1.A Neonatal Intussusception induced by Congenital Ileal Polyp in a two-day-old male newborn.

Intussusception is an interesting condition and is one of the most common causes of intestinal obstruction in the infant. We experienced a case of neonatal intussusception in a two-day-old male. The patient developed bloody stool, without a history of vomiting-after passing meconium, on the second day of life. Diagnosis of ileo-cecal type intussusception, which was induced by congenital polyp, was confirmed by exploratory laparatomy performed on the 4th day of life. The polyp was found at the proximal portion of the ileum, 45 cm from the ileo-cecal valve. In Korea a 40 day old patient was the youngest previously reported. We have presented this case with a review of the literature.

2.Effect of Botulinum Toxin A for Upper Limb Function in Post Stroke Spastic Hemiplegia.

OBJECTIVE: Botulinum Toxin A (BoNT-A) is one of the therapeutic methods for the spastic decrement of the upper limb which appears from the patient after stroke. Decrement of stiffness is announced from many studies, but the effect of functional improvement was few examined closely so far clearly. The purpose of this study is to evaluate the effects of BoNT-A for the improvement of upper limb function in post stroke spastic hemiplegia. METHOD: Eleven chronic stroke patients (mean age 44.4 years) were included and injected BoNT-A (Dysport®, Ipsen, UK) according to degree of stiffness of each individual. Motor assessment scale, Box and Block test, Peg board test, Ashworth Scale, manual muscle test, Visual analogue scale, and subject satisfaction were applied at the baseline, and 1 week, 1, 2 and 3 months after interventions. All subjects were also submitted to ongoing rehabilitation therapy. RESULTS: Motor assessment scale results were statistically significant improvements at 1, 2 and 3 months after intervention (p<0.05). Also, statistically significant decreases in muscle tone as determined by the Ashworth scale were found at 1 week, 1, 2 and 3 months after injections (p<0.05). CONCLUSION: Botulinum Toxin A could derive the functional improvements as well as the tone reduction of upper limb in post stroke spastic hemiplegia.
3.A Case of Periosteal New Bone Formation Associated with Behcet? Disease.

Behcet? disease is a recurring illness characterized by the triple symptom complex of aphthous stomatitis, genital ulceration, and uveitis. The disease is multisystemic disorder with involvement of skin, gastrointestinal tract, blood vessels, central nervous system, joints, and epididymis. Vascular involvement of Behcet? disease affects both arteries and veins and blood vessels of all sizes. Periosteal new bone formation is one feature of hypertrophic osteoarthropathy. Other features are clubbing and arthritis. Periosteal new bone formation on the bones of the lower extremities without other features of hypertrophic osteoarthropathy has been reported in patients with varicose veins, vitamin A intoxication, infantile cortical hyperostosis, tuberous sclerosis, congenital syphilis and chronic renal failure with hyperparathyroidism, and chronic venous insufficiency. However, periosteal new bone formation has not, to our knowledge, been described in patients with Behcet? disease yet although similar findings are occasionally noted in polyarteritis nodosa. We report a case of Behcet? disease complicated by leg swelling with periosteal new bone formation of right lower leg.

4.Risk of Rapid Lung Function Decline in Young Adults With Chronic Obstructive Pulmonary Disease: A Community-Based Prospective Cohort Study

It is unclear whether young adults with chronic obstructive pulmonary disease (COPD) are at an increased risk of rapid lung function decline. A total of 2,934 Korean adults aged 40–49 years who had consecutive lung function measurements were included. COPD was defined as pre-bronchodilator forced expiratory volume in 1 second (FEV 1 )/forced vital capacity < lower limit of normal. The risk of rapid decline in FEV 1 , defined as ≥ 60 mL/year, was assessed using multivariable logistic regression analysis. In the multivariable model, a significantly higher risk of rapid decline in FEV 1 was observed for the COPD group compared with the non-COPD group (adjusted odds ratio, 1.89; 95% confidence interval, 1.18–2.95), which was especially significant in subjects with FEV 1 less than the median value (< 110%pred) (P interaction = 0.017) and inactive physical activity (P interaction = 0.039). In conclusion, the risk of rapid FEV 1 decline was higher in young adults with COPD than in those without COPD, especially in those with FEV 1 less than the median value and inactive physical activity.
5.Clinical Characteristics and Prognostic Factors in Pediatric Chronic Rhinosinusitis with Polyps.

BACKGROUND AND OBJECTIVES: Chronic rhinosinusitis with nasal polyps is an uncommon pathology in the pediatric population and a challenging problem to otolaryngologists. In this study, we aimed to assess the clinical characteristics and postoperative results of children who underwent sinus surgery due to nasal polyps. SUBJECTS AND METHOD: We retrospectively reviewed medical records of 45 pediatric patients who had sinus surgery from 2009 to 2012. We studied the relationship between clinical parameters and postoperative results. RESULTS: Forty-five patients (18 women and 27 men with an age range of 8 to 17 years) were treated surgically in our hospital. We found statistically significant correlation between the preoperative CT scores (p=0.043), the nasal obstruction symptom scores (p=0.032) and postoperative recurrence, but not between other parameters. CONCLUSION: In this study, prognostic factors affecting the postoperative outcome were preoperative CT score and nasal obstruction symptom score.
