1.Present View of Cataract Surgery in Korea.

During last decade especially after Korea-war a large number of new preparations have been added to our every day practice for this it seems very interesting to know the therapeutic habits of korean ophthalmologist today. As its test case, a questionaire about senile cataract extraction which they usuallyprescribed was circulated, and seventy questionaires were maled out. (1) in the culture from conjuctiva, the large number of ophthalmologist, 72%, do not or can not do because of having no such equipment or of thinking no necessary. (2) in the properative use use of mydriatics, 32% of ophthalmologists used Homatropine of atropine just before operation, and it is very interesting to note that 24% of them used Homatropine the day before operation. (3) in the akinesia, 40% of ophthalmologists ussed Van Lint or modified Van Lint type akinesia, and only 8% of them used O Brien type. (4) in the bridle or stay suture, 80% of them usually used and 20% used sometimes. (5) in the retrobulbar injection 70% of them usually prescribed, and 16% sometimes 20 of them have the experience of intraorbital hemorrhage after such injection. (6) in the corneoscleral suture, 84% of the them used such suture, and majority of them sutured 3 places, 10, 12 and 2 o'clock position and also large number of them used Mc lean type suture. (7) in the corneoscleral incision, 76% of ophthalmologist, used keratome and scissor, and also 16% used Graefes knite, and only 8% of them used Bardparker knife because of only economical reasons. (8) About iridectomy, 60% of them used perpheral button hole iridectomy and 20% used peripheral iridotomy. (9) in the lens extraction, 72% of them used intracapsular extraction including one loop extraction and majority of them used such technique since last several years. (10) After lens extraction, 52% of them injected air in the anterior chamber, and 20% used normal saline. (11) After operation, 32% of ophthalmologists employed steroid therapy and just same percent of ophthalmologists did not employed such therapy because of delayed wound healing. 36% of them gave no answeres. (12) When found binocular cataracts, 60% of them operated both in same time only because of economical reasons, and 40% of them operated seperately, because it gave too much stress to the patient physically or mentaly, and also it is safety.

3.A study on the lipids of Chinese liverfluke, Clonorchis sinensis.

The present study was undertaken to observe the quality and quantity of lipids in the adult worms of Chinese liverfluke, Clonorchis sinensis. Lipid extraction was done by the methods of Folch et a1. (l957) and Kenny (1952), and then the extracted lipid fractions of the worm were separated by thin layer chromatography. Those fractions were also subjected to perform the quantitative analyses of glycerides, cholesterols and phospholipids. The results obtained were summarized as follows: Total amount of glyceride was 37.56 mg per gram of worm tissue and the amount of monoglyceride was 8.34 mg per gm; diglyceride, 15.46 mg per gm; and triglyceride, 12.86 mg per gm. Total amount of cholesterol was 3.30 mg per gm of worm tissue, and the esterified cholesterol (1.72 mg/gm) was a little more than that of free cholesterol (1.26 mg/gm). The following 8 phospholipids were detected in the worm tissue of C. sinensis, i.e., lysophosphatidylcholine, phosphatidylcholine, phophatidylinositol, sphingomyelin, phosphatidylglycerol, phosphatidylserine, phosphatidylethanolamine and an unknown phospholipid.

5.A Serous Papillary Cystadenoma of Low Malignant Potential in Paratesticular Tissue.

The serous papillary cystadenoma of low malignant potential in the paratesticular tissue is a tumor characterized by its resemblance to ovarian serous papillary tumor of low malignant potential. Several possible origins have been proposed but it is thought to originate from the Mullerian-type epithelium just like ovarian serous tumor of low grade malignancy. Because of the rarity of this tumor, the natural history and treatment have not been established. Herein we report on a serous papillary cystadenoma arising in the paratesticular tissue of a 12 year old . To our knowledge, this is the first reported case in the Korean literature. The size of this cystic tumor in this patient was 0.9x0.9x0.8 cm and had a sponge-like cut surface. The papillary structure was lined by both ciliated and nonciliated secretory cells, the same cellular structure as the salpinx. Areas with just a few mitoses and stratification of epithelial cells were noted, but invasion into the paratesticular tissue was absent, suggesting borderline malignancy. He is doing well after 5 months follow up. Prognosis of this scrotal tumor is thought to be better than the ovarian tumor of similar histology due to a better possibility of early detection.
6.A Cases of Endodermal Sinus Tumor in Sacrococcygeal Region

A endodermal sinus tumer (Teilum) is a highly malignant germ cell tumor showing a selective overgrowth of yalk sac endoderm intimately associated with the extraembryonic mesoblast. Several cases of the tumor originating from the ovaries have been reported in the literature. We present a case of the tumor with a rare origin from the sacrococcygeal region.

7.Intramedullary Screw Fixation for Clavicle Shaft Fractures: Comparison of the Anterograde versus the Retrograde Technique.

BACKGROUND: The purpose of this study was to investigate the difference between two nailing approaches of intramedullary screw fixation, the retrograde nailing versus the anterograde nailing, on the radiological and clinical outcomes in patients with clavicle shaft fractures. METHODS: From April 2002 to August 2014, we enrolled a total of 22 patients with clavicle shaft fractures to participate in this study. Twelve patients received retrograde intramedullary nailing and 10 received anterograde nailing. The average duration of follow-up was 12 months. In all the patients, we took follow-up radiographs of the anteroposterior and the axial views to assess the postoperative radiological outcomes. We measured the visual analogue scale (VAS) score, American Shoulder and Elbow Surgeons (ASES) score, and the range of motion (ROM). RESULTS: Clinically, we did not find a statistically significant difference in the retrograde group and the anterograde group in terms of the duration to bone union, the VAS score the ASES score and the ROMs. Radiologically, we found that the difference in the clavicle shortening of the affected arm and the unaffected arm did not show a statistically significant difference at the immediate postoperative assessment. we found that the difference in the clavicle shortening of the affected arm between the immediate postoperative and the final follow-up value did not show a statistically significant difference. CONCLUSIONS: We found that both the retrograde nailing and the anterograde nailing gave a favorable outcome for clavicle shaft fractures. Although we saw evidence of clavicle shortening after intramedullary screw fixation, this was not a factor that influenced clinical outcome.

8.Histopathologic Study of Erythema Nodosum: Licke Lesions in Behcet's Disease.

A histopathological study of erythema nodosum-like lesions in Behcet's disease was performed on 55 patients with incomplete and suspect types of Behcet's disease. Relatively common characteristics in histopathology of erythema nodosum-like lesions could be found in the patients with incomplete type of Behcet's disease. The histopathologic findings of erythema nodosum-like lesions in 21 patients with incomplete type of Behcet's disease were as follows : l. A moderate lymphocytic infiltration was found around the blood vessels and the sweat glands in the dermis. In the subcutaneous tissue, besides lymphohistiocytic infiltration, neutrophils were present in significant number in 9 cases (42.9%) 2. Vasculitis of small vessels was found within the periphery of fat lobules in 7 cases (81.0%), Five cases (23.8%) also showed thrombophlebitis. Seven cases (33.3%) showed histiocytic granuloma formation within the fat lobules. Microabscess formation in the subcutaneous tissue was seen in 5 cases (23.8 %), but it was not certain whether this was the charateristic feature.

9.Basal Cell Carcinoma and Actinic Keratosis Treated with Intralesional Injection of Recombinant Alpha-2 Interferon.

BACKGROUND: The effects of intralesional recombinant alpha-2 interferon(IFN) in the treatment of basal cell carcinoma(BCC) and actinic keratosis(AK) would be of value in selected patients. OBJECTIVE: Our purpose is to evaluate the differences of cellular immune reactions between the responded and the unresponded cases treated with IFN. METHODS: We treated five patients with BCC and two patients with AK with the intralesional injection of recombinant alpha-2 IFN. We performed immunohistochemical stainings with paraffin-embedded specimens from the pretreated and posttreated skin lesions. RESULTS: Four patients(three with BCC and one with AK) were healed and three patients showed improvement after treatment. In immunohistochemical stainings, there was an increased number of T cells in the dermal infiltrate of the responded cases but not in the unresponded cases. CONCLUSION: Intralesional IFN may induce T cell-mediated immune response at the site of the tumor by immune modulation.

10.Calcifying Synovial Sarcoma.

A case of calcifying synovial sarcoma, occuring in the deep muscle fascia of the left thigh is reported. The presence of extensive calcification in synovial sarcoma is a favorable sign for prognosis. The patient was a 31-year-old female. The tumor mass had been present for 3 years, accompanying local tenderness. X-ray revealed a soft tissue tumor with central calcification, which was located between the adductor magnus and brevis of the left thigh. The tumor size was 7 x 5.5 cm. There was no connection with the knee joint or the femur. Grossly, the tumor was a relatively well circumscribed hard tumor with massive calcification. Microscopically, the tumor was composed of predominantly spindle cells with accompanying hyalinization, numerous spherical concretions and ossification. The epithelial component was not clearly noted. Mitotic figures were rarely noted in the densely cellular area. Immunohistochemical staining for EMA, S-100, vimentin, and carcinoembryonic antigen was negative while vimentin and cytokeratin were weakly and focally positive. EM study revealed multiple desmosome-like calcification intercellular junctions with a slit-like lumen and an incomplete basal lamina, which suggest that tumor show these cells were undergoing epithelia] differentiation. Above evidence suggest that this tumor is a synovial sarcoma associated with a large area of calcification, a so called calcifying synovial sarcoma. No recurrence was noted in one and half years of follow up.
