1.Study of Acupuncture Zhongzhu(SJ3) and Waiguan(SJ5) by Functional MRI(fMRI)

Objective To observe the distribution of the activated cerebral regions after acupuncture stimulation of Zhongzhu(SJ3) and Waiguan(SJ5) by functional magnetic resonance imaging(fMRI) technique.Methods A total of 8 cases of volunteer healthy subjects were recruited for this study.Before and during manipulation of the needle,cerebral images were sampled separately by fMRI scanner and analyzed by AFNI software.Results During manipulation of the needle inserted in the unilateral Zhongzhu(SJ3) and Wauguan(SJ5),the highly-activated cerebral structures were cingulated gyrus,temporal gyrus,cerebellum,occipital gyrus,etc.Conclusion The effect of acupuncture analgesia was probably produces by the interaction of multiple brain structures of functional connectivity rather than through the activation of a single brain region.
2.The prevention and treatment of early stage postoperative inflammatory intestinal obstruction after laparoscopic and open radical resection of colorectal carcinoma

Objective To investigate the incidence, cause,prevention and treatment of early postoperative inflammatory intestinal obstruction(EPIIO)in patients undergoing laparoscopic and open radical resection of colorectal cancer. Methods From September 2000 to September 2006,483 cases of colorectal cancer were divided into two groups according the procedures received: laparoscopic surgery(232 cases)and open surgery(251 cases).The incidence of early postoperative intestinal obstruction was compared between the two groups. Results 1.In laparoscopic group,14 cases(5.69%,14/246)were converted to open surgery;2.The incidences of EPIIO in LS and OS group were respectively 3.02%(7/232)and 7.97%(20/251)(P<0.05,x2=5.60=.In LS group,6 EPIIO caseswere cured conservatively, one underwent surgery, while in open surgery group,18 cases recovered conservatively, two received surgery. Conclusions 1.Significantly less cases suffered from EPIIO in laparoscopic group.2.Most EPIIO cases will recover conservatively, in cases with repeated symptoms or when strangulation of bowel is suspected open surgery should be attempted.
3.Complications following laparoscopic and open radical resection of colorectal cancer

Colorectal cancer is a common malignancy in the digestive tract,open and laparoscopic surgery are the main treatment methods.Anastomotic fistula,anastomotic bleeding,ileus,voiding and sexual dysfunction are the common postoperative complications.Chylous fistula,incision infection,pulmonary infection and incision metastasis occasionally occurred.Compared with open surgery,laparoscopic surgery does not increase the postoperative complications,and it can be safely applied in the radical resection of colorectal cancer.
4. Expression of MMP-9 in brain tissue of mouse cryptococcal meningitis model

Objective: To explore the expression of MMP-9 in mouse brain tissue and its significance. Methods: Cryptococcosis ssuspension alone (fungal suspension group) or combined with aprotinin (aprotinin group) was injected in to female C57 BL/6 mice via femoral vein cannulation. The expression of MMP-9 was examined immunohistochemically 8 h later. Meanwhile, the mouse brain tissue homogenate colony count (CFU) was observed and the results were compared with that of the immunohistochemical examination. Results: The expression of MMP-9 was markedly increased in the fungal suspension group compared with the normal control group (P<0.01); the expression in the aprotinin group was not significantly different from that of the normal control group (P>0.05). The amount of cryptococcosis in the brain tissue was positively correlated with expression of MMP-9. Conclusion: MMP-9 is overexpressed in the brain of mice with Cryptococcal meningitis; MMP-9 might lead to increased permeability of blood brain barrier.

6. Activity detection of extracellular proteinase and serine protease secreted by Cryptococcus neo formans

Objective: To evaluate the activities of extracellular proteinase and serine protease secreted by Cryptococcus neoformans (C. neoformans) of different sources and serotypes and incubated under different conditions. Methods: Thirty-six C. neoformans strains of different sources and serotypes were incubated in protein agar medium at 30°C and 37°C separately. Activities of extracellular proteinase and serine protease were analyzed by determining Clearance Halos(CHs) produced by C. neoformans. Activity changes of the 2 proteases were analyzed after treated with serine protease inhibitors. The activity and the molecular weight of extracellular proteinase in the concentrated supernatant were measured using zymography assay. Results: Thirty-three of 36 C. neoformans strains produced specific Clearance Halos around the colonies, and their mean CHs at 30°C and 37°C were 0.558±0.170 and 0.575±0.169, respectively (P>0.05). The mean CHs of serotype A(n=13), B(n=13), and D/AD (n=6) strains were 0.564±0.144, 0.515±0.078 and 0.482±0.072, respectively (P>0.05); the mean CHs of clinical(n=23), environmental(n=9),and uncapsuled strains(n=4) were 0.570±0.177, 0.513±0.069, and 0.942±0.075, respectively (P<0.05). The mean CHs of control group (without protease inhibitor) and groups treated with protease inhibitor (1.2 mU and 1.6 mU) were 0.459±0.188, 0.975±0.287 and 0.733±0.252, respectively(P<0.01). Rich extracellular protease was found in the concentrated supernatant. Conclusion: The incubation temperature (30°C and 37°C) has no effect on activities of extracellular proteinase and serine protease, and the activities of clinical strains are significantly stronger than those of other 2 strains; the activities in different serotypes groups have no significant difference and can be inhibited by serine protease inhibitors.
7. Cell microarray in detection of serine protease expression in Cryptococcus neo formans

Objective: To determine the serine protease expression in Cryptococcus neoformans (C. neoformans) by cell microarray technique,so as to investigate the role of serine protease in the pathogenesis of C. neoformans. Methods: The cell microarray was constructed with tissue microarray. Thirty-six strains of C. neoformans of different sources and homologous serotypes were examined for their serine protease expression by cell microarray technique and immunohistochemistry. Results: Strong expressi on of serine protease was found in 25(67.0%) strains. The rates of strong serine protease expression in serotype A, B and D/AD strains were 46.2%(6/13), 92.3%(12/13) and 66.7%(4/6), and in environment-isolated, clinically isolated and uncapsuled strains were 55.6%(5/9),82.6%(19/23) and 25%(1/4), respectively. Serine protease expressions in serotype B and clinically-isolated strains were significantly higher than that in other group (P<0.05). Conclusion: Mic roarray of strain cells is a new method for identifying pathogenic fungus. Higher expression of serine protease in clinically-isolated strains is associated with strong virulence of clinical isolates strains, suggesting that serine protease plays a major role in the pathogenesis of C. neoformans.

9.Epidemiological Analysis of Leprosy in Zhejiang Province

Objective To analyze the epidemiological trends of leprosy and evaluate the effectiveness of control in Zhejiang Province. Methods A retrospective study was conducted. Results By the end of 1999, 16 262 leprosy patients had been registered, of them 11 935 had been cured and only 97 were still remained. In 1999, active cases decreased by 99.02% over 1973. The epidemiological trends of leprosy showed the increase of mean age at onset and type ratio, and the decrease of incidence in children. Conclusion The epidemicity of leprosy has been continuously dropping in Zhejiang Province due to the sustained effort and effective control.
10.Diagnosis of acute rejection by bile cytology and cytokine gene expression in rat liver transplantation model

Objective To evaluate a reliable and noninvasive method for the diagnosis of acute rejection after liver transplantation.Methods The allogeneic, syngeneic and immunosuppresive models of orthotopic rat liver transplantation were established, and acute rejection was graded according to histopathological change. Bile was sent for cytology by microscopy,and the sequential detection of bile IL 2,IFN ?,IL 4 and IL 6 gene expression was performed at day 1 through day 7. Results The number of cells in bile was persistently high in allogeneic group,and blast and lymphocytes appeared in bile. IL 2 and IFN ? were detected only in allogeneic group,with a specificity of 70% and 67%(7/10 and 8/12),and a sensitivity of 39% and 44%(7/18 and 8/18)respectively.Conclusion Bile cytology and detection of IL 2 and IFN ?mRNA transcript are useful adjuvant diagnosic methods for acute rejection after liver transplantation.
