Objective: To explore the impact of renal function injury on diagnostic value of NT-proBNP in patients with heart failure (HF).
Methods: A total of 420 patients with cardiovascular disease at (50-75) years of age were divided into 2 groups based on left ventricular ejection fraction (LVEF): Control group, the patients with normal cardiac function, LVEF≥40%,n=232 and HF group, LVEF<40%,n=188. According to estimated glomerular ifltration rate (eGFR), each group contained 4 subgroups by Normal renal function (eGFR≥90 ml/min·1.73m2), Mild renal injury (90>eGFR≥60 ml/min·1.73m2), Moderate renal injury (60>eGFR≥30 ml/min·1.73m2) and Severe renal injury (eGFR<30 ml/min·1.73m2). The changes of NT-proBNP level at different subgroups were observed and the optimal cut-off values of NT-proBNP for HF diagnosis were measured.
Results: Compared with Control group, HF group had increased blood level of NT-proBNP,P<0.05; NT-proBNP level was negatively related to eGFR (in all patients:r=-0.664, in Control group:r=-0.686 and in HF group:r=-0.721,P<0.05). Within Control group, NT-proBNP level was similar between Normal renal function and Mild renal injury subgroups,P>0.05, while it was much higher in Moderate and Severe renal injury subgroups than Normal renal function subgroup,P<0.05. Within HF group, Severe renal injury subgroup had increased NT-proBNP level than other subgroups,P<0.05. The best cut-off value of NT-proBNP for HF diagnosis in patients with normal or mild renal injury was 1070 pg/mL (sensitivity: 91.8% and speciifcity 72.6%); with moderate renal injury was 7121 pg/mL (sensitivity: 80.2% and speciifcity: 89.7%); with severe renal injury was 33344 pg/mL (sensitivity: 83.3% and speciifcity: 80%).
Conclusion: Moderate to severe renal function injury could increase circulating level of NT-proBNP and therefore, the cut-off value of NT-proBNP for HF diagnosis should be elevated accordingly in patients of HF combing renal injury.

In this paper we established a method of preparing chromosomes of peritoneal cells and made use of the method for observing the Y-chromosome of marrow and peritoneal cells of female mice which had been irradiated with 850 rad ~(60)Co-?, rays and then transplanted with marrow cells of male mice. The results showed that the chromosome karyotypes of the marrow cells of the female mice were of the XY type of male mice. It means that the marrow cells of the recipients were substituted by those of the donors. At the same time, the chromosome types of the peritoneal cells of the recipients were also of the XY type. It is suggested that the peritoneal cells were derived from the marrow cells.By the use of the method for assaying CFU-C and CFU-S of mouse marrow cells, CFU-C and CFU-S of peritoneal cells of mice were assayed. The result demonstrated that there were a number of CFU-C and CFU-S among the peritoneal cells. On the observation of the cell morphology of CFU-C and CFU-S, it was discovered that CFU-C differentiated mainly into mononuclear phagocytes, while CFUS had the ability to differentiate into erythrocytie series and granulocytic series, which implies that there were some multipotential stem ceils among peritoneal cells of mice. When more than 10~6 peritoneal cells per mouse were transplanted into the mice which had received 850 rad of ~(60)Co-? rays, their life span could be prolonged. It is still a question unsetteled that whether the stem cells in the peritoneal cavity differentiate diroctly into the mononuclear phagocytes or the mononuclear cells in the blood penetrate into the peritoneal cavity and then differentiate into the macrophages.

Chinese Journal of Postgraduates of Medicine 2017;40(2):117-120
Objective To investigate the association of residual renal function with intradialysis dialysis hypotension among maintenance hemodialysis patients. Methods Fifty-six patients who had underwent maintenance hemodialysis from March 2014 to November 2015 were enrolled according to researching protocol in this study. Data of baseline and clinical parameter of every hemodialysis session were collected. The patients were divided into two groups by 24 h urine output:urine group (24 h urine output≥100 ml, 22 patients)and no urine group (24 h urine output<100 ml, 34 patients). The clinical data were compared between two groups. Results In the 12 weeks’ treatment, 965 times hemodiafiltration were performed in 56 patients, and the rate of intradialysis hypotension was 24.04%(232/965). The rate of intradialysis hypotension in urine group was 19.28%(80/415), in no urine group was 27.64%(152/550), and there was significant difference (P=0.000). The levels of interdialytic weight gain, ultrafiltration volume, ultrafiltration percentage , pre-dialysis systolic and diastolic blood pressure in urine group were significantly lower than those in no urine group (P<0.01). The levels of pre-dialysis weight, intradialysis systolic blood pressure variability, intradialysis diastolic blood pressure variability, heart rate variability, hemoglobin, serum albumin in two groups had no significant differences (P>0.05). Pearson correlation analysis showed that residual glomerular filtration rate (rGFR) had negative correlation with interdialytic weight gain (r =- 0.257, P = 0.001). Kaplan-Meier analysis showed that with the increase of dialysis time, the rate of intradialysis hypotension increased. The comulative rate of intradialysis hypotension in no urine group was higher than that in urine group (log-rank = 14.67, P =0.000). Conclusions Residual renal function is associated with intradialysis hypotension, and it is important for clinician to consider a residual renal function protection schedule for maintenance hemodialysis patients.
