4.A practical guide for multivariate analysis of dichotomous outcomes.

A dichotomous (2-category) outcome variable is often encountered in biomedical research, and Multiple Logistic Regression is often deployed for the analysis of such data. As Logistic Regression estimates the Odds Ratio (OR) as an effect measure, it is only suitable for case-control studies. For cross-sectional and time-to-event studies, the Prevalence Ratio and Cumulative Incidence Ratio can be estimated and easily interpreted. The logistic regression will produce the OR which is difficult to interpret in these studies. In this report, we reviewed 3 alternative multivariate statistical models to replace Logistic Regression for the analysis of data from cross-sectional and time-to-event studies, viz, Modified Cox Proportional Hazard Regression Model, Log-Binomial Regression Model and Poisson Regression Model incorporating the Robust Sandwich Variance. Although none of the models is without flaws, we conclude the last model is the most viable. A numeric example is given to compare the statistical results obtained from all 4 models.

5.A systematic approach for the diagnosis and treatment of idiopathic peptic ulcers.

An idiopathic peptic ulcer is defined as an ulcer with unknown cause or an ulcer that appears to arise spontaneously. The first step in treatment is to exclude common possible causes, including Helicobacter pylori infection, infection with other pathogens, ulcerogenic drugs, and uncommon diseases with upper gastrointestinal manifestations. When all known causes are excluded, a diagnosis of idiopathic peptic ulcer can be made. A patient whose peptic ulcer is idiopathic may have a higher risk for complicated ulcer disease, a poorer response to gastric acid suppressants, and a higher recurrence rate after treatment. Risk factors associated with this disease may include genetic predisposition, older age, chronic mesenteric ischemia, smoking, concomitant diseases, a higher American Society of Anesthesiologists score, and higher stress. Therefore, the diagnosis and management of emerging disease should systematically explore all known causes and treat underlying disease, while including regular endoscopic surveillance to confirm ulcer healing and the use of proton-pump inhibitors on a case-by-case basis.

6.Ureteral Injuries Acquired During Abdominal and Pelvic Surgeries.

For five years of period, 1956-1951, twelve cases of ureteral injuries acquired during abdominal and pelvic surgery were experienced as follows . Original surgical procedures which caused the injuries were radical total hysterectomy, 9 cases; simple total hysterectomy, 1 case; abdominal procedure for intra-abdominal hemorrhage, 1 case; and repair for vesicovaginal fistula, 1 case. Of twelve pervious procedures, 9 cases were for uterine cervix cancer, 1for uterine myoma, 1 for vesico-vaginal fistula and anther for rupture of a horse-shoe kidney. Injuries found were uretero-vaginal fistula, 4; bilateral complete severance, 1; unilateral complete severance. 2; simple ligation. 5; and ureterocutaneous fistula, 1. Lateralization of injuries was bilateral, 3; unilateral, 9; of which 7 were on the right and 2 on the left. Urological repairing procedures included 3 ureteroneocytostomies, 1 ureteral anastomosis to tube made from bladder flap.,1 end-to-end oblique anastomosis, 1 end-to-end transverse anastomosis, 1 deligation and 1 nephrectomy. It will be wise and advisable that a complete urological work-up and bilateral catheterization should be performed prior to abdominal and gynecological procedures to prevent accidental injury to the ureter.

8.An unusual collision tumour masquerading as a basal cell carcinoma on the nose.

When two or more cutaneous tumours coexist in a single lesion, it is known as a cutaneous collision or contiguous tumour. Various combinations of collisions have been described. Collision tumours often have misleading clinical and histological presentations, and can be a diagnostic challenge. Chondroid syringomas are mixed cutaneous tumours of dual origin, and like collision tumours, are often confused with the more commonly seen cutaneous lesions. As chondroid syringomas are rare, their involvement in collision tumours is an even more peculiar occurrence. We report an unusual case of a cutaneous collision tumour on the nose involving an intradermal naevus and chondroid syringoma. To the best of our knowledge, this is the first time such a combination is reported.

9.Staphylococcal thoracic aortitis complicated by aortic dissection

BACKGROUND: The diagnosis of aortitis is often delayed as symptoms are largely non-specific. We report a case of Staphylococcal thoracic aortitis in a 73-year-old Chinese woman complicated by aortic dissection. METHODS: The patient presented with pyrexia of unknown origin, and a contrast enhanced computed tomography aortogram revealed a large thrombus at the anterior aspect of the ascending aorta with two large ulcerations as a result of a chronic type A aortic dissection. A hemiarch replacement with a 28 mm Gleweave Vascutek graft was performed with resuspension of aortic valve commisures. Aortic thrombus cultures were positive for coagulase negative Staphylococcus aureaus, and histology showed chronic dissection of the aorta. RESULTS: The patient was treated with intravenous cefazolin for a 6-week duration and made good progress. CONCLUSIONS: This case highlights Staphylococcal infective aortitis complicated by dissection presenting as fever of unknown origin. Timely diagnosis is essential as progression to catastrophic rupture may occur.

Interstitial cystitis/bladder pain syndrome (IC/BPS), which is characterized by bladder pain and irritative voiding symptoms, is a frustrating disease without effective treatment. The cause is still largely not understood, although urothelium ischemia/hypoxia, apoptosis, denudation, and infiltration of inflammatory cells are common histopathological findings. The current uncertainty regarding the etiology and pathology of IC/BPS has a negative impact on its timely and successful treatment; therefore, the development of new treatment modalities is urgently needed. Herein, we present advances in our knowledge on this topic and review the potential application of regenerative medicine for the treatment of IC/BPS. This article provides information on the basic characteristics and clinical evidence of stem cells, platelet-rich plasma (PRP), and low-energy shock waves (LESWs) based on a literature review with a search strategy for articles related to IC/BPS, stem cells, PRP, and LESW published in MEDLINE and PubMed. Stem cells, PRP, and LESW, which modulate inflammatory processes and promote tissue repair, have been proven to improve bladder regeneration, relieve bladder pain, inhibit bladder inflammation, and increase bladder capacity in some preclinical studies. However, clinical studies are still in their infancy. Based on the mechanisms of action of stem cells, PRP, and LESW documented in many preclinical studies, the potential applications of regenerative medicine for the treatment of IC/BPS is an emerging frontier of interest. However, solid evidence from clinical studies remains to be obtained.
