2.Clinical and Mycological Observation of Tinea Versicolor.

Fifty-seven patients with Tinea versicolor were studied clinically and mycologically, from May 1980 to August 1981, at the Department of Dermatology, Korea University Hospital. Also observed were twenty-four Tinea versicolor patients, who were factory workers frorn Wolsan area, and whose diagnoses were confirmed by clinical and mycological examination. The results obtained were as follows: 1. During the study period, out of a total of 5475 out patients, there were 57 cases of Tinea versicolor for an average of 1.04 percent. In Wolsan area, out of a total of 1835, there were 24 cases of Tinea versicolor(1.3%). 2. The average age of all patients at visit was 27.9 years, the oldest patient was 69 year-old and the youngest patient was 13 year-old. The onset of disease was observed most commonly in June, July and August, these patients comprising 55.6 percent of the whole series, 3. The male to female ratio was 7:1, excluding workers from Wolsan area. 4. Distribution of lesions were anterior chest(21.4%), back(18.0%), alxlomen (17%), axilla(16.5%), neck(12.6%), pubic area(4.4%), upper extremites (4.4%), hip(3.4%), and lower extremites(2.4%). The incidence of hyperpigmented lesions was 71.8% and of hypopigmented lesions was 28.2%.

3.A Case of Subcorneal Pustular Dermatosis.

Subcorneal pustular dermatosis is a chronic, benign, relapsing pustular eruptions characterized histologically by subcorneal pustules which contain abundant neutrophils. We report a case of subcorneal pustular dermatosis occured in a 71-year-old male with typical climcal lesions. Biopsy of a pustule showed a subcorneal cavity with numerous neutrophils, which were also scattered throughout the Malpighian layer below. The result of imrnunofluorescent studies was negative. His eruption is controlled by DDS (25-100mg daily) but the eruptions recur quickly on sessation of the drug.

4.A Case of Malignant Angioendothelioma.

Malignant angioendothelioma is an uncommon malignant vascular tumor featuring endothelial cell, and it occurs most frequently in sixth and seventh decade. Most lesions occur over the face and scalp, and may be solitary or multiple, The lesion usally appears as a localized dark red to purplish nodules or plapue with small satellite nodules. Histopatholgic finding shows a nodule eomposed of irregular vascular spaces lined by plump endothelial cells stroma. We present a case of malignant angioendo thelioma on a 42 year-old woman. The patient had purplish nodule with satellite lesions on the right cheek. This case was confirmed by clinical picture, histologic findings and electron microscopic examination.

5.A Case of Sarecoid Dactylitis.

We report a case of sarcoid dactylitis in which there have been significant bone changes. A 46 year-old female presented with digital erythematous clubbing-like changes and sausage-like swellings of middle phalanges of both hands for 3 months. She had also several freely movable nontender subcutaneous nodules on her trunk and extremities for 3 years. There was a mild leukopenia and decreased cell-mediated immunity. The chest X-ray revealed bilateral hilar lymphadenopathy with increased interstitial lung markings. The X-ray of the hands and feet revealed paraarticular osteolytic lesions and altered trabecular patterns. Histopathologically there were several well-circumscribed granulomas without any caseating necrosis. We treated her with oral prednisone and hydroxychloroquine with some improvement.

BACKGROUND: Since the bioavailability of itraconazole capsule is influenced by patients gastric acidity, it results in treatment failure due to its low dissolution and subsequent low absorption when administered in fasting. Itraconazole Melt-Extrusion tablet has been lately developed in order to improve its dissolution profile. It is the first clinical study to evaluate the efficacy and safety of itraconazole Melt-Extrusion tablet in Korea. OBJECTIVE: This study was conducted to evaluate the efficacy and safety of itraconazole melt-extrusion tablet 400mg daily for 1 week(pulse therapy) for hyperkeratotic type of tinea pedis and manus. METHODS: A clinical and mycological investigation was made of 812 outpatients with hyperkeratotic type of tinea pedis and/or tinea manus who had visited at 52 general hospitals under the lead of the Korean Dermatological Association from June to December, 1998. Patients confirmed by clinically and microscopically as hyperkeratotic type of tinea pedis and/or tinea manus were administered 2 tablets twice a day for one week and followed up for 8 weeks from the start of the medication. RESULTS: The results were summarized as follows; 1. Clinical symptoms of hyperkeratotic type of tinea pedis and/or tinea mauns were significantly improved at the end of study, week 8(p<0.001). 2. Clinical response rate, defined as more than 50% decrease of the sum of the clinical symptom scores, was 79.3%(512/646). 3. Mycological cure rate, dafined as both culture and KOH negative at week 8, was 78.2%(244 /312). 4. 40(5.5%) patients, of the 727 patients evaluable for drug safety evaluation, were reported to have adverse event. CONCLUSION: Itraconazole Melt-Extrusion tablet 400mg/day for 1 week (pulse therapy) is effective and safe in the treatment of hyperkeratotic type of tinea pedis and/or tinea manus.
