2.Skin Metastasis of Adenoid Cystic Carcinoma of Parotid Gland.

Adenoid cystic carcinoma is known to be a neoplasm of the major and minor salivary glands but it also occurs in the lacrimal gland, external auditory canal, easphagus, breast and skin. Adenoid cystic carcinoma of salivary gland is a slow-growin nalignant tumor. Although metastasis of this tumor clevelnps in about half the cases, its meta taes to the overlying skin is not common, moreover the skin metastasis to the remote site is raely found. Primary cutaneous adenoid cystic carcinoma is rare and histologically indistingisl able from skin metastasis of adenoid cystic carcinoma of the salivary gland. A 53 year-old man presented a solitary, well defined, erythen at us, 1.3 x 1.3cm sized nodule on the vertex of the scalp, which revealed the typical histopatholog ndings of adenoid cystic carcinoma. One and half years ago, a tumor of the parotid gland va removed and proved to be a cribriform pattern of adenoid cystic carcinoma. The skin lesion was diaghosed as a skin metastasis of the alenoid cystic carcinoma of parotid gland rather than a primary cutaneous adenoid cystic carcinona because a similar pattern of adenoid cystic carcinorom had been discovered on the parotidg an l although it developed on the scalp, the usual site of primary cutaneous adenoid cystic carcinama.

3.A Case of Multiple Intravascular Papillary Endothelial Hyperplasia.

Intravascular papillary endotholial hyperplasia is a relatively rare (istose which is characterized by the development of endothelialdined papillary projections in a vascula humen. It is commonly found to be associated with thromboticrraterial and is now considered a reative process of the endothelium rather than a neoplastic one. Although this disease usually presents petri dish or bluish nodule on the head, neck or upper extremiti it can develop anywhere on the bod, . There are many reports about lesions in unusal locations, but t.hey are almost always solitary. A 61-year-old woman complained of multiple, tender, 1 x 1 to 4 x 5 creasized nodules on both hands, antecubital fossa, chest, abdomen, left shoulder and right calf. These the nodules showed reddish to bluish colors and had long hisbiries from 1 year to 7 years. Four our 13 lesions were excised for histopathologic diagnosis intravascular papillary endothelial hyperplaian 3 lesions and cavernous hemangioma in 1 lesion.(

6.Standardization and Validity Study of Korean Nomenclatures of The Home Health Care Classification.

The purpose of this study was to develop and test the validity of standardized Korean nomenclature of the Home Health Care Classification(HHCC), developed by Saba at the University of Georgetown. The four phases of the study were : (1) Two professors and 15 graduate students, who were taking a nursing intervention course, translated the HHCC into Korean. (2)12 nurses with various clinical backgrounds reviewed each nomenclature taking into consideration of the definition and activities, and the Delphi method was used to determine the best appropriate nomenclature for each term; (3)20 academic and clinical experts in nursing were given a questionnaire to rate the validity of each Korean nomenclature using a 5 point Likert scale ranging from very inappropriate to very appropriate; (4) Five members of the Korean Nurses Association Research Committee reviewed the survey results and determined the most appropriate Korean nomenclature for each term of the HHCC. Most of the nomenclatures had a score of more than 4.0, but several terms had a score lower than 4.0. They are Ambulation therapy(3.1.1), Anticipatory grieving(3.11), Sexual patterns alteration(3.17), Nursing care coordination(3. 22), Physical therapist service(3.22), Regular diet(3.33), and Instrumental activities of daily living(3.44). These research results will be included in the nursing vocabulary dictionary to be published by the Korean Nurses Association.

7.Clear Cell Adenocarcinoma of the Urinary Bladder Accompanied by Vesical Endometriosis.

A case of clear cell adenocarcinoma arising in the female urinary bladder, which is accompanied by endometriosis of the urinary bladder and the uterus, is reported. The carcinoma protruded into the vesical lumen as a fungating mass, and had a tubulocyotic pattern. The tumor cell had intracytoplasmic glycogen and electron microscopically short microvilli on their surface, resembling clear cell acenocarcinoma of the female genital tract including ovary. This is the fourth case report of clear cell adenocarcinoma complicating vesical endometriosis, and may support the view that clear cell carcinome arises from endometriosis which, in turn, from the Mullerian remnant.

9.Serum Leptin in Cord Blood and Its Relation with Birth Weight and Metabolic Parameters.

BACKGROUND: Leptin, produced in the adipose tissue, is involved in the regulation of body weight. The release of the leptin is increased in obese adults even in children. This study investigated whether the serum leptin in cord blood was related to babys birth weight and metabolic parameters. METHODS: 71 pairs of singleton pregnancy babies and their mother were studied. Babies are classified in LGA (large for gestational age), AGA (appropriate for gestational age), SGA (small for gestational age) three groups. After delivery, cord blood and maternal venous blood samples were drawn. We measured the plasma leptin, insulin-like growth factor (IGF)-1, insulin and proinsulin in cord and maternal serum. RESULTS: The concentration of leptin from cord blood was increased in LGA babies and decreased in SGA babies compued with the level in AGA babies. There was positive correlatian (r=0.55, p<0.01) between the plasma leptin level in cord and birth weight. There were positive correlatian between both the plasma proinsulin (r=0.37, p<0.01) and IGF-1 (r=0.32, p<0.01) and birth weight, too. But there was no difference between female and male baby's cord blood leptin level. In multiple regression analysis, cord blood leptin level was found independent factor related to birth weight ( p=0.001) CONCLUDION : The plasma leptin, proinsulin and IGF-1 is correlates to the birth weight. These data provide evidence that leptin and proinsulin are highly related to the nutritional status already during the fetal periods, and effect on the intrauterine fetal growth.

10.A Case of 13 Ring Chromosome Syndrome.

We have experienced a case of 13 ring chromosome in a 40-month-old girl who demonstrated psychomotor retardation with delayed speech, growth retardation, hearing loss(left), microcephaly, trigonocephaly with flat occiput, hypertelorism, epicanthal folds, microophthalmia, broad prominamt nasal bridge, high arched palate, micrognathia, large auricles and other anomalies. Cytogenetic studies of peripheral blood lymphocytes with differential staining of chromosomes revealed 46, XX, r13. Her parents' karyotypes were normal. We reported the case with the review of the associated literatures.
