1.Risk factors for portal venous thrombosis under anticoagulation therapy after operation of portal hypertension

Objective To investigate the risk factors for portal vein thrombosis (PVT)under anticoagulation therapy after surgery in patients with portal hypertension.Methods We made a retrospective analysis of clinical data of 96 portal hypertension patients for surgical treatment at our hospital.All the patients with postoperative PVT or without were divided into two groups.Risk factors that may predict PVT were analyzed.Results PVT developed in 41 (40.08%)of 96 patients after surgery.Risk factors such as sex,age,Child-Pugh classification,type of operation,portal pressure and the pressure difference before and after surgery,preoperative prothrombin time, preoperative platelet count,spleen index,and portal vein diameter were not predictors of PVT.However,splenic vein diameter was an independent risk factor for PVT (P = 0.036);postoperative PVT tended to develop when the splenic vein diameter was larger than 1 1 mm.Conclusion Preoperative color Doppler testing of splenic vein diameter can predict PVT after surgery in patients with portal hypertension.

Objective To explore the correlation between the gene polymorphism of glucocorticoid receptor intron 4 G1666T (GR G1666T) and intracerebral hemorrhage(ICH ).Methods Frequencies of genotypes and alleles of GR G1666T in 48 pations with ICH(ICH group),48 ICH patients combining essential hypertension(EH) (ICH+EH group),46 EH patients (EH group) and 71 healthy people (NC group) were analyzed by a polymerase chain reaction for restriction fragment length polymorphism (PCR-RFLP).The gender,body mass index(BMI),blood pressure(BP),the levels of plasma angiotensinogen,renin,blood glucoseand and blood lipid were compared among the various genotype of GR G1666T in ICH patients.Results There was no obviously difference of G1666T genotype and allele frequencie among the 4 groups.However,the G allele frequence of females in groups ICH,ICH+EH and EH were significantly higher than that in NC group(all P
3.Role of Angiotensin Ⅱ Receptors(ATRs) in Metabolism of Myocardial Collagen

Objective To investigate the role of angiotensin Ⅱ receptors in metabolism of m yocardial collagen. Methods The collagen volume fraction(CVF), perivascular circumferential area(PVCA), hydroproline concentration(HC), ratio o f collagen type Ⅰto type Ⅲ(Ⅰ/Ⅲ),angiotensin Ⅱ(AngⅡ) content and the maximal binding capaci ty (Bmax) of ATRs were studied. The methods of radioimmunoassay(RIA),biochemistry assay, and pathological examin ation were used. Results Ventricular myocardial CVF, PVCA, HC, Ⅰ/Ⅲ, AngⅡ and B max of ATRs in WKY rats were significantly higher than those in sham -operation(SO)(P<0.05 or P<0.01),suggesting the collagen was abnormal ly accumulated in rats heart muscles. AngⅡ was positivily correlated with HC(r1=0.9045, P<0.01). After treament of Irbesartan, All above parameter were reduced significantly(P<0.05 or P<0.01), abnormally accumulated collagen was disap peared. While after treament of CGP42112A,an ATR2 antagonist, Bmax of ATRs was markedly decreased(P<0 .01), but CVF, PVCA, HC,Ⅰ/Ⅲ, AngⅡ showed little changes(P >0.05). Conclusion Obvious hyperplasia of myocardial collagens and remode ling of collagen network occurred after pressure overload. Local produced AngⅡ involves in the process,it's effects is mainly mediated by angiotensin Ⅱ 1 type receptor(ATR1)

To compare the early-phase secretion between islet α cells and β cells in subjects with normal glucose tolerance and to explore the influencing genetic factors on the function of islet cells.40 subjects with normal glucose tolerance and family history of diabetes ( FH+ ) and 55 healthy volunteers without family diabetes history ( FH ) were collected.Fasting and L-arginine stimulating insulin,glucagon,and tasting free fatty acid,as well as other indicators were determined in all subjects.Insulin resistance was evaluated by homeostasis model assessment for insulin resistance.After adjnsting for sex,age,and body mass index,the insulin secretion peak of the two groups reached both at two min,and began to decline at four min,the peak multiple of FH+group was significantly less than that of FH-group (7.29 vs 8.88,P<0.05) ; glucagon secretion peak of both groups reached at two min and began to decline by four min.Fasting glucagon and peak multiple were not significantly different (P>0.05) ; The ratio of fasting insulin to fasting glucagon of the two groups was without significantly difference ( P>0.05 ).Under diabetes genetic background,the function of β cells decreases even in subjects with normal glucose tolerance.
6.Molecular mechanism of familial hypertriglyceridemia caused by lipoprotein lipase gene mutation (C310R/E396V)

Objective The purpose of this study is to investigate the molecular mechanisms of p.C310R(c.T928C) and p.E396V(c.A1187T) lipoprotein lipase(LPL) gene mutations in vitro, which may help to construct the spectrum of LPL gene mutations and phenotype. It also can provide accurate early diagnosis for high-risk population of familial hypertriglyceridemia and provide the basis for the development of gene targeted therapy. Methods Genomic DNA was extracted from proband′s family members′ peripheral blood cells and screened by whole-exome sequencing to verify candidate gene variations. PCR products were afterwards directly sequenced again to confirm corresponding LPL variants. At the cellular level, lentiviruses containing LPL mutations were constructed and then transfected into COS-1 cells. Functional significance of the mutants was corroborated by analyzing LPL activity and mass in the cell medium and lysates via ELISA and enzyme-fluorescent method. mRNA was assayed by RT-PCR to confirm the effect on gene transcription. Results DNA sequence analysis revealed that the proband was a heterozygote for a novel c.T928C mutation in exon 6 of LPL gene, while his nephew was a compound heterozygote for the c.T928C mutation in exon 6 and a novel c.A1187T mutation in exon 8. In vitro studies, these two mutations can cause decreased activity and mass of extracellular LPL(P<0.05). Moreover, further investigation indicated that LPL C310R mutation tremendously affected post-transcriptional modification of LPL gene, whereas LPL E396V mutation dampened intracellular LPL trafficking. Conclusion Both the mutations are pathogenic by reducing the activity and mass of LPL in the plasma, which affected normal metabolism of triglycerides.
7.The change of miR-146a in carcinoma tissues and peripheral blood of patients with papillary thyroid carcinoma

Objective To study the change of miR-146a in the development and progression of papillary thyroid carcinoma(PTC).Methods Real-time fluorescence quantitative polymerase chain reaction was used to detect the change of miR-146a in tumor tissues of 43 cases of PTC and 36 cases of nodular goiter,also in peripheral blood of 32 cases of PTC and 23 cases of nodular goiter.The correlation of miR-146a change and the clinicopathological parameters of patients with PTC was analyzed.Results ① The relative content of miR-146a in the 43 patients with PTC and 36 patients with nodular goiter was 0.0280 ± 0.0131 and 0.0212 ± 0.0111 respectively.The difference had statistical significance (P < 0.05).miR-146a in PTC tissues was significantly correlated to clinicopathological parameters such as lymph node metastasis,tumor stage (P < 0.05),and it was unrelated to age or tumor size(P >0.05).② The relative content of miR-146a in peripheral blood of 32 patients with PTC and 23 patients with nodular goiter was 0.0891 ±0.0419 and 0.0922 ±0.0456 respectively.The difference had no statistical significance(P > 0.05).There was no significant correlation between miR-146a in peripheral blood of patients with PTC and age,tumor stage,lymph node metastasis,or tumor size of patients (P > 0.05).Conclusions MiR-146a expression is upregulated in patients with PTC,and there is a significant correlation between miR-146a content and clinicopathological parameters such as lymph node metastasis,tumor stage,especially in advanced stage.It suggests that miR-146a may play a role in carcinogenesis and development of PTC.
8.The reason, diagnosis and treatment of postoperative early cardiac arrhythmia in 500 patients who received orthotopic liver transplantation

ObjectiveTo summarize and investigate the incidence,reason,diagnosis and treatment of postoperative early cardiac arrhythmia after orthotopic liver transplantation(OLT).Method A retrospective study was made for the incidence,dignosis and treatment of cardiac arrhythmia (two weeks after OLT) following OLT from June 2004 to January 2012 in the Beijing You-An Hospital.ResultsIn 500 patients who received OLT,Cardiac arrhythmia developed in 82 patients (16.4%).Among these cardiac arrhythmia,35(7.0 %) were sick sinus syndrome (including severe sinus bradycardia ),18 (3.6 % ) were paroxysmal supraventricular tachycardia,21(4.2 %) were atrial fibrillation,8(1.6 %,including 2 patients with torsades de pointes) were ventricular tachycardia and 4 (0.8 % )were cardiac arrest.Mortality rate after OLT relate to cardiac arrhythmia was 0.4% (2 patients).Cardiac arrhythmia was mainly correlated with four factors:(1)whether patient had heart disease before OLT or not(x2 =15.82,P<0.01),(2)Prolonged QT interval in patients with end-stage liver disease before OLT(x2 =11.00,P<0.01).ConclusionsCardiac arrhythmia was common complication after OLT,and it can lead to death of recipients.Careful evaluation to recipients before OLT,controlling fluid load after OLT,keeping the balance of the electrolyte,acidity and alkalescence,giving intensive monitor to patients with heart disease before OLT and prolonged QT interval are the key factor to reduce incidence and mortality of cardiac arrhythmia.Application of medication and cardiac pacemaker can prevent cardiovascular accident after OLT.
9.Population genetic quality analysis of outbred KM mice from Shanghai Seed Center

Objective To analyze and evaluate the population genetic quality of outbred KM mice from National Rodent Seed Center ( Shanghai ) .Methods A total of 30 outbred KM mice were randomly chosen.The genetic characteristics of the population were determined by PCR and STR scanning using 30 selected microsatellite loci. Popgen1.32 software was used to process the data.Results Thirty microsatellite loci shared 123 alleles in the KM mouse population.The average effective allele number and the average heterozygosity were 2.3989 and 0.5342, respectively.The average polymorphism information content ( PIC ) was 0.4735.Conclusions The outbred KM mouse population of Shanghai Seed Center has genetic stability and genetic diversity, and is satisfied with the genetic characteristics of closed colony laboratory animal.
10.The effect of web-based histologic laboratories for general practical students

ObjectiveTo assess the satisfaction of general practical students' perception with the two modes of histologic laboratory instruction,and obtain the advantages and disadvantages of each mode.MethodsWeb-based database was established and in the questionnaires were delivered to the general practical students ( n=193 ).Results75% of students agreed that the case study component of the webbased laboratory instruction was arranged in order.82% of students identified with the detailed word explanation attached to every image presented.ConclusionStudents support the progression of web-based histologic laboratory instruction,but it is difficult to replace traditional microscope with web-based database in histologic laboratory instruction.
