1.Mycosis Fungoides Treated with PUVA.

We report a case of mycosis fungoides in a 37 year-old male patient who had relatively well defined, irregular, erythematous plaques covered with acales on the buttock, lower extremities, anterior chest and back. Histopathological findings showed epidermotropism and Pautrier microabscesses in the epidermis and infiltration of atypical lymphocytes in the upper dermis. Electron microscopic findings showed mycosis cells with convoluted nuclei and prominent nucleolei He had been received PUVA therapy combined with topical and systemic corticosteroid for 4 months. Skin lesions were improved markedly after treatment with total 262 joules/crn2 and then maintained with 14 joules/cm2 in every week.

2.A Patient with Xeroderma Pigmentosum Developing Malignant Melanoma and Basal Cell Epithelioma.

Xeroderma pigmentosurn is a photosensitive hereditary disorder and especially rare in Korea. A 30-year old male patient had been suffered from walnut sized tumor and several ulcerated papules on the face, and scattered brownish macules on the sun-exposed area. Clinical features and histopathologic findings were consistent with malignant malanoma and basal cell epithelioma developing in a xeroderma pigmentosum respectively. Malignant melanoma was surgically excised.

The results of ABO blood typing in 64-year-old patient with chronic renal and heart failure were positive with anti-B, delayed and weak positive with anti-A in the slide method for the cell typing, mixed-field agglutiniation by light microscopy in the tube method for the cell typing, and also was negative in anti-A, lectin and positive in anti-H, therefore blood typing of this patient was confirmed to A3B.

4.Development of Herpes Zoster during Treatment of Bullous Pemphigoid with Immunosuppressive Drug.

A 47 year-old woman with generalized fresh tense or ruptured bullae was diagnosed as bullaus pemphigoid by histopathological and direct immunofluorescent test. We treated her with prednisolone and bethamethasone for 3 weeks. And soon after, herpes zoster developed along the right T9 dermatome. It is not precise whether true association or only incidental concomitance, but, there may be present some relatioship.

5.A Case of Acrosclerosis Transformed from Guttate Morphea.

Clinically, the transformation of morphea to scleroderma is rare. We report a case suggested acroselerosis transformed from guttate morphea. Patient was 30-year old woman who had generalized bean-sized depigmented macules except face since early childhood. About one year ago, Raynauds phenomenon, edema and sclerotic change of the both hands and feet were developed. On the laboratory findings, there were positive reaction for rheumatoid factor and antinauclear antibody test and increase of erythrocyte sedimentation rate. Histopathologically, there were even increased collagen fibers, atherosclerotic change of the vessels and atrophic sweat glands.

6.A Case of Acral Lentiginous Melanoma.

We report herein a case of acral lentiginous melanoma in a 65-year-old female. An about 2x3cm sized erythematous to brownish mass with central ulceration was located on the left heel accompanied by enlargement of the left inguinal lymph nodes. Histopathologic findings showed lentiginous proliferation of atypical melanocytes with pseudoepithliomatous hyperplasia and invasion of tumor cells from the epidermis into the dermis. Focal pigmented melanocytes were also noted. Complete surgical excision and chemotherapy with DTIC were done. Marked edematous swelling on the left lower leg was developed after 8 months and 1 month later she was expired.

7.Transthoracic Fine Needle Aspiration Cytology: Review of 213 cases.

A total of 213 fine needle aspirations from pulmonary lesions in 193 patients performed from January, 1986 to March, 1989 were analyzed. The cytologic diagnoses were unsatisfactory in 10, negative in 60, atypical in 6, suspicious in 11 and malignant in 126 cases. The cytologic types of the malignant cases were 47 squamous cell carcinomas, 40 adenocarcinomas, 10 small cell carcinomas, 6 large cell carcinomas and 10 metastatic tumors. They were verified by the histologic confirmation in 31 cases and by the clinical data in the remainder. There were 5 false-negative cases and none was false-positive, representing 96% sensitivity and 100% specificity. Primary lung cancers were accurately typed in 73% of histologically confirmed case. Cell blocks, prepared in 99 cases, were helpful in tumor typing of 11 cases.

8.The Study of Malodor Reduction after Periodontal Treatment.

Bacterial byproducts and volatile sulfur compounds(VSC) have been found to be the leading intra-oral agents, specifically, the byproducts of gram negative anaerobic bacteria have been implicated as primary factors of halitosis in patients presenting with periodontal disease. The objective of this study was to determine the correlation between periodontal treatment and the subsequent reduction in the level of halitosis. Forty-three subjects presenting with periodontal disease were examined before periodontal treatment, one week after treatment, one month after treatment, and finally, two months after treatment, using a portable sulfide monitoring Halimeter(R) to measure the VSC concentrations at the prescribed intervals. The results of the study were as follows: 1. Significant decreases in the mean VSC concentration were observed at the one week, one month, and two month post-op intervals relative to the pre-op measurement. (p<0.05) 2. Significant decreases in the mean VSC concentration were observed in subjects after completion of flap operations. Significant decreases in the mean VSC concentration were observed at the one and two month post-flap operation measurement relative to the VSC concentration at one week (p<0.05), but no significant differences between the one month and two month VSC concentrations were found. (p<0.05) 3. Significant decreases in the mean VSC concentration were observed in subjects after completion of subgingival curettage (p<0.05). Significant decreases were found between the one week and one month measurements and between the one month and two month measurements, but significant differences were not observed between the one week and two month measurements. (p<0.05) The results of this study show significant decreases in VSC concentration in test subjects after periodontal treatment. It can be inferred from the results above, that periodontal disease is a significant contributing factor of halitosis, and that treatment of periodontal disease can been an effective means of reducing VSC concentration in patients presenting with halitosis concurrent with periodontal disease.

9.Studies on the Transmissibility of Pathogenic-Organisms to Liver by Larvae of Liver Fluke and Hookworm.

In order to confirm whether the migrating larvae of parasites could carry pathogenic organisms into liver and cause hepatitis, a series of experiments has been carried out. The summary of the results is as follows: 1. Clonorchis sinensis A few of the excysted larvae of Clonorchis sinensis penetrated into the peritoneal cavity, but they could not penetrate the liver tissues. The artificially introduced Clonorchis sinensis in the tissues were all destroyed within 3-5 days. There was no manifestation of diffuse inflammatory changes due to the inoculation of the parasites, though the sampled micro-organisms, Staphylococcus aureus, were confirmed from the surrounding area. 2. Hookworm The larvae carried pathogenic organisms to liver tissues either by cutaneous or oral infection, but there was no manifestation of hepatitis due to the micro-organisms: In conclusion, it is indicated that liverfluke and hookworm may transmit pathogenic organisms to the liver during their migration.

10.The Effects of Platelet-Rich Plasma on Regeneration around Dental Implant Defects.

The current interest in periodontal tissue regeneration has lead to research in bone graft, root surface treatments, guided-tissue regeneration, and the administration of growth factors as possible means of regenerating lost periodontal tissue. Several studies have shown that a strong correlation between platelet-rich plasma and the stimulation of remodeling and remineralization of grafted bone exists, resulting in a possible increase of 15-30% in the density of bone trabeculae. The purpose of this study was to study the histopathological correlation between the use of platelet-rich plasma and a bone xenograft used in conjunction with a non-resorbable guided-tissue membrane, e-PTFE, compared to a control group with regards to bone regeneration at the implant fixture site. Implant fixtures were inserted and graft materials placed into the left femur of in the experimental group, while the control group received only implant fixtures. In the first experimental group, platelet-rich plasma and BBP xenograft were placed at the implant fixture site, and the second experimental group had platelet-rich plasma, BBP xenograft, and the e-PTFE membrane placed at the fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2, 4, and 8 weeks after implant fixture insertion. The results of the experiment are as follows: 1. The rate of osseointegration to the fixture threads was found to be greater in the first experimental group compared to the control group. 2. The histopathological findings of the second experimental group showed rapid resorption of BBP with subsequent new bone formation replacing the resorbed BBP. 3. The second experimental group showed new bone formation in the area adjacent to the fixture threads beginning two weeks after fixture implantation, with continued bone remodeling in the areas mesial and distal to the fixture. 4. Significant new bone formation and bone remodeling was observed in both experimental groups near the implant fixture sites. 5. The rate of osseointegration at the fixture threads was greater in the second experimental group compared to the first group, and the formation of new bone and trabeculae around the fixture site occurred after the fourth week in the second experimental group. The results of the experiment suggest that a greater degree of new bone formation and osseointegration can occur at the implant fixture site by utilizing platelet-rich plasma and bone xenografts, and that these effects can be accelerated and enhanced by concurrent use of a non-resorbable guided tissue membrane.
