he Magnetic. resonance imaging has been used widely to evaluate the disk position without any interruption of the TMJ structures, and the dynamic MRI presenting computed serial imaging or the video-recorded simulation images is thought to be very effective to evaluate the disk position under function. This is to study the correlation between the clinical diagnosis and the findings of dynamic MRI for diagnosis of internal derangement of the 7 patients were examined clinically, and the movement of TMJ meniscus was reviewed in the dynamic MRI. MRI was very reliable to diagnose the amount of anterior displacement of articular disc, the structural abnormality of temporomandibular joint, the cause of functional limitation, and to differentiate the muscle related pain & dysfunction.

Speckled lentiginous nevus(SLN) is characterized by a tan background patch on which more darkly pigmented macules and papules are randomly distributed. The presence of agminated Spitz nevi with SLN is well known, but a solitary Spitz nevus arising on a SLN is rare. We herein report a case of SLN with two Spitz nevi developed individually and congenital melanocytic nevus in a 9-year-old boy.

Isoconazole nitrate is one of the broad-spectrum antimycotic agents recently developed from imidazole derivatives. Authors performed ulticenter trials to evaluate the therapeutic effect of Travogen and Travocort in superficial fungal diseases. In the usual superficial fungal diseases, Travogen was applied 4 weeks. In the inflammatory and eczematoid superficial fungal diseases, Travocort was applied 2 weeks initially and then followed by 2 weeks application of Travogen. One hundred and ninty one patients with superficial fungal diseases were included in this study during g months from December, 1982 to August, 19$3 The obtained results were as follows; The overall cure rate of Travogen and Travocort in each superficial fungal diseases were gl 7g (tinea pedis), 98. L% (tinea cruris), 1ppg, (tinea corporis), 9g. 100% (tinea manus), 1ppg (tinea capitis) and 1pgg (candidiasis) respectively. 6.Volume-reserving Surgery after Photodynamic Therapy for Biliary Papillomatosis: A Case Report.

Biliary papillomatosis is rare, and its pathogenic mechanisms are not yet clear. Because of its high risk for malignancy transformation, surgical resection is regarded as a standard treatment. Photodynamic therapy (PDT) has been used by the intravenous administration of hematoporphyrin derivative followed by laser exposure. A photochemical process causes disturbance of the microvascular structure and degradation of membrane. Cholangitis is a major complication after PDT. A healthy 56-year-old man was diagnosed with biliary papillomatosis involving the common hepatic duct, both proximal intrahepatic bile ducts (IHD), and the right posterior IHD. After biliary decompression by endoscopic nasobiliary drainage, PDT was performed to avoid extensive liver resection and recurrence using endoscopic retrograde cholangiographic guidance. After portal vein embolization, the patient underwent extended right hemihepatectomy. Following administration of chemoradiation therapy with tegafur-uracil and 45 Gy due to local recurrence at postoperative 13 months, there was no local recurrence or distant metastases. This is the first case report on PDT for biliary papillomatosis in Korea. Preoperative PDT is beneficial for reducing the lesion in diffuse or multifocal biliary papillomatosis and may lead to curative and volume reserving surgery. Thus, PDT could improve the quality of life and prolong life expectation for biliary papillomatosis patients.

8.Carcinosarcoma of Pancreas.

Carcinosarcoma of the pancreas is a rare malignant tumor that shows a combined or mixed proliferation of carcinomatous and sarcomatous cells. This tumor has been variously called carcinosarcoma, pleomorphic large cell carcinoma, giant cell carcinoma, and undifferentiated carcinoma. A 52-year-old man was hospitalized for evaluation of his epigastric pain and jaundice. An abdominal computed tomography revealed the presence of a poorly enhancing mass, arising from the head of the pancreas. Pylorus preserving pancreaticoduodenectomy was performed. The final pathologic diagnosis was undifferentiated carcinoma with 2 distinct components. One component was a conventional infiltrating pancreatic ductal adenocarcinoma, and the other component was sarcoma. We present here a case of carcinosarcoma of the pancreas along with a review of the literatures.

Pulmonary hypertension is an increase in blood pressure in the pulmonary artery, pulmonary vein or pulmonary capillaries. Depending on the cause, pulmonary hypertension can be a severe disease with markedly decreased exercise tolerance and right-sided heart failure. Pulmonary hypertension can present as one of five different types: arterial, venous, hypoxic, thromboembolic, or miscellaneous. Chronic obstructive pulmonary disease with severe pulmonary hypertension is a rare disease. A 52-year-old man presented with a complaint of aggravating dyspnea. The mean pulmonary arterial pressure was 61.5 mmHg by Doppler echocardiogram. The patient was prescribed diuretics, digoxin, bronchodilator, sildenafil, bosentan and an oxygen supply. However, he ultimately died of cor pulmonale. Thus, diagnosis and early combination therapy are important.

10.Odontogenic Myxoma of Maxilla: A Case Roport.

Odontogenic Myxoma of the jaws is a rare benign tumor derived from embryonic mesenchymal elements of dental anlage. It appears to originate from the dental papilla, follicle or periodontal ligament. Odontogenic Myxoma of the maxilla is less frequent but behaves more aggressively than that of the mandible, because it spreads through the maxillary sinus. Radiographically, the tumors present as unilocular or multilocular radiolucent lesions with well-defined borders with fine, bony trabeculae. On gross examination, the tumor appears as a smooth, glistening, gelatinous, lobulated mass. On microscopic examination, these neoplasms exhibit loose arrangement of stellate-shaped cells. The intercellular substance is a mucinous and homogeneous matrix. We report a case of odontogenic myxoma of the maxilla observed in our clinic with good prognosis after partial maxillectomy.
