1.A Study on Histogenesis of Glomerular Obsolescence in Renal Tuberculosis: An Appraisal for the Clarification of Low Prevalence of Hypertension.

Glomerular obsolescence is a common histologic feature in various renal diseases, of which histogenesis remains unclarified especially in renal tuberculosis, and little has been attributed for the postulation of low prevalence of hypertension in such occasion. One hundred and fifty consecutive nephrectomy specimens with renal tuberculosis under the age of 50 were histopathologically analyzed along with comparison of 18 cases of chronic pyelonephritis, 15 hydronephrosis and 10 acute traumatic kidneys. Glomerular sclerosis following tuberculous glomerulitis seemed little significant in its histogenesis except in one case, which presented epitheliold cell reaction and crescent formation. Wrinkling and thickening of glomerular basement membrane and subsequent simplification of tufts with mesangiolysis(Stage I and II) represented the major alternative glomerulopathy in nodular renal tuberculosis, whereas in confluent ulcero-cavitary lesion the development of hyalin-knot within the Bowman's space and collapse of the tufts (Stage III and IV) comprised the bulk, aside from frequent intraglomerular shunt formation between afferent and efferent arterioles mostly in autonephrectomized cases, Arteriolosclerosis was found in 17.2 % among nodular type, while 26.7% in ulcero-cavitary, variety, partly responsible for the histogenesis of glomerular obsolescence. Those cases with moderate arterio and arteriolar sclerosis showed a high tendency of transformation into stage III-IV obsolescence especially in ulcero-cavitary tuberculosis. Relative prevalence rate of systolic hypertension in ulcerocavitary tuberculosis was 5.8%, two times higher than nodular type, but in only one case the post-operative blood pressure returned to normal range. With the above findings it is assumed that glomerular oboslescence of so-called hypertensive glomerulopathy is a subtype of ischemic nature, and induced not only by vascular is chemic changes but also compromised through the intra-renal tubular obstruction and intraglomerular capillary shunt formaion may attribute to the development of secondary hypertension, though rare, in renal tuberculosis associated with arteriosclerot ic changes as well.

2.A Case of Epidermal Nevus Syndrome Associated with Urogenital Abnormalities.

The epidermal nevus syndrome is a disorder characterized by epidermal nevi and associated neurologic, skeletal, and other abnormalities. A 15-year-old female patient presented with extensive, bilateral systematized, verrucous plaques, involving the face, trunk, both extremities, and anogenital area. The onset of the lesions were at the age of 2 months. Abdominal CT showed the absence of the left kidney, teratoma of the right ovary and left ovarian cyst. This is a case of epidermal nevus syndrome, which was composed of extensive epidermal nevi, congenital solitary kidney, left ovarian cyst and right ovarian teratoma.

3.Clinical Observation on Renal Tumors.

A clinical observation was made on the patients with renal tumors who had been admitted and treated in the Department of Urology, Seoul National University Hospital during the period from Jan. 1965 to Sept. 1973. The following results were obtained. 1. During these period, a total of 2, 886 patients were admitted and 44 cases were confirmed as renal timor, giving a rate of 1.53%. 2. Of 44 cases, 28 cases were male and 16 cases were female patient. 3. Of 44 cases, renal cell carcinoma was 27(61%), transitional cell carcinoma of renal pelvis 9(20%), Wilms' tumor 7(16%) and hamartoma was 1 case(2.2%). 4. Chief complaints were as follows: hematuria in 48%, loin pain in 34%, abdominal mass in 24% and weight loss with fatigue observed in 18% of patients. 5. Raised ESR in 70%, anemia in 49%, hypertension in 30% and abnormal liver function test was found in 14% of patients as a systemic effects of renal tumors.

A statistical observation was made on 204 cases of congenital anomalies of the genitourinary tract in patients who had been admitted to Department of Urology, Seoul National University Hospital during the period January 1965 through December 1973 and the following results were obtained. 1. During the period, 204 cases of congenital anomalies of the genitourinary tract were found among 2.910 cases hospitalized, giving a rate of 7.O%. 2. The cases of congenital genitourinary tract anomaly had been increased annually, from 3.2% in 1965 to 9.8% in 1973. 3. The most cases of congenital anomaly of the G-U tract were found in the age group below 19 years old (66.7%). 4. The most vulnerable organ for the G-U tract anomalies were urethra (38. 9%), testis (25.9%) and kidney (11.8%). 5. Hypospadias were observed in 69 cases (33.8%), cryptorchidism in 51 cases (25.O%) and nephroptosis in 13 cases (6.4%). 6. 12 cases of combined G-U tract anomaly were also found.

6.Ectopic Ureter with Cystic Terminal Dilatation.

An unusual case of incomplete double ureter on right side and complete double ureter associated with single ectopic ureteral opening on left side was presented. Distal end of the ectopic ureter was shown cystic dilatation of hen egg size and opened into the vaginal fornix. Diagnosis was made by retrograde ureterography, double dose pyelography and renal angiography and she was treated by ureteroheminephrectomy.

7.A Neonatal Intussusception induced by Congenital Ileal Polyp in a two-day-old male newborn.

Intussusception is an interesting condition and is one of the most common causes of intestinal obstruction in the infant. We experienced a case of neonatal intussusception in a two-day-old male. The patient developed bloody stool, without a history of vomiting-after passing meconium, on the second day of life. Diagnosis of ileo-cecal type intussusception, which was induced by congenital polyp, was confirmed by exploratory laparatomy performed on the 4th day of life. The polyp was found at the proximal portion of the ileum, 45 cm from the ileo-cecal valve. In Korea a 40 day old patient was the youngest previously reported. We have presented this case with a review of the literature.

8.Clinical Studies of Abdominal Tumors in Infants and Chi1dren: Ten Year Review.

We have clinically observed 39 patients with abdominal tumors who were admitted in Severance Hospital during the past 10 years from April 1960 to March 1970. Unfortunately follow-up study couldn't be made. The following results were obtained. 1) Of the total number observed, 25 patients were male and 14 patients female. 2) The majority of the patients, 34 (87%) were under 6 years of age and 20 (51%) were between 1 to 3 years of age. The youngest patient was a 3-month-old female with Wilms' tumor. 3) In our series, Wilms' tumor (15 cases, 38.5%) was the most common and neuroblastoma (8 cases) was next most common tumor in the abdominal cavity. In addition we found another 4 cases of neuroblastoma in other sites. We observed hepatoma (8cases), ovarian tumor (3cases), lymphosarcoma (2cases), mesenteric cyst (one case) and adenoma of the left adrenal gland (one case) in that order of frequency. 4) In 15patients with Wilms' tumor, hypertension was observed in 8 patients and hematuria in 4 patients. Compared with Wilms' tumor, no hematuria was noted in the cases with neuroblastoma and hypertension occurred in 3 of these. 5) On abdominal X-ray, calcification was present in 4 cases with neuroblastoma (50%) and was present in only one of 15 patients with Wilms' tumor. 6) Comparing the sites of metastasis between Wilms' tumor and neuroblastoma, 4 of 15 patients with Wilms' tumor had lung metastasis and 4 of 8 patients of neuroblastoma had bony metastasis. Liver metastasis was found at autopsy in one case of neuroblastoma. 7) Three cases of hepatoblastoma were under 2 years of age and three cases of hepatocarcinoma were over 12 years of age. 8) Three cases of ovarian tumor were benign cystic teratoma. Tortion of the ovarian pedicle occurred in one of 3 patients with severe low abdominal pain. All patients were discharged in good condition after salphingo-oophorectomy. 9) One case of lymphosarcoma of the ileum was associated with intussusception. 10) A one year o1d female with adenoma of the left adrenal gland had typical Cushing's syndrome.

9.Two Cases of Spontaneous Rupture of the Kidney.

Two cases of spontaneous kidney rupture with literatural review was presented at first in Korea. 1) 56 years male patient and 25 years male patient admitted to our department of Urology through Emergency Room after onset of 2 and 1 day. after treatment at local clinic. 2) Chief complaints were one side flank pain which was followed by total gross hematuria. 3) Preoperative diagnoses were kidney rupture due to necrotic pelvic tumor in 56 years male and hypernephroma in 25 years male patient. 4) Operation was procedured at 8 days and 4 days after admission respectively. 5) The operative procedure were nephrectomy in both cases. 6) The pathological diagnoses were pelvis rupture due to renal stone and parenchymal rupture due to renal infarction in each case.

10.Results of Subcutaneous Lengthening with Smooth CD Rod in Young Children with Scoliosis before Growth Spurt.

For young children with scoliosis before growth spurt, suhcutaneous lengthening without fusion was designed by Harrington and modified by Moe and Luque. However, many problems including spontaneous fusion, rod breakage, and hook disloclgement have been ohserved. CotrelDubousset(CD) instrumentation was sometimes used, but it usually resulted in failure due to soft tissue adhesion around the rough surface of ordinary CD rod. We tried to use the smooth CD rod, transvcrse-pedicle clawing on the upper part, and pedicle screw inscrtion on upper and lower part of the curve to reduce the hardware failures. Among 8 patients in whom suhcutaneous lengthening with smooth CD rod was carried out hetween October l992 and Suly 1996. 4 cases perfomed with final spinal fusion were analysed. There were I central core disease, 1 multicore disease and 2 idiopathic scoliosis(infantile and juvenile type). Mean age at the first operation was l0.0(8.8-11.8) years, and the Risser sign was all grade 0 except one with grade 1. Suhcutaneous lengthening was performed every 5 or 6 months Mean lengthening duration was 22(9-39) months and mean age at spinal fusion was 11.7(9.6-13.8) years. Mean Cobb angle decreased from 7ldegrees (55degrees-88degrees) at preoperative stage to 32 (10degrees-59degrees) at the last follow-up. There were 5 complications during 21 operations, and three hardware failures comprised 2 hook dislodgcment and 1 screw pull-out. Crankshaft phenomenon happened in I case who had had a posterior fusion in young age(9.6 years) due to laminar fracture. The suhcutaneous lengthening with smooth CD rod can he another option of treatment for young children with severe scoliosis. prescrving the powth potential of involved vertebrae with few complications.
