1.Analysis of the level of high-sensitivity C-reactive protein level and serum lipids in elderly patients with early chronic kidney disease

ObjectiveTo observe the changing of the level of high-sensitivity C-reactive protein level and serum lipids in elderly patients with early chronic kidney disease. Methods90 cases of early chronic kidney disease were collected and 90 cases without chronic kidney disease served as control.The levels of total cholesterol(TC),triglyeride(TG),high density lipoprotein(HDL),low density lipoprotein(LDL)and high-sensitivity C-reactive protein (hs-CRP)were measured. ResultsThere was significant difference between early chronic kidney disease group and the control group on TG,HDL and hs-CRP(all P<0.05).The level of HDL in study group was significantly lower than those in control group(P<0.05=. ConclusionThe patients with early chronic kidney disease often had hypertriglyceridemia,declining HDL and elevating hs-CRP.It should be paid attention early.
2.Therapeutic effect of topiramate on infantile spasms

Objective To observe the clinical effects of topiramate together with low-dose ACTH and prednisone、vitamine B6、gamma globulin on infantile spasms. Methods 134 infantile spasms patients were selected and randomly divided into control group and treatment group, each group 67 patients. The control group was treated with sodium valproate. The treatment group received low-dose ACTH and prednisone、topiramate、vitamine B6 、gamma globulin. The two groups all completed a therapeutic period of 3 months. Results The effect of the treatment group compared with the control group has significantly statistical difference (P < 0.05) ;the result of the treatment group'electroencephalogran compared with that of the control group also has significants difference (P < 0.05). Conclusion The therapeutic effect of low-dose ACTH and prednisone、 topiramate、 vitamine B6、 gamma globulin together on infantile spasms was much better than just taking sodium valproate only.
3.The applications of vaginal hysterectomy of non-prolapsed uterus lesion in primary hospital

ObjectiveTo explore the application value and techniques of vaginal hysterectomy of non-prolapsed uterus lesion in primary hospital. Methods140 cases who suffered from uterine fibroid without adhesion,glandular fibroids,functional bleeding without uterine prolapse and malignant disease were selected and divided into observation group(80 cases with vaginal hysterectomy)and control group(60 cases with routine abdominal hysterectomy).Then the clinical treatment effect was analyzed. ResultsCompared with control group,peri-operative bleeding (150 ±23)ml vs(243 ±41)ml,operation time(65 ± 14.0)min vs(90 ± 27.0)min,body temperature recovery time (3 ±0.5)d vs(4 ±0.8)d,anus exhaust time(14 ± 1.8)h vs(26 ±2.7)h,the length of time(5 ±0.8)d vs(11 ±2.9)d in observation group were all reduced,differences were significant(all P<0.01)and no case was transferred to opening appendectomy. ConclusionFor these disease including uterine fibroids with no adhesion,glandular fibroid uterine bleeding,the vaginal hysterectomy was safe and feasible,and had the advantage of smaller trauma,less bleeding,shorter hospitalization time,faster recovery etc.It was worthy to be popularized in primary hospital.
4.Clinical study on myocardial enzymes change of patients with severe pneumonia in ICU

Objective To observe the clinical significance of myocardial enzymes change of patients with severe pneumonia in intensive care unit (ICU). Methods 60 patients with severe pneumonia ( severe pneumonia group) and 58 patients with ordinary pneumonia( pneumonia group) were studied,and 60 healthy subjects were studied as normal control group. The CK-MB,CK,HBDH and AST levels of the research objects were analyzed by automatic chemistry analyzer. Results The enzyme indicators of severe pneumonia group were higher than the pneumonia group and normal control group, compared with the pneumonia group and normal control group, the differences were significant( all P < 0. 05 ); The enzyme indicators of the pneumonia group compared with normal control group the difference was not significant(P > 0. 05). The effective rate of severe pneumonia was 96.7% ,ineffective 2 cases died in the treatment. The effective rate of pneumonia group was 100.0% ,and no deaths occurred. Conclusion There was myocardial damage in patients with severe pneumonia,myocardial enzyme could be used as a valuable indicator for diagnosis, observing tendency, and predicting prognosis of patients with severe pneumonia in ICU.
5.Experimental study on the effect of yam polysaccharide for diabetic mice

Objective To explore the effect of yam polysaccharide for the diabetic mice. Methods Ninty mice were randomly divided into three groups ,yam polysaccharide group(n = 30), glibenclamide group (n = 30) and control group(n = 30). Mice were Intraperitoneal injection of alloxan (200mg/kg) to establish diabetic model. Yam polysaccharide group was administered yam polysaccharide for 12d. Results Yam polysaccharide significantly decreased the blood sugar of the diabetic mice. There were not significantly different in two groups (P > 0.05).Conclusion Yam polysaccharide had obvious role in decreasing blood glucose.
6.Diagnosis value of color Doppler ultrasound of the carotid atherosclerosis plaques in diabetic patients

ObjectiveTo explore the diagnosis value of color Doppler ultrasound of the carotid atherosclerosis plaques in diabetic patients. MethodsColor Doppler ultrasonic assessment of carotid was performed in 124 diabetic patients(test group)and 100 normal controls(control group).The intima-media thickness(IMT)and the carotid atherosclerosis plaques parameters were measured.The results were compared and analyzed between two groups. ResultsThe IMT and the incidence rate of carotid atherosclerosis plaques in the test group were higher than those in the control group(all P<0.05).There was no statistical difference in the site of the plaques between test and control group(P>0.05). ConclusionThe incidence rate of the carotid atherosclerosis of type 2 diabetes was higher.Carotid color Doppler ultrasonography was an optimal technique for screening and diagnosing atherosclerosis.
7.Clinical Significance of Serum Cardiac Troponin T and Serum uric acid levels in Aged Patients with Chronic Congestive Heart Failure

Objective To evaluate the correlations of serum cardiac troponin T and serum uric acid levels in in aged patients with chronic congestive heart failure(CHF) and the effect of cardiac troponin T(cTnT) on the prognosis of the CHF. Methods 200 cases of congestive heart failure patients(Cardiac function NYHA class Ⅰ-Ⅳ) were enrolled for retrospective analysis and divided into four groups ,50 cases per group;randomly picked 50 cases of pantients with cardiac function NYHA class Ⅰ as a control group; and analyzed the relationship of serum uric (UA) and cTnT level and the occurrence of CHF. All the patients were arranged into normal group and higher group according to cTnT concentration. Cardiac events, including repeated or aggravated heart failure or cardiac death,in patients with increased cTnT during hospitalization were compared to those without it. Results In congestive heart failure group (Cardiac function NYHA class Ⅱ-Ⅳ) ,the cTnT and UA contents increased significantly than that in control group.Incidence of cardiac events was higher in patients with increased cTnT than without it (P < 0.05). Conclusion The heavier the congestive heart failure, the blood uric acid and troponin T were more elevated, so they were closely linked with congestive heart failure, and were the factor for onset of congestive heart failure, and could be combined to evaluate the severity of heart failure in aged patients. The cTnT could predict the prognosis of the old patients with CHF.
8.The effect of anterior fusion with the internal fixation in treatment of fracture-dislocation complicated with spinal cord injure

Objective To study the the effect and rate of complications of anterior fusion with the internal fixation in treatment of fracture-dislocation complicated with spinal cord injure. Methods 46 patients with fracturedislocation complicated with spinal cord injure were selected as group A 23 cases and group B 23 cases according to the methods of treatment,group A was treated with anterior fusion with the internal fixation,group B was treated with cervical posterior, then the total effective rate, surgical time, the rate of complications, ASIA scores and SF-36 scores before and after the treatment of the two groups were studied. Results The total effective rate of group A(91.3% )was higher than that (73.9%) of group B ( 17/23 ) ( x2 = 6. 38, P < 0. 05 ), surgical time ( 83.6 ± 23.5 ) min of group A was shorter than that of group B( 106.8 ±25. 1)min(t =5.37,P <0. 05) ,the rate[13.0% (3/23)]of complications of group A was lower than that of group B[26. 1% (6/23)] ,ASIA scores and SF-36 scores of group A after the treatment were better than those of group B(P <0. 05) ,there were significant differences. Conclusion The effect of anterior fusion with the internal fixation in treatment of fracture-dislocation complicated with spinal cord injure was better,and the the rate of complications was lower,and it was worthy of exploitation and promotion.
9.Therapeutic effect of the regimen containing Moxifloxacin in intensive phase for pulmonary tuberculosis patients

Objective To explore the effect and safety of the regimen containing Moxifloxacin in intensive phase for pulmonary tuberculosis patients. Methods 88 cases with initial tuberculosis(each group of 44 cases) were studied in comparison in the same period. The treatment group contained isoniazid, rifampicin and moxifloxacin of antituberculosis regime and the control group contained isoniazid,rifampicin and pyrazineamide. Results After 2 months intensive phase anti-tubercular treatment ,the sputum negative conversion rates was 95.0% and control's was 94.4%(P > 0.05); the absorb rate of lesions: the treatment group 77.3 % and the control group 79.5%; the liver damage rate: the treatment group and control group was 68.2% ,34.1% respectively(P < 0.05). Conclusion Anti-tuberculosis intensive phase regimes containing moxifloxacin for tuberculosis treatment was safe and effective in initial treatment of pulmonary tuberculose.
10.Experience on laparoscopic cholecystectomy with report of 168 cases in the initial stage

Objective To discuss the strategies of lower complications of LC in the initial stage. Methods The clinical data of 168 patients,from Sep. 2008 to May. 2010 ,treated with laparoscopic cholectectomy were retrospectively analyzed. Results LC was performed in all patients(100%). None of the series had intra-abdominal bleeding,biliary tract injury and no other serious complications were observed. Conclusion Through correct perioperative management and meticulous operation, LC was safe and complications lower in the initial stage.
