steroid), and normal porcine whole blood as control (group 4) for 3 hours. RESULTS: Duration of heart beat was significantly prolonged in group 2 and group 3. Histological examination showed widespread HAR features but was gradually delayed in groups 2 and 3 compared to group 1. The absolute levels of serum creatine kinase-MB and Troponin I increased gradually, and was lower in group 3. Serum hemoglobin levels were rapidly increased in groups 3 and 4, compared to group 1. Extracellular potassium level increased sharply from the beginning of blood perfusion in groups 1, 2 and 3, compared to group 4. CONCLUSION: Pretreatment of human whole blood, including immunoglobulin depletion, CVF and steroid reduced and delayed the destruction of pig myocardium by HAR. However, the increased extracellular potassium levels in groups 1, 2 and 3 reflected that these treatments could not prohibit myocardial injury by HAR.

10.A Case of Infective Endocarditis with Erysipelothrix rhusiopathiae.

Erysipelothrix rhusiopathiae is an uncommon pathogen in human, which causes zoonotic infection. The most common type of human disease with E. rhusiopathiae is a localized skin infection (erysipeloed). E. rhusiopathiae endocarditis is very rare, which usually develops in normal heart valves and causes a variety of complications leading to valve replacement. We described a case of E. rhusiopathiae endocarditis which developed in a 56-year-old man without a predisposing valvular heart disease. In this case, aortic valve and mitral valve were involved and emergency valve replacement was required, which improved patient's condition markedly. Intravenous therapy with penicillin G and subsequent ceftriaxone were given for 8 weeks after surgery and he was recovered without relapse.
