1.Results of Culture Obtained from the Tip of Used Foley Catheter.

Positive bacterial culture was obtained in 33 out of 70 cultures of the tip of used Foley catheter, performed during the period of 20 months from March l st, 1977 to October 31st, 1978 and the following results were obtained. l. Almost twice much positive bacterial culture was obtained from the culture of used catheter tip (45. 7%) compared with the urine culture taken from the same patient at the same time (24. 3%). 2. Positive culture rate of the catheter tip in the group who was catheterized at operating room (32.7%) was much lower than in the group catheterized at ward (76.2%). Very few positive culture was obtained from the catheter tip used in a patient who had normal urine findings previous to catheterization if this indwelling catheter removed within 72 hours. 4. The most common organism on the culture of the catheter tip was proteus sp. (9 cases), and less commonly, pseudomonas (3 cases), enterococci (3 cases), yeast (3 cases), E. coli (2 cases), klebsiella (2 cases), alcaligenes (2 cases), enterobact.(2 cases) and citrobact.(2 cases) were found.

2.Comparison of Chlamydia Trachomatis Antigen Detection Rate between Endourethral Swab and Urine Examination.

Chlamydia trachomatis is known as a major causative microorganism in non-gonococcal urethritis(NGU) in men. We examined two kinds of specimens simultaneously, endourethral swab and urine, in each patient to determine the reliability of these two different specimens for the detection of the Chlamydia trachomatis antigen in male NGU patients using ELISA method. Tota1 273 patients entered this study were divided into two groups according to sampling order of urine and endourethral swab. In group A(141 patients), we took endourethral swab first, then first portion of voided urine was caught. In group B(132 patients), endourethral swab was performed after urine sampling. Twenty five out of 273 patients(9.2%) showed Chlamydia trachomatis antigen positive in endourethral swab sample and 1 out of 273 patients(0.4%) was positive in urine sample. There was no significant difference of antigen positive rate of endourethral swab examination between group A and group B. Our data suggest that examination of endourethral swab is more reliable method for the detection of the Chlamydia trachomatis antigen than urine examination in male NGU patient.

3.Causative organisms and antibiotic sensitivity of Urinary Tract Infection in 1979.

419 bacterial strains in 395 specimens of urine(significant bacteriuria) were found during the period of 12 months from January 1,1979 to December 31,1979, and the following results were obtained: 1. The ratio of male to female was 1.1:1(male;210,female;185) and the most common age group was 6th decade in male and 3rd decade in female. 2. 85.4% was gram negative bacilli group,14.1 percent was Gram positive cocci group and 0.5% was fungus in 419 isolated strains the isolated organisms were E.coli(35.1%), Serratia(11.2%), Proteus(11.2%), Psuedomonas(9.8%), Streptococcus(7.2%), Staphylococcus(6.0%), Klebsiella(5.7%), alcaligencs(3.3%), Citrobacter(2.6%), Enterococcus(1.0%), fungus(0.5%), acinebacter(0.2%) and flavobacter(0.2%) . In male patient E.coli(17.4%), Serratia(14.3%), Proteus(17.0%), Psuedomonas(15.2%), Streptococcus(7.2%) were the common causative organisms and in the female patients E.coli(57.0%) was the main invader. 3. In 183 patients of urologic department, the indwelling catheter and other instrumentation was the most common cause of bacteriuria in male and the uncomplicated infection was the most common one in female./ the most common causative organism of bacteriuria in the cases of uncomplicated infection obstruction and urinary stone is E.coli but Pseudomonas, Proteus and Serratia a well as E.coli were common in indwelling catheter and instrumentation group. 4. The Gram negative bacilli were sensitive to amikacin(80.5%), colimycin(70.7%), tobramycin(64.4%), and gentamicin(56.0%) and the Gram positive cocci were sensitive to carbenicillin(76.3%) tobramycin(46.7%) amikacin(43.5%) penicillin(41.4%) TAO(40.8%) and Gentamicin(40.4%).

4.A Case of Pheochromocytoma of the Bladder.

A 44-year-old female patient was admitted to Korea University Hospital with a history of frequency, pain and headache after urination for 2 years. The past history was not remarkable and she never had trouble with hypertension, Urinalysis revealed microscopic hematuria and urinary VMA level was 9.33mg it 24 hour urine Cystoscopy showed the presence of ovoid tumor mass measuring approximately 4cm in greatest diameter. A biopsy specimen, obtained by TUR showed histological evidence of pheochromocytoma. She underwent segmenal resection of bladder at the 14th day of admission and the postoperative course was uneventful.

5.A Case of Vesicoappendiceal Fistula.

A 32 year-old male patient was admitted to Korea University Hospital on July 30. 1980 with complaints of terminal hematuria, pain on urination and frequent urination for 3 months duration. Four months prior to admission this patient had an episode of fever and pain in the right lower quadrant of the abdomen. Urinalysis revealed microscopic hematuria and pyuria. Urine culture yielded E. coli. Cystoscopy showed the presence of small papular masses and vesicles over the upper lateral area to the right ureteral orifice. On operation a fistula was found between the bladder and appendix with heavy adhesion. In the area of adhesion, fecal material was noted, Patient underwent debridement of adhesive tissue, partial cystectomy and appendectomy and the postoperative course was uneventful.
