1.An Epidemiological study for on Outbreak of Typhoid Fever.

An epidemiological study was conducted, from April 28 to June 3, 1975, to investigate source and course of infection of typhoid fever occurred in Munhyeon dong, Nam Gu, Busan. The obtained results were as follows : 1. The entity of unknown febrile disease was identified as Salmonella typhi, D group and it's symptoms were abdominal pain, diarrhea, hepatomegaly, nausea and vomiting, splenomegaly, rose spots, tenderness of ileocecal region etc., in that order. 2. The average duration from onset to recovery was 25.2 days and incidence rate was 39.5 per 1,000 population. 3. The source of infection was presumed a charity patient. 4. By the duration of report on the cases from onset, 10-19 days group was the highest. 5. The positive rate of initial stool culture was 38.9% and secondary was 5.6%. 6. Distribution of the cases by age and sex showed that 5-9 years old group was the highest as 30.5%, and male was higher than female. 7. Distribution of the cases by education level, under primary school as 66.6% was the highest. By the living standard, the highest was low income earner group as 77.8%. 8. Utilization rate of medical facilities was drug store (41.7%), herbal drug (8.3%), clinic (5.6%) in that order.

2.The Clinical Study on the Root Coverage Effects with Free Standing Connective tissue Graft.

A mucogingival grafting procedure has been developed to cover denuded root surface. The subepithelial connective tissue graft technique is very predictable and allows for a good esthetic results and minimum patient discomfort on the palate. However, in areas where there is a lack of vestibular depth and keratinized attached tissue, the presence of frena or heavy muscle attachment, covering the connective tissue graft with a mucosal flap is very difficult. The purpose of this study is to evaluate an alternative technique of root coverage using the free connective tissue graft. The results were as follows: 1. Probing depths didn't seem to vary significantly from the preoperative to postoperative period. 2. The amount of keratinized tissue showed an increase of 5.9+/-0.97mm from the preoperative level. 3. Total clinical exposed root coverage increase 72.2% compare with preoperative level. 4. The shrinkage from gingival margin is 4.2+/-1.15mm and the mean shrinkage rate is 40.1%. 5. The depth of the vestibule increased with the average distance from cementoenamel junction to mucogingival junction being 7.4+/-1.65mm.

3.The SEM Observation of The Various Root Treatment Effect On Furcation Area.

In periodontal regeneration treatment, access to the frucation area is very difficult. Thus complete removal of plaque, calculus and endotoxin is somewhat impossible. In this study, teeth that were extracted due to periodontal disease were used. The furcation area was treated with periodontal curette, ultrasonic scaler, roto bur and they observed using SEM. The result was follows 1. The group treatment with curette showed remaining plaque, the cementum existed in most of the surface and partial dentinal tubule orifice could be seen. 2. The group treatment with ultrasonic scaler showed less removalof plaque compared to curette and irregular surface could be seen. 3. The group treatment with roto bur showed cleaner surface and many dentinal tubule orifice could be seen compared to the curette and ultrasonic scaler groups. Thus when suing treatments such as bone grafting or guided tissue regeneration, it is considered that the furcation area should be treatment with Roto bur.

4.Effect Of Tetracycline And Root Planing Methods On The Root Surface.

Recently, alteration on chemical treatment on the root and removal of pathologic bacteria, which is the main reason for periodontal disease, by complete removal of infected cementum layer was been emphasied In this study, teeth extracted due to periodontal disease were root planed using periodontal curette and roto bur. Then they were treated with different concentration of Tetracycline HCl at different time. The state of root surface and change in the pre and post treatment was observed. The results were as follows. 1. The group treatment with periodontal curette and saline showed remaining plaque, debris and irregular surface and no dentianl tubule orifice could be seen. 2. The group treatment with periodontal curette and tetracycline HCl showed process compoed with decacified material and there was concanity seemed to be a lacunae of cementocyte. 3. The group treatment with roto bur and saline, there was no remaning plaque and partial dentinal tubule orifice could be seen but smear layer covering them. 4. The group treatment with roto bur and tetracycline HCl showed various shape and size dentinal tubule orifice could be seen. From the results, roto bur showed cleaner surfaces than treatment with periodontal curette. But still smear layer existed. Thus for regeneration of periodontal tissue, chemical treatment using tetracycline HCl should follow roto bur treatment. And it is considered that the treatment time is more important than the concentration of tetracycline HCl.

5.The Comparative Study of Alveolar Bone Level and Root Form of the Mandibular Molar on Radiographic Image and Clinical Examination.

Periodontal defects of the furcation are characterized by several inherent anatomic factors that can make successful periodontal therapy difficult and results unpredictable. The severity and rate of occurrence of periodontal disease are directly related to the location of the furcation relative to the cemento-enamel junction and anatomical form of the root by limiting the accessibility and effectiveness of the periodontal instrumentation. This study investigated the reliability and accuracy of panoramic radiograph diagnoses of the periodontal state of mandibular molars, particularly regarding the diagnosis of furcation area periodontal defects, treatment planning, and prognosis prediction. This study examined a total of 110 teeth belonging to 33 subjects (19 male, 14 female) presenting with incipient to moderate periodontitis 4-7mm pocket depth. The alveolar bone level, length and width of the root trunk, and root separation angle were measured using the panoramic radiograph and compared to the results taken directly by retracting a full-thickness flap. The results of the study are as follows: 1. Data regarding the alveolar bone level of the mandibular first molar showed that the directly taken surgical measurements resulted in 5.1+/-0.9mm that was slightly deeper than the corresponding panoramic measurement resulted in 4.8+/-0.8mm, but these differences were statistically insignificant (p>0.05). 2. The data of the directly taken surgical measurement of the mandibular second molar (5.1+/-1.1mm) was slightly deeper than the corresponding panoramic measurement (4.7+/-1.2mm), but these differences were statistically insignificant (p>0.05). 3. The measured values of the length and width of the mandibular first molar root trunks were determined to be 4.1+/-0.6mm and 7.3+/-0.9mm, respectively, while the values of the mandibular second molar root trunks were determined to be 4.6+/-1.3mm and 7.6+/-0.9mm respectively. The differences between these values were found to be statistically significant (p<0.01). 4. The measured values of the root separation angle showed that the mandibular first molars averaged 34.5+/-4.4degrees, while the mandibular second molars averaged 23.0+/-10.0degrees. The differences between these values were found to be statistically significant (p<0.01). The results of the study show that when directly taken surgical measurements of periodontal defect depth are compared to depths indirectly measured using standard panoramic radiograph, the values are slightly deeper, but that these values are statistically insignificant. These results imply that panoramic radiograph is a reliable and effective means of making clinical diagnosis of incipient to moderate periodontitis.

Authors investigated the possible role of intravascular hypercoagulable states on the etiology of Kegg-Clave-Perthes diesease. Forty-five patients with Legg-Clave-Perthes disease(31 avascular stages and 14 reossification stages) and twenty-two normal control patients were subjected to study for evaluation of coagulation and fibrinolysis system by means of the tests which included antiphospholipid antibody(APA), Protein C, Protein S and antithrombin- III (AT- III) for evaluation of coagulation system, and tissue type PIasminogen activator(tPA), Plasminogen activator inhibitor(PAI), D-dimer for fibrinolytic system. APA increased significantly in Legg-Clave-Perthes patients(p=0.016) as compared with control group, while Protein C(p=0.040) and Protein S(P=0.0001) decreased significantly in Legg-Clave- Perthes disease. AT- III increased in Legg-Clave-Perthes disease(p=0.0000). In contrast, there were no statistically significant differences in PAI, tPA, D-dimer between the Legg-Clave-Perthes disease and control group. There were no differences in all parameters between the avascular stage and reossification stage in patients with Legg-Clave-Perthes disease, Suggestive of possible inherent effect in coagulation system(hypercoagulable states) which does not change with time. Based on the above findings authors presumed that hypercoagulable state may contribute to the development of Legg-Calve-Perthes disease. However, to elucidate the etiology of Legg-Calve-Perthes disease, further extensive investigation should be followed, which include the familial tendency of hypercoagulable state, relationship with other multifactorial causes such as alcohol and steroids, and confirmation of intravascular thrombosis or decreased blood perfusion in the femoral head. Also, the significance of abnormally elevated AT-III on the disease should be answered.

8.The SPM Study on the Change of Titanium Surface Roughness following Air-powder Abrasive and Application Time of Citric Acid.

The Peri-implantitis causes inflammation of periodontal tissue and bone loss. It contaminates surface of implants. Therefore, guided bone regeneration has been used for the treatment of this disease. For the reosseointegration of the exposed surface, various mechanical and chemical methods have been used for cleaning and detoxication of implant surface. Among these methods, air-powder abrasive and oversaturated citrate are known to be most effective. However, these treatments may deform implant surface. In this research, changes of surface roughness they were examined. 10 experimental machined titanium cylinder models were fabricated to be used for control groups. Each of them was air-powder abraded for 1 minute and they were named group 1. And then, group 1 were burnished with cotton pellets soaked with citrate for 30 seconds(Group 2), 1 minute(Group 3), 3 minutes(Group 4), and 5 minutes(Group 5) burnishing were applied for grouping respectively. Each group were examined with SPM, and their surface roughness were measured and analyzed. 1. Surface roughness of titanium decreased when it was air-powder abraded for 1 minute. It was statistically significant. 2. When Air-powder abraded titanium were treated with citrate for 3 minutes, Their surface roughness was the lowest. Titanium treated for 1 minute was the second lowest and 30 seconds was the third and titanium burnished for 5 minutes was the highest. 3. Surface roughness of titanium which was treated with citrate was decreased till 3 minutes, which was statistically significant. There was no statistical significance from 30 seconds to 1 minute and from 1 minute to 3 minutes, and there was statistical significance from 30 seconds to 3 minutes. 4. Oxide layer was formed when titanium is exposed to air, and it was removed when air-powder abraded. It was made when treated with citrate. It is thought that citrate treatment is necessary after the air-powder abrasion, and 1 minute is clinically and qualitatively adequate for burnishing time of citrate.

9.Evaluation of the wear of the periodontal curet's cutting edge.

The quality of periodontal instrument cutting edge is a basic element of effective root planing procedure. Using instruments, the sharp edge is changed into blunt or beveled edge. With the blunt instrument, the periodontal treatment can't be carried into accuracy and effective. The study on the wear of periodontal curet is insufficient, there are few publications about the change of sharpness of cutting egde after using instrument and a certen reports were published on the study of scanning electron microscope(SEM) examination. In this study, to declare the number of strokes for sharpening of instruments, the changes of cutting edge is measured by the clinical methods, tactile sensitivity examination and refraction light-white line test after scaling strokes and root planing strokes. SEM test was added for defined the changes of cutting edges. The 7/8 Gracey curets that have been never used was tested. Maxillary molars which were extracted from the School of Dental Medicine, Dankook University was used. Subjected teeth had attachment loss more than 6 mm in bucco-lingual surface and sufficient calculus of a band type in cervical area. The strokes of curet were executed 3, 5, 7, 9, 11, 13 times on scaling stroke and 10, 15, 20, 25, 30, 35 times on root planing stroke. A resident has periodontal experience over 3 years carried out the clinical examinations those tactile sensitivity examination and refraction light-white line test 5 times. The case there being tactile sensitivity certenly is 2, the case being felt tactile sensitivity is 1, and the case there not being tactile sensitivity is 0. The visual examination was recorded as following. The case that refracted white line is not recognised is 2, the case that uncerten is 1, and the case that acknowledged is 0. The results were obtained as follows. 1. After scaling strokes, the tactile sensitivity was reduced after 11 strokes and disappeared in 13 strokes. 2. In tactile sensitivity after root planing procedures, sensitivity was reduced after 25 strokes and disappeared in 35 strokes. 3. In case of visual examination, the detection of refracted white line was increased after 9 strokes of scaling procedures and the accuracy of wear wasn't showed after root planing procedures. 4. In SEM, metal projection was observed on new periodontal curet cutting edge and it was disappeared after scaling procedures. 5. In SEM, the cutting edge was showed changing linear into an aspect of the surface after 5 strokes of scaling procedures and 10 strokes of root planing procedures and showed beveled edge in 11 strokes of scaling procedures, 25 strokes of root planing procedures. The results of 3-type examination indicated that the sharpening of curet should be performed after 11 strokes of scaling procedures and 25 strokes of root planing procedures.

10.Biological Effect of Platelet Rich Plasma on the Initial Attachment, Proliferation and Cellular Activity of Osteoblast.

For reconstruction of the bony defect, various artificial substitutes were developed. Among them, there has been a study of calcium phosphate coated bone substitutes for increasing attachment of osteoblasts in vivo. The purpose of this study was to evaluate the effects of serum and platelet-rich plasma (PRP) on calcium phosphate coated culture plate for the initial attachment, proliferation and activity of osteoblasts. After sampling the blood from white rats and concentrating by centrifugation, the amount of attachment of PDGF-BB and TGF-beta on the calcium phosphate coated culture plate was measured. Cultured HOS and ROS 17/2.8 cell was measured on attachment level and proliferation rate of osteoblasts. Alkaline phosphatase activity of HOS and ROS 17/2.8 cell was measured for studying on the activating rate of osteoblast. 1. Counting the amount of platelets of seperated plasma and PRP, the average number of platelets was 177,003 cell/microliter in plasma, and 1,656,062 cell/microliter in PRP, which was about 9 times as high as in plasma. 2. Amount of PDGF-BB deposited at calcium phosphate coated plate had increased by the total amount of plasma and PRP on the culture plate, whereas TGF-betahad been deposited on the plate only when treated by 50microliter of PRP(p<0.01). 3. After plating serum and PRP for 3 hours, we attached with HOS and ROS17/2.8 cell for 1 hour and 4 hours. There were no significant difference of the attachment between serum and control group, whereas there were significantly difference of the attachment between depositioning of PRP and control group. 4. After attaching plasma and PRP for 3 hours, cell number has much increased when HOS and ROS17/2.8 cell had been cultured for 48 hours(p<0.05). 5. After attaching plasma and PRP for 3 hours, concentration of alkaline-phosphatase has increased when HOS and ROS17/2.8 cell had been cultured for 48 hours(p<0.01). These results suggested that PRP affected on initial cell attachment rather than proliferation and activation of osteoblasts at calcium phosphate coated plate.
